Find the point where the fact disappears before changing settings: check whether the app saved it, searched the right memory scope, retrieved it, passed it into the model’s context, and then used it correctly. A missing fact in storage points to a write problem; a stored fact that never appears in results points to scope or retrieval; a retrieved fact missing from the prompt points to context handling. A fact present in context but contradicted may need correction—or may expose a model reasoning problem.

First, what does “memory” mean in your app?

Local AI apps may use “memory” to mean different things: a saved preference or fact from conversation, a stored chat, or information retrieved from documents. These features may have separate storage, search, and repair controls, so identify which one you expected to work before troubleshooting.

Document retrieval is especially easy to confuse with conversational memory. AnythingLLM, for example, distinguishes documents attached to a chat thread from documents embedded in a workspace; which one is available depends on how the document was added. See AnythingLLM’s document guide. Note the app and version, model, the fact you expected it to recall, and where you originally saved or uploaded it.

Where in the memory chain did the fact disappear?

Trace the fact through the stages below. The symptoms are clues, not proof: apps implement memory differently, and a failure in one stage can look like a failure in another.

What you find Likely area to check
The fact is absent from the app’s memory store. Memory settings, permissions, or the model’s write/extraction operation.
The fact is stored, but does not appear in search results. User, agent, session, workspace, or other scope filters; then retrieval and indexing.
The fact appears in retrieved results but not in the model’s context. Tool-call handling or context assembly and limits.
The fact is in context, but the answer conflicts with it. Outdated or duplicate facts, correction handling, or the model’s reasoning.

Use the app’s memory viewer or search tools where available. Open WebUI documents list_memories and search_memories; Mem0 documents get_all and search. Their interfaces differ, so consult the documentation for the installed version: Open WebUI memory documentation and Mem0 API documentation.

How do you troubleshoot each failure point?

1. Check whether the app saved the fact

Search for a distinctive phrase, name, project, or exact identifier. If the fact is missing, verify that memory is enabled and that the app or model actually performed a write. A model may be able to answer a question without saving the answer as durable memory.

In Open WebUI, check the administrator’s global Memories setting and the user’s permissions. For the selected model, the documentation also calls for Native Function Calling and the Memory category to be enabled. Open WebUI cautions that smaller local models may manage memory inconsistently, so a failed write can be a model/tool-calling issue rather than a search issue. 2. Check that the search uses the same identity and scope

A fact can be present but invisible to a query made under a different user, agent, run, session, app, workspace, or path. Compare the identifiers used when the fact was written with those used when it is searched. Mem0’s APIs support scope filters and its documentation recommends using identifiers such as user_id, agent_id, or run_id where appropriate. AnythingLLM’s thread and workspace document scopes are another example of why the selected feature matters.

3. Test retrieval with both exact terms and a paraphrase

Try the exact name or ID as well as a conceptual paraphrase. Keyword matching can help with precise terms; semantic retrieval is intended to find related meaning, not guarantee an exact lookup. Mem0 describes semantic, keyword, entity, and temporal signals. In a system that exposes results or scores, inspect which candidates were returned and whether the relevant one was filtered out.

If the app offers a similarity threshold, lowering it may surface borderline matches, but it can also add irrelevant results. Change one setting at a time and judge both whether the desired fact appears and whether the returned material remains relevant. Retrieval behavior depends on the configured vector store and, where used, reranker; Mem0’s self-hosted setup is not identical to hosted features. 4. For documents, verify ingestion, index, and embeddings

Confirm the file finished ingestion and is present in the intended knowledge base or workspace. If you changed the embedding model, existing documents may need to be reindexed before retrieval works as expected. Open WebUI also documents missing per-file indexes after vector database migration, partial reset, or direct deletion; its suggested recovery is to reindex the knowledge base or re-upload a file outside one. The steps depend on the installation, so use the version-matched Open WebUI RAG troubleshooting guide.

Re-embedding can take time and may incur charges if the embedding model is paid. Do not assume that changing a model automatically rebuilds the old index.

5. Check whether retrieved material reached the model

A search result is not useful if it is omitted from the prompt the model receives. Inspect the assembled context or context indicator if the app exposes one. Open WebUI says it trims retrieved content to fit its assumed context capacity. AnythingLLM warns that continuing with full text after exceeding the context window can lead to pruning and inaccurate behavior. If relevant content is being crowded out, reduce competing context or use the app’s intended retrieval mode.

There is also a narrower Open WebUI failure to consider when a memory tool call seems to vanish: its documentation says an endpoint that omits the streamed tool-call index field can cause a call to be dropped and yield a blank assistant response. This is a documented Open WebUI/connection compatibility issue, not a general explanation for memory failures. Check the Open WebUI compatibility notes if those symptoms match.

6. Replace obsolete or conflicting memories explicitly

If the wrong fact is actually stored, inspect it before changing retrieval settings. Open WebUI supports updating, replacing by memory ID, and deleting memories. Mem0 explains that its automatic extraction path is additive: a new fact may not silently overwrite an older one. Update or delete obsolete entries and check for duplicates. How can you confirm the fix?

Retest with one harmless fact that should remain useful. This controlled check helps separate a successful write from successful retrieval and use; it is a troubleshooting method, not a procedure every app exposes in the same way.

  1. Add a single durable fact using the app’s normal memory feature.
  2. Verify that the fact appears in the memory store or viewer, if available.
  3. Ask a direct question about it, then ask a paraphrased question.
  4. Inspect search results or the assembled context if the app exposes them.
  5. Change or correct the fact, then check whether the obsolete version is gone and the updated version is used.

If the test fails, return to the first stage where the fact went missing rather than changing several settings at once.

What should you compare when choosing a local-memory setup?

There is no universally best configuration established by the cited documentation. Compare the trade-offs that affect your use case:

  • Data location: whether memory and retrieval remain local or call hosted services.
  • Search needs: exact names and identifiers versus conceptual or time-sensitive questions.
  • Model capability: whether the chosen local model reliably extracts facts and issues structured tool calls.
  • Index maintenance: which embedding model is in use and whether changing it requires rebuilding existing indexes.
  • Scope: how the app separates users, workspaces, agents, sessions, threads, or runs.
  • Context budget: how much retrieved text can reach the model and what the app trims when the limit is reached.
  • Operational complexity: whether the setup requires managing a vector store, reranker, or other retrieval components.

Mem0 documents local Ollama model and embedder configurations alongside several vector-store choices; Open WebUI’s RAG guide lists local embedding options and recommends reindexing after embedding-model changes. These are configuration options, not an independent ranking of memory quality. Consult the official documentation for your installed app and version, especially for model-tier guidance and endpoint compatibility.
