Set an incident alert threshold so it signals a condition that warrants the response attached to it—not merely a metric that has moved. Page a person promptly when customer or business impact calls for immediate action. For every alert, define its meaning, owner, severity, first response, runbook, backup coverage, and escalation conditions and timing. There is no universal threshold or timeout: choose values for the workload, then validate them against incidents and drills.

1. Start with service impact, not a convenient metric

Map the service before choosing alert rules. Identify critical features, user-visible journeys, dependencies, and the customer groups affected if each part fails. A metric is useful for paging when it helps identify meaningful impact or gives responders a timely signal to act.

Consider latency, errors, dependency health, and discrepancies between client-side and server-side measurements. A server metric can appear healthy while users experience a problem, or a dependency can fail in a way that affects only one feature. AWS’s guidance on preparing for incidents also calls out feature criticality and how commonly a system fails as monitoring considerations.

2. Choose a threshold and duration together

A threshold answers how far a signal must move; a duration answers how long the condition must persist. Set both using observed behavior, service objectives, and the impact you can tolerate. The right duration depends on the signal: a brief fluctuation may not warrant human intervention, while a sustained or rapidly worsening condition may.

AWS’s alarm guidance says thresholds and durations should be chosen so that every firing alarm requires investigation. That is a useful test for any paging rule: if responders would routinely ignore it, it is not a good page in its current form. The guidance does not prescribe a general percentage, threshold, or duration that applies across workloads. See AWS Incident Detection and Response alarm criteria for criteria specific to that managed service’s workload onboarding.

3. Define severity by impact and urgency

Write down what each severity means in terms responders can apply consistently. Describe the affected scope, customer or business consequences, and whether action is needed now, soon, or preventively. Avoid labels such as “high” without an operational definition.

AWS Incident Manager provides illustrative impact levels ranging from critical, broad application failure to limited intermittent failures and urgent preventive action before customers are affected. These are examples, not a required severity scale; teams should define their own scopes. AWS’s incident lifecycle guidance describes impact levels and response phases.

Reserve an urgent page for significant customer or business impact that needs immediate operator attention—for example, a loss of revenue or degraded customer experience. A preventive alert can still matter, but if customers are not yet affected, its urgency and response may differ from an active outage. The rule should make that distinction clear.

4. Attach a response to every alert

An alert should tell its owner what happened and what to do next. AWS Well-Architected states: “Establishing a clear and defined process for each alert in your system is essential for effective and efficient incident management.” Its OPS10-BP02 guidance identifies missing response processes, unclear ownership, and overload as anti-patterns.

For each alert, document:

  • Meaning and impact: what condition fired, which service or feature is implicated, and what impact is known or suspected.
  • Accountable owner: the team or role responsible for investigating and resolving it.
  • First action: a specific diagnostic check or safe mitigation, not simply “investigate.”
  • Runbook: the current procedure or knowledge base, including required permissions and relevant dashboards.
  • Next contact: who to engage if the first responder cannot resolve the condition or needs authority or expertise.

Group related alarms when doing so reduces noise and yields a more meaningful response. Automate repeatable actions where practical, but limit automation to actions whose safety and authorization are understood.

5. Make escalation a timed, explicit path

Escalation rules should specify what advances an incident, who is engaged at each stage, how they are contacted, and what happens if nobody acknowledges the page. Include backup responders: a primary contact may be unavailable, and an unacknowledged alert needs a defined next step.

Document the path in operational terms:

  1. Trigger: state the condition for escalation, such as worsening impact, a failed mitigation, or no acknowledgment within the team’s chosen window.
  2. Order and roles: name the primary responder, backup, incident lead or specialist roles, and the order in which they are engaged.
  3. Channels and timing: list the contact method and acknowledgment window for each stage, including the next action when that window expires.
  4. External dependencies: record vendor contacts and escalation details where a third party may need to act.
  5. Authority to mitigate: identify safe, pre-approved actions and who can authorize actions outside that scope.

AWS’s onboarding questionnaire includes a ten-minute interval as an example field value; it is not a general recommendation or industry standard. Select acknowledgment windows and escalation timing based on impact, coverage, and how quickly action must occur. In AWS Incident Detection and Response’s managed-service onboarding context, alarms must engage workload resolvers at the same time as, or before, that service’s engagement. 6. Validate the rules and improve them after use

Test the whole chain, not just whether a monitoring system can emit a notification. Rehearse primary and backup contacts, acknowledgment timeouts, runbook access, communication routes, and any pre-approved mitigation. AWS recommends drills and game days as ways to exercise escalation paths.

After incidents and exercises, review whether pages matched genuine impact, whether noise or flapping created unnecessary engagement, whether an earlier signal was missed, and whether the assigned responder could act. Update thresholds, durations, response plans, and runbooks based on what happened. Remove or retune alerts that do not lead to useful investigation; add or adjust signals when they would have exposed impact sooner. AWS’s per-alert process guidance and incident lifecycle guidance both support regular review and post-incident refinement.

A practical review checklist

  • Does the signal connect to a critical feature, customer journey, dependency, or business outcome?
  • Does the threshold and duration indicate a condition that warrants the stated response?
  • Is the severity tied to concrete impact and urgency rather than an undefined label?
  • Is there an accountable owner, an actionable first step, and a usable runbook?
  • Are primary and backup responders, contact channels, escalation triggers, and acknowledgment windows explicit?
  • Can responders safely perform the documented mitigation, and are vendor steps clear where needed?
  • Have drills and incident reviews tested whether the alert is timely, actionable, and appropriately routed?
