Your running shoes may be contributing to foot or knee pain if symptoms started after a shoe change, the pair rubs or presses on your feet, or the soles show uneven wear. Those clues do not prove the shoes caused the pain: a sudden increase in distance or speed, hills, a new surface, and injury can also be factors. Check the fit and wear, stop or modify running that hurts, and seek medical advice for severe, persistent, or worsening symptoms.

Could my running shoes be causing my foot pain?

Possibly, especially if the pain began soon after switching shoes, the shoe pinches or rubs, or an older pair is visibly worn. But timing and shoe condition are clues, not a diagnosis. Foot pain can also follow a sudden increase in running, more uphill running, or other training changes.

Heel or sole pain and pain around the ball of the foot are among the problems runners may notice. The NHS describes heel pain as potentially associated with doing much more running suddenly, running uphill, or wearing shoes that are worn or insufficiently supportive. The American Orthopaedic Society for Sports Medicine (AOSSM) describes gradual, low-grade heel pain with plantar fasciitis and lists forefoot pain among common running problems. These descriptions cannot establish that footwear is the cause.

Could my running shoes be causing my knee pain?

A shoe change can be one possible contributor, but knee pain has many causes. AOSSM lists increased mileage, a terrain change, and a running-shoe change among possible factors for patellofemoral pain, which is centered around the kneecap. Hospital for Special Surgery describes patellar tendon pain below the kneecap and iliotibial (IT) band pain often on the outside of the knee. Rapid training increases or returning to running can also accompany knee pain. Pain location alone cannot identify the shoe as the cause.

Check the timeline before blaming a pair

Write down when the pain first appeared and what changed around that time. Include the pair you wore, when you switched models or shoe types, roughly how much you have used the pair, and any change in weekly distance, pace, hills, surface, or return-to-running schedule. A close timing link makes the shoes worth inspecting, but does not prove causation. AOSSM identifies training errors such as rapid mileage changes, hill running, and interval training as common injury sources.

Inspect fit and wear

Check how the shoes feel

Pay attention to pressure, rubbing, or a heel that shifts. Comfort and fit are individual; a shoe that suits someone else may not suit your feet.

Look at both shoes

Set the pair on a flat surface and compare them. Check for uneven wear on the soles and creasing in the midsoles. These are reasons to inspect the shoes, not proof that they caused pain. Cushioning and support materials can wear down without obvious external signs, according to Northwestern Medicine. Nationwide Children’s Hospital also notes uneven wear and midsole creasing as replacement clues, while cautioning that aches cannot all be attributed to shoe wear.

Use mileage as a rough guide, not a cutoff

Actual wear varies with use, running surface, body weight, and shoe design. Do not treat a mileage figure, visible wear, or a new ache by itself as a diagnosis.

What to do if you suspect a shoe change

  1. Stop or modify running that causes pain. Do not keep running to compare shoes or provoke symptoms.
  2. Review the timeline and inspect fit and wear. Note footwear and training changes together, rather than assuming one explains the other.
  3. Avoid changing several training variables at once. If you return to a previously comfortable pair, do so only if it is safe and you can run without pain. Feeling better in it may be informative, but this is not a validated diagnostic test.
  4. Get assessed when symptoms warrant it. Repeatedly switching shoes is not a substitute for medical evaluation of persistent, severe, or worsening pain.

There is no established home test that proves a particular shoe caused an individual runner’s pain. Evidence also does not support confidently predicting that a specific cushioning, stability, or minimalist feature will prevent or cause an individual injury. A 2022 review discusses uncertainty in footwear-injury paradigms and notes that evidence on upper components is very limited, with more research needed on insole and outsole effects. FootCareMD says data do not establish whether cushioned or minimalist shoes are better.

When to stop running and seek medical care

  • Knee pain: NHS guidance says not to run with knee pain and to consult a GP or physiotherapist if it remains after a week of rest.
  • Heel or sole pain: Seek assessment if it does not go away; substantial swelling of the heel or sole calls for prompt care.
  • Severe symptoms: AOSSM recommends immediate medical treatment for severe pain, swelling, loss of motion, or a change in running form.

For fit context, Northwestern Medicine sports-medicine physician Steven E. Mayer, MD, says: “Your foot structure, the way you move and the activities you do all play a role in determining which shoe will feel best and help keep you comfortable.”
