Fix architecture drift by checking each outdated architecture decision record (ADR) against today’s requirements and the system as built. Reaffirm decisions that still fit; document changed decisions in new ADRs that supersede—not erase—the old ones; then reconcile implementation and related artifacts. Finally, assign review ownership and a cadence that fits how quickly the system changes.

What an outdated ADR does—and does not—mean

A stale ADR is a reason to investigate, not proof that its decision is wrong. Requirements, constraints, available solutions, or the consequences of a choice may have changed; the original decision may still be appropriate in the current context. Google Cloud’s Architecture decision records overview and the UK government’s Architectural Decision Record Framework both treat context and consequences as information that should be revisited when circumstances change.

Keep three questions separate: Does the original choice still make sense? Does the software actually follow it? Do the documents accurately describe both the choice and the current state? A record can be stale while the decision remains valid, and updating a record does not bring non-compliant code into line.

How to repair outdated architecture decision records

1. Find decisions that need review

Collect the ADRs and flag those linked to changed requirements, platforms, dependencies, interfaces, quality attributes, team ownership, implementation exceptions, or known architecture violations. Also look for decisions that people rely on but cannot find in the project’s accessible documentation. Google Cloud describes ADRs as useful for recording choices among engineering options and for making decisions accessible to the team.

2. Compare each record with current reality

For each candidate, review its context, requirements, accepted decision, and stated consequences. Compare them with current code, configuration, diagrams, interfaces, and supporting documents. Ask the people responsible for the affected system whether assumptions, constraints, or consequences have changed.

Classify the result plainly:

  • Still valid: The original decision remains appropriate in the current context.
  • Changed: The team has made, or needs to make, a different decision.
  • Unclear: Available evidence is insufficient to establish what was decided or why.

The UK framework says an ADR should capture context, decision, consequences, stakeholders, and links to supporting material, and should be reviewed when its context or consequences change. If the history cannot be reconstructed confidently, record the uncertainty rather than presenting an inference as fact.

3. Reaffirm valid decisions or supersede changed ones

If the decision remains valid, leave its accepted rationale intact and record the review outcome in the team’s change-history mechanism. If the decision changes, write a new ADR with the current context, considered options, accepted choice, rationale, and consequences. Link it to the earlier record, identify the earlier ADR as superseded, and retain it in the decision log.

Do not silently rewrite an accepted ADR so it appears the new choice was always in force. AWS guidance on the best practices for using architectural decision records describes preserving the reason for a change; its architectural decision record process treats a different choice as a new proposal that supersedes the earlier ADR once accepted. Google Cloud puts it succinctly: “If you make adjustments, include the previous decision and why a change is made.”

4. Bring implementation and artifacts into line

Compare the accepted decision with the system as built, including relevant code, configuration, diagrams, interfaces, and supporting documentation. If legacy implementation does not comply, track that work separately from the ADR so readers can distinguish what the team decided from what remains to be done.

AWS recommends either bringing code or artifacts into compliance gradually as related changes are made, or recording explicit technical-debt work for a refactor. Choose the approach that fits the affected system and make ownership and follow-up visible; changing documentation alone does not remediate an implementation gap.

How to keep ADRs discoverable and current

Choose a home that makes records easy for affected developers and stakeholders to find and review. Google Cloud and AWS describe both version-controlled Markdown near the code and a shared wiki or central document location as viable approaches. There is no universally best platform; compare the options against your team’s workflow:

Whichever location you choose, make ownership clear, link each record to relevant code and supporting material, and keep the collection accessible from the project’s main documentation. Microsoft’s guidance on maintaining an ADR also notes that teams can reconstruct brownfield decisions retroactively where reliable historical information exists.

Set a review cadence that fits the system

The cited guidance recommends regular review and updates when context or consequences change, but it does not establish a universal monthly, quarterly, or annual interval. As a practical team policy, choose a recurring cadence based on the system’s change rate and add event-triggered reviews when requirements, constraints, available solutions, context, or consequences change. Record who is responsible for review and how the outcome is captured; do not rely on an unspecified future reminder.

Optional: use a tool for the record-keeping workflow

ADR Tools is an open-source command-line tool for creating and maintaining Markdown ADRs, including records that supersede earlier ones. It can help with the mechanics of maintaining a decision log, but a tool cannot determine whether a decision’s context has changed or whether the implementation follows it. Those checks remain part of the team’s review.
