Prevent pitcher’s-hand blisters by reducing repeated seam friction: keep nail edges smooth, maintain calluses conservatively, manage moisture, and stop to inspect skin when discomfort or redness begins. If a blister forms, protect it and keep it clean; do not assume tape or a bandage can stay on the hand during a game. Playing rules differ by league, and MLB prohibits pitchers from pitching with an attachment such as tape or a bandage on either hand or wrist.

Why pitchers get blisters—and where they usually form

A blister develops when repeated friction and pressure separate layers of skin. For pitchers, the fingers repeatedly grip and release the ball against its seams. Professional baseball athletic trainers identify the middle finger as the most common site, followed by the index finger and, less often, the thumb. A 2016 review of MLB pitchers also describes frequent blisters at the tips of the index and long fingers.

Moisture can make the friction worse. Patrick Wesley and Mark Shires, athletic trainers with the Baltimore Orioles, note that friction tends to increase as the hand becomes clammy or damp. Jagged nail edges and uneven calluses may also add irritation. The pitcher-specific guidance describes causes and professional practice; it does not establish one prevention method as superior in controlled comparisons.

How to reduce the chance of a blister

Check nail edges and calluses

  • Inspect the fingertips that contact the seams, particularly the middle and index fingers, and check for tenderness, redness, rough skin, or a sharp nail edge.
  • Keep nails at a normal length and smooth jagged edges with a fine file. The baseball athletic-trainer guidance recommends light filing as needed.
  • Keep calluses even, but do not treat aggressive shaving or filing as a required home procedure. Callus maintenance is discussed in professional team care, not as a consumer technique with a specified safe protocol.

Manage moisture without affecting the ball or grip

Pitcher-specific athletic-trainer guidance discusses team- or clinician-approved antiperspirants and drying agents to reduce moisture buildup. Do not apply an arbitrary substance to the pitching hand or ball: the substance may affect grip, and the applicable competition rules may restrict its use. Follow team medical guidance and check league rules.

Respond early to discomfort

At the first localized pain, discomfort, or redness, stop the activity and inspect the skin rather than continuing to add friction to the area. The American Academy of Dermatology (AAD) advises stopping an activity when pain or redness appears in a friction-prone spot. Pads, gels, or sprays may be considered for recurring friction areas under general sports-dermatology guidance, but that does not establish that they are allowed on a pitcher’s hand during competition.

How to care for a blister

If the blister is intact

For an ordinary small blister, leave the skin roof intact when possible, keep the area clean, and cover it loosely with a clean bandage. The roof helps protect against infection. Avoid routinely popping or draining the blister. MedlinePlus notes that draining is generally avoided, except when a blister is large, painful, or likely to burst on its own.

If it has drained or burst

Wash the area with soap and water, apply petroleum jelly, and cover it with a bandage. Do not peel away the remaining roof of skin. Avoid the friction that caused the blister while it heals.

If it is painful or affects throwing

A pitcher should involve an athletic trainer or clinician if pain, an open area, or a change in grip is affecting throwing. A 2025 article from the Professional Baseball Athletic Trainers Society describes team care that may include sterile drainage and surgical glue, with attention to infection control and notification of officials about interventions or substances. That is a description of professional team practice—not instructions for draining a blister at home.

When to get medical advice

Seek prompt clinical assessment for pus, spreading or increasing redness, warmth, swelling, worsening pain, or fever. Get advice as well for multiple unexplained blisters or if the person has diabetes or circulation problems. These signs and conditions are identified in MedlinePlus guidance; AAD sports guidance recommends consulting a dermatologist or sports-medicine doctor for concerning skin symptoms.

When can a pitcher return to throwing?

The cited medical and baseball sources do not establish a universal number of rest days for pitchers. AAD says most blisters heal on their own in one to two weeks, but that general estimate is not a pitcher-specific clearance timeline. Return depends on the wound’s condition, pain, and whether the pitcher can grip and throw without aggravating the area. A clinician or athletic trainer can assess those factors; do not use a fixed timetable as a substitute for that assessment.

Can you tape a blister on the pitching hand during a game?

Check the rules for the athlete’s league and level before putting an attachment or substance on the pitching hand. MLB’s rule glossary says a pitcher may not pitch with an attachment such as a bandage, tape, or bracelet on either hand or wrist. It also says a pitcher who contacts the mouth must wipe the pitching hand dry before touching the ball or pitching rubber.

The 2023–24 NCAA rulebook says a pitcher must not use a bandage or other distracting item on the pitching hand, wrist, or fingers. Its note allows an athletic trainer to use a substance to stop bleeding if it dries sufficiently before play resumes. This wording is specific to that edition; confirm the current rule and umpire interpretation for the athlete’s level. Do not assume MLB’s wording or an NCAA exception applies in another league.

What the available injury figures do—and do not—show

A 2016 review by McNamara, Ensell, and Farley reported that six MLB players accounted for seven blister-related disabled-list stints since 2010, totaling 151 days on the disabled list; it also reported eight minor-league players spending time on the disabled list due to blisters since 2012. These are historical counts from that review, not current injury rates or an estimate of an individual pitcher’s risk. The cited sources do not provide a broader current incidence figure.
