A suspected wrist fracture is assessed with an examination and usually X-rays; a normal first X-ray does not always rule out a break. Treatment depends on where the fracture is and whether the bone is stable and properly aligned. Some fractures need only a splint or cast, while others need realignment or surgery.

What counts as a wrist fracture?

“Wrist fracture” often means a break in the distal radius—the end of the radius closest to the wrist. The radius runs along the thumb side of the forearm. Other bones in and around the wrist, including the scaphoid, can also fracture, so the exact location matters. The American Academy of Orthopaedic Surgeons (AAOS) 2024 plain-language summary describes distal radius fractures and their evaluation and treatment.

How a suspected fracture is diagnosed

Examination and symptoms

A clinician will ask about the injury and assess pain and tenderness, swelling, bruising, and any visible change in the wrist’s shape. These signs can prompt evaluation, but they do not establish by themselves whether a bone is broken.

X-rays and other imaging

X-rays are generally the first imaging test. Several views may be taken to assess the bones from different angles. The American College of Radiology (ACR) describes a typical three-view wrist examination as posteroanterior, lateral, and 45-degree semipronated oblique views; it cautions that two views may be inadequate in acute hand and wrist trauma. A normal or unclear first X-ray does not always settle the question if the examination and symptoms still suggest a fracture. Some distal radius and scaphoid fractures may not be visible on initial radiographs. Depending on the findings, a clinician may request additional X-ray views, CT, or MRI, or arrange reassessment. The ACR lists these as possible next imaging options when initial radiographs are normal or equivocal but suspicion remains; the choice is a clinical decision, not something to determine from an image report alone. How treatment is chosen

The plan depends on the fracture’s position and stability, whether it affects the joint surface, and the person’s overall health, age, and activity demands, including work and hobbies. Age or activity alone does not determine treatment. The aim is to support healing while addressing any displacement or other features that make the fracture less stable.

Splint or cast

A fracture that is stable or has not shifted may be immobilized in a cast or splint. For an undisplaced distal radius fracture, Cambridge University Hospitals NHS Foundation Trust describes support with a removable splint or temporary cast and gives six weeks as an example for that specific injury. That duration is not a universal rule for all wrist fractures; follow the treating team’s instructions. See the Cambridge University Hospitals patient information on distal radius fracture.

Reduction to realign the bone

If bone fragments have shifted, a clinician may perform a reduction—a procedure to move them back into a better position. The treating team considers the fracture pattern and alignment when deciding whether reduction and subsequent immobilization are appropriate.

Surgery

Some fractures need surgery because of their alignment or other fracture features. A broken wrist does not automatically mean an operation: the AAOS describes treatment as individualized, and the available guidance does not establish one surgery threshold that applies to every patient. Ask the treating clinician how the fracture’s stability, joint involvement, health factors, and daily demands affect the recommendation. When to seek urgent medical advice

Seek prompt medical advice if wrist pain is severe, you cannot move the wrist or hold things, the wrist’s shape or colour has changed, or you have lost feeling in some or all of the hand. These are reasons for urgent assessment, not a way to diagnose the injury yourself. See NHS guidance on broken arms and wrists.

What to expect after assessment

The clinician will explain what the images show and whether the wrist needs immobilization, realignment, or a surgical opinion. If symptoms remain concerning despite an initially normal or unclear X-ray, ask what follow-up or further imaging is appropriate. Keep any cast or splint on for the period and in the way your treating team advises, and contact them if you have concerns about your symptoms or the fit of the immobilization.
