You cannot reliably prove at home that an odor is a phantom smell. Ask someone else whether they notice it and check whether it changes when you move to another location, but treat those observations only as clues—not as a diagnosis or proof that there is no hazard. If gas, smoke, chemicals, or another danger could be involved, follow local safety guidance and move to safety rather than waiting to see whether someone else smells it.

What “phantom smell” means

Phantosmia is the perception of an odor when no odor source is present. The sensation is real to the person experiencing it; the term does not mean the person is imagining it or being untruthful. Smells may be pleasant or unpleasant, come and go or persist, and be perceived in one nostril or both. The odor’s character alone does not reveal its cause. NIDCD and Cleveland Clinic describe phantosmia as a smell disorder.

These terms describe different changes in smell perception, not a way to diagnose yourself. NIDCD distinguishes phantosmia from parosmia and smell loss.

What you can check at home—and what it cannot tell you

You can gather observations that may help explain the experience, but no reviewed clinical source describes a reliable home test that confirms phantosmia.

  • Ask another person whether they notice the odor. If they do, that is evidence of a possible environmental source. If they do not, it does not prove the odor is phantom: people differ in smell sensitivity, and an odor may be localized or intermittent.
  • Notice whether it follows you between locations. An odor that persists across rooms or settings may be less tied to one obvious local source, but this is only a clue, not a validated test.
  • Record when it occurs and how long it lasts. Note whether it is constant or episodic and whether it changes with location or circumstances. These details can help a clinician assess the problem.

Do not use your nose—or another person’s nose—as the sole safety check. The NHS warns that smell loss can make it harder to detect gas leaks, fires, or spoiled food. If a gas leak, fire, chemical release, or poisoning is plausible, follow local emergency or utility instructions and get to safety as appropriate. Do not stay in a potentially dangerous place to test the smell.

Why phantom smells happen

Possible causes include recent respiratory or sinus illness, head injury, aging, some medicines, and neurological conditions. Mayo Clinic also lists temporal lobe seizures, inflamed sinuses, brain tumors, Parkinson’s disease, and COVID-19 among possible causes of phantosmia. NIDCD discusses a wider range of contributors to smell disorders, including nasal growths, smoking, hormonal changes, dental problems, chemical exposures, and radiation treatment. NHS Scotland clinician guidance says isolated phantosmia without focal neurological symptoms or signs does not, by itself, call for neurological investigation or treatment. When to contact a healthcare professional

Seek medical advice if a smell change persists or concerns you. The NHS advises seeing a GP if your sense of smell has not returned to normal in a few weeks. Cleveland Clinic advises contacting a provider for phantom smells lasting more than three weeks. These are recommendations from different sources, not one universal cutoff: you do not need to wait for either threshold if you are worried or have other symptoms. NHS; Cleveland Clinic.

What a clinician may do

A clinician may ask about the odor, how long episodes last, whether one or both nostrils are involved, and relevant health history, such as recent illness, injury, or exposures. Assessment can include an ear, nose, and throat examination and a smell test supervised by a health professional. Depending on the findings, a clinician may consider nasal endoscopy or CT or MRI imaging; these tests are not required for everyone. See NIDCD, NIH, and Cleveland Clinic.

  • When the episodes began, how often they happen, and how long they last
  • Whether you notice the odor in one nostril or both
  • Whether it changes with location or situation
