An irregular heart rhythm alert from an Apple Watch is a screening signal that may suggest atrial fibrillation (AF, also called AFib); it is not a diagnosis. If you receive one and have not previously been diagnosed with AF, Apple advises you to talk to a doctor. Seek emergency help for chest pain, pressure, tightness, or a suspected heart attack—do not wait for a watch reading or try to check an ECG first.
What does an irregular heart rhythm notification mean?
On Apple Watch, the irregular rhythm notification feature checks pulse readings from the watch’s optical sensor periodically for a pattern that may be consistent with AF. An alert means the feature found that pattern across multiple readings. It does not mean a clinician has confirmed AF, and Apple says the feature can sometimes indicate an irregular rhythm other than AF.
The feature is a screening prompt, not a diagnostic test or a guide to treatment. FDA-hosted labeling for Apple’s feature says it analyzes pulse-rate data opportunistically when sufficient data are available and that readings are captured when the wearer is still. The FDA-hosted labeling also states that “the absence of a notification is not intended to indicate no disease process is present.”
Why might the watch alert—or not alert?
It checks periodically, not continuously
The feature is not constantly monitoring for AF. It samples in the background when conditions allow, usually while you are still; the number and spacing of readings can vary with activity. As a result, an episode may not be captured, and you may not receive an alert even if you have AF or another health issue.

No alert does not rule out a problem
A lack of notification is not a clean bill of health. If you have symptoms such as a rapid, pounding, or fluttering heartbeat, dizziness, or fainting, Apple advises talking to a doctor even if the watch has not alerted you. Do not wait for another notification to seek advice about symptoms.
What should you do after an Apple Watch alert?
If you have not been diagnosed with AF
Contact a doctor to discuss the alert. It is a reason to follow up, not proof that you have AF. A clinician can consider your symptoms and medical history and decide whether further assessment is appropriate.

If you already have an AF diagnosis
Apple says irregular rhythm notifications are not designed for people already diagnosed with AF. Apple’s separate AF History feature is intended for diagnosed users and presents retrospective estimates; it is not an alert during an AF episode. Ask your clinician how to monitor your condition rather than treating an irregular rhythm notification as a management tool.
Do not change medication based on the watch
Do not start, stop, or change medication because of an alert or an ECG-app classification without discussing it with a doctor. Apple specifically cautions users not to change medication without talking to a doctor.

When is an irregular rhythm alert an emergency?
Apple states that “Apple Watch cannot detect heart attacks.” If you have chest pain, pressure, or tightness, or think you may be having a heart attack, call emergency services. Do not delay care to take a watch ECG or wait for another notification. Apple’s ECG guidance also says the app cannot detect heart attacks, blood clots, stroke, or several other heart-related conditions.
How is the ECG app different from an irregular rhythm alert?
The two Apple Watch features use different signals and work differently. The irregular rhythm notification samples pulse data in the background; the ECG app records an electrical signal when you start a recording. Neither feature can diagnose every heart problem.

What a watch ECG can—and cannot—tell you
Apple describes the ECG app as similar to a single-lead ECG; a standard office ECG usually uses 12 leads. A sinus-rhythm result describes only the 30-second recording you took. It does not establish that you are healthy, that your rhythm is always normal, or that an earlier irregular rhythm alert was incorrect. An AF result in someone without a prior AF diagnosis should be discussed with a doctor. The app cannot identify all other cardiac conditions.
Taking an ECG recording
On supported Apple Watch models, rest your arms and touch the Digital Crown with a finger from your opposite hand; the recording takes 30 seconds. Apple says you can use the ECG app when symptoms occur or after an irregular rhythm notification. It is not a substitute for urgent care when emergency symptoms are present.

Who can use Apple’s heart rhythm features?
Apple’s support guidance says irregular rhythm notifications are available only in certain countries and regions, are not intended for people under 22, and are not designed for people already diagnosed with AF. The feature also requires current software and setup, and AF History must not be enabled for irregular rhythm notifications. Availability and eligibility can vary, so check Apple’s current instructions for your location and device.
Apple says the ECG app is available on Apple Watch Series 4 or later and all Apple Watch Ultra models, subject to regional availability, and is not intended for people under 22. These are Apple-specific requirements; they should not be assumed to apply to another wearable brand.
What if the alert came from a different wearable?
The guidance above describes Apple Watch. Other manufacturers may use different sensors, alert thresholds, eligibility rules, and instructions. Check the official guidance for the exact make and model of your device, and treat its notification as a reason to follow the manufacturer’s recommended medical follow-up—not as a confirmed diagnosis.
