Can you undo an AI agent’s completed action?
Sometimes, but stopping the agent is not the same as rolling back its work. OpenAI explains that an action’s reversibility depends on the action and the app involved; it gives document edits and email recall as examples of possible correction, while noting that some actions cannot be undone. Monitoring may identify a concern after an action has already completed. OpenAI’s guidance is explicit: “A stopped request does not undo earlier actions.” Treat stopping as containment, then investigate and recover through the affected application’s own controls where possible. OpenAI’s misalignment monitoring documentation also cautions that alerts are not a complete audit history.
What to do after an unintended action

Stop further actions and retries
Stop dispatching actions for the affected task or conversation. Do not automatically retry an action whose status is blocked or uncertain: repeating a tool call could repeat its side effect. Review any safety alert or error and the agent’s recent activity before proceeding.

Reconstruct what happened
Gather the request and response IDs, tool-call names and arguments, outputs, approval decisions, and relevant records from the connected app. Compare the sequence with the user’s original request. Monitoring alerts can help identify a concern, but application-level records are important for establishing what actually changed.

Check the current state before changing it again
Determine whether the operation is pending, completed, or externally visible, and inspect the current app state. Consult the app’s current recovery instructions for that specific action. Do not assume that issuing the opposite-sounding command will reverse the original effect: the app may have changed since then, or someone may already have seen or acted on the result.

Use the app’s recovery control, then verify
If the app offers an undo, restore, recall, cancellation, or other recovery path for the specific action, follow its instructions and confirm the resulting state. If no such control exists, consider whether a separate corrective action is appropriate. Have a person review that correction when it affects people, money, access, or information shared outside the intended scope.

Resume only after confirming a safe state
Check the app state and adjust the permissions, task instructions, or approval boundaries that contributed to the incident. OpenAI’s monitoring guidance does not establish a general method for resuming a conversation stopped by monitoring; if the prior workflow cannot be safely continued, use a new one with corrected safeguards.

How to prevent unintended actions
Put human approval before consequential tool calls
In the OpenAI Agents SDK human-in-the-loop workflow, a tool can require approval. The run pauses for a human decision, and the SDK supports resuming a serialized paused run after approval or rejection. The review should cover the actual operation and its arguments, not merely the general task. The documented approval flow fails closed when a callback cannot safely inspect malformed or unusable arguments.
Review high-risk or ambiguous changes; stop safely if review is unavailable
OpenAI’s API cybersecurity guidance recommends review for ambiguous or high-risk changes, audit records, and failing closed when review is unavailable. A fail-closed design does not proceed with the consequential action when the required check cannot be completed.

Limit tool permissions at runtime
Constrain an agent to the tools and operations needed for its task. Google Cloud’s agent governance documentation describes runtime policies for enforcing business rules and preventing unsafe combinations of tools. Narrower permissions reduce the scope of actions an agent can take if its behavior goes wrong.
Define boundaries and preserve traceable records
Set out which actions are allowed, which are denied, and when the agent must escalate to a person. Anthropic’s agent implementation workflow guidance recommends defining the resulting blast radius and using traceable identifiers that connect actions to agent instances during investigation. Keep enough records to link a request, tool call, approval, and resulting app state.

Use monitoring as detection, not rollback
Monitoring can miss issues or flag legitimate activity, and an alert can arrive after an action completes. OpenAI recommends continuing to use application safeguards, including human approval for consequential actions. Monitoring adds a detection layer; it does not replace permissions, approval gates, or app-specific recovery controls.

There is no universal undo method established across the platforms discussed here. Evaluate the app’s recovery option and the agent’s controls against the operation you need to protect:
- Reversibility: Can the completed operation itself be undone, or can you only make a separate correction?
- Timing: Is recovery available only before the external effect completes, or after it as well?
- Approval placement: Can a person inspect the operation and its arguments before execution and understand its likely consequences?
- Traceability: Can an operator connect the request, agent, tool call, approval, and resulting app state in records?
- Scope and failure behavior: Are permissions limited to the intended operation, and does the system stop safely if required review is unavailable?
The exact recovery steps depend on the connected app and the action taken. Check that app’s current documentation and verify the state before allowing the agent to act again.
