Nausea, vomiting, diarrhoea and constipation are common digestive side effects of GLP-1 medicines. Smaller meals, bland foods and sensible fluid intake may help, but persistent or severe symptoms need medical advice. Do not change or stop your prescribed medicine without guidance from your prescriber or pharmacist.
Which digestive side effects can GLP-1 medicines cause?
Nausea, vomiting, diarrhoea and constipation are among the common gastrointestinal effects reported with GLP-1 medicines. The UK Medicines and Healthcare products Regulatory Agency (MHRA) says these effects may affect more than 1 in 10 patients and can last several days; that is not a precise rate for every medicine or patient group. They are more likely near the start of treatment or after a dose increase. The MHRA explains that these medicines slow digestion, which is consistent with effects such as nausea and changes in bowel habits. MHRA guidance covers UK-approved uses and safety advice.
Symptoms and instructions vary by medicine, formulation and approved use. For example, NHS semaglutide information applies to semaglutide, not automatically to every GLP-1 medicine. Read the current leaflet for your prescription and ask your clinician or pharmacist about advice specific to it.

Guy’s and St Thomas’ NHS Foundation Trust advises that nausea, heartburn and vomiting often settle within 3 to 4 weeks after starting treatment or increasing a dose. This is general patient guidance, not a guaranteed timeline. The trust also notes that effects may feel more intense during the first six months and just after a dose. Its dietary advice suggests trying the following if they suit you:

- Eat small meals more often rather than large meals.
- Try bland, dry foods such as crackers, or plain foods such as dry toast, porridge, rice, pasta or potatoes.
- If it feels better, choose cold foods over hot foods.
- Other plain options the trust lists include eggs, plain yogurt, tofu, bananas, cucumber, melon and steamed vegetables.
- Drink fluids regularly. Avoid fatty or strong-smelling foods if they make symptoms worse, and do not lie down soon after eating.
These are options to try, not a prescribed diet or a guarantee of relief. If symptoms are persistent or worsening, ask your care team rather than trying to manage them through food changes alone.
What may help with constipation or diarrhoea?
Constipation
Guy’s and St Thomas’ suggests drinking fluids, adding fibre from fruit, vegetables, whole grains, beans or lentils, and moving when able—for example, taking a short walk. It also lists kiwi fruit, chia or flax seeds, and dried fruit as food options. People respond differently, so worsening constipation should be discussed with your care team.

Diarrhoea
The trust recommends plenty of low-calorie fluids and smaller, more frequent meals, and says diarrhoea often improves as the body adjusts. Diarrhoea can also contribute to dehydration, so fluid intake is not a substitute for medical help if you cannot keep fluids down or develop warning signs.

Contact your GP or care team
Contact a GP or care team if side effects worsen. Guy’s and St Thomas’ specifically advises seeking help if you vomit more than three times a day, cannot keep fluids down, have dark urine or feel lightheaded. These can be signs of dehydration or that the medicine may not be suitable for you.

Get urgent medical help for warning signs
- Severe stomach pain that radiates to your back and does not go away can be a warning sign of pancreatitis. Seek urgent medical help.
- Sudden or rapidly deteriorating vision while taking semaglutide needs urgent medical contact under MHRA guidance.
- If you suspect a serious side effect, contact an appropriate health service urgently.
The MHRA’s UK guidance describes gastrointestinal symptoms that can sometimes lead to severe dehydration and identifies these urgent warning signs.
Should you change or stop your dose?
Do not stop, skip, reduce or retime a dose as a general self-care measure. The NHS semaglutide page says, “Keep taking your medicine unless you’re advised to stop,” and advises speaking to a pharmacist or doctor about bothersome or persistent effects. That instruction is specific to NHS information for semaglutide; for any medicine, follow its leaflet and ask your prescriber or pharmacist before making a change. Medicines differ in strength, formulation, use and approved conditions, so do not switch between them without professional advice.

What do the reported numbers mean?
MHRA figures provide context, but they are not a prediction of an individual’s risk. In an October 2024 safety update, the MHRA said common gastrointestinal side effects may affect more than 1 in 10 patients. It also reported 7,228 Yellow Card reports of nausea, vomiting and diarrhoea associated with GLP-1 receptor agonists indicated for weight management, counted as of 28 October 2024. Yellow Card reports are reports of suspected effects, not an incidence rate or proof that a medicine caused each event. Read the MHRA safety update.

Where can you get support in the UK?
Your prescriber or pharmacist is the right contact for medicine-specific symptoms and dosing advice. In England, NHS England describes wraparound care, including dietary and activity support, in its tirzepatide weight-management pathway. That pathway is specific to England and this context; access routes differ across the UK. NHS England’s pathway guidance explains the England arrangements.
