Use the tests below to isolate the failing layer instead of repeatedly clicking Retry.
Start with a one-minute diagnosis
First cancel Setup and restart Windows. Then determine whether the failure is specific to one source or affects the computer generally.

This matrix is a troubleshooting heuristic, not proof. The wording is legacy installer language rather than a universal modern Windows error code. Microsoft’s archived SMS Installer documentation lists bad media, corrupt source files, other read/write errors, and a process-sequencing problem as possible causes: Microsoft KB 192140.
Safe first steps for any installer
- Cancel the installation and restart Windows.
- Obtain the installer again from the software publisher or another legitimate distribution channel. Re-downloading helps only when the original copy is incomplete or corrupt.
- If the download is a ZIP, ISO, CAB, or other archive, download it again and extract it to a short local path such as
C:InstallApp. - Move the installer to the computer’s local internal drive. Do not run it from a network share, cloud-synchronised folder, or removable drive while diagnosing the problem.
- Check that the destination drive has sufficient free space.
- For a trusted installer that requires elevation, right-click it and choose Run as administrator.
- Close other applications temporarily. If security software reports a detection, inspect its event history and verify the file’s provenance; do not leave antivirus protection disabled.
If the source is a CD or DVD
Inspect and clean the disc
Look for scratches, cracks, dirt, or delamination. Clean gently from the centre outward with a soft lint-free cloth. Do not heat the disc, aggressively polish it, or apply an unverified “repair” method.

Separate the disc from the drive
Try the disc in another optical drive or computer, and try a different known-good disc in the original drive. If the disc works elsewhere, the first drive may be dirty, ageing, or unable to read that media reliably. Historical troubleshooting discussions also identify the drive’s cable and controller as possible causes, rather than assuming the hard disk is at fault: AnandTech discussion.
Copy the disc before running Setup
- Create a folder such as
C:InstallDiscCopy. - Copy the entire disc contents into it.
- If Windows reports a read error, test the disc in another reader. A failure that follows the disc points to damaged media; a failure limited to one reader points to the drive or its connection.
- If copying completes, run Setup from the local copy. A successful copy does not prove the installer is compatible, but it removes the optical read path from the next test.
For an old game or application, a legitimate digital re-release or publisher-provided installer may be more reliable than repeatedly using a deteriorating disc.

If the installer was downloaded
Verify that the file is genuine and complete
- Use the publisher’s site or a reputable distribution channel; avoid cracks, modified repacks, peer-to-peer copies, and unknown mirrors.
- Compare the downloaded size with the publisher’s stated size when one is provided.
- When the publisher supplies a SHA-256 hash, calculate yours with this optional command in Command Prompt:
certutil -hashfile "C:Pathinstaller.exe" SHA256
Compare the output exactly with the published value. - Re-download the whole archive if extraction reports CRC or checksum errors; do not keep reusing partially extracted files.
Use a local, uncomplicated path
Extract again to a short folder on a local drive and run Setup there. This avoids network interruptions, removable-media errors, and cloud-sync conflicts. If the same installer works on another computer, investigate the original computer rather than downloading the same file indefinitely. Historical reports describe alternate copies helping some users, but those anecdotes do not establish a universal fix: newbie.org example.
When several installers fail
Stop treating each failure as a bad download. Back up important data before testing a possibly failing drive.

Check storage and Windows logs
- Confirm free space on the destination volume.
- Open Event Viewer and review Windows Logs → System for disk, NTFS, storage-controller, or device-reset events around the failure.
- Use the storage manufacturer’s diagnostic utility where available. On supported Windows versions,
chkdsk C: /scanperforms an online scan, but it is not a complete hardware-health test and should not replace a backup or manufacturer diagnostics.
Test memory and the transfer path
Run a reputable memory diagnostic when failures move between files, archive extraction produces CRC errors, large copies fail, or replacing the source changes nothing. Reseat or replace removable media and data cables where applicable. RAM, hard disks, optical drives, IDE cables, and motherboard controllers have all been raised in historical user troubleshooting, but those reports are hypotheses and experiences—not proof of the cause in your case: AnandTech discussion.
Use Windows integrity checks for their intended purpose
sfc /scannow checks protected Windows system files. It may help with damaged Windows components, but it does not directly test an installer, CD/DVD, or USB device and is not a guaranteed fix.

Consider security software and locks
Endpoint protection can quarantine an installer, block a child process, or hold a file open. Verify authenticity, scan the file, and inspect the security product’s detection history first. If the publisher confirms a false positive, use a narrowly scoped temporary exception through the product’s documented process, then remove it. Close backup, indexing, overlay, or similar tools only when there is a concrete indication that they are locking the file. Old advice to disable antivirus generally should not be followed; one historical game guide is not a current universal recommendation: archived RPGDot discussion.

Check compatibility before changing hardware
For a driver, confirm the exact computer model, hardware revision, Windows release, and 32-bit or 64-bit architecture. Use the manufacturer’s package intended for that combination. A file can be authentic yet still be wrong for your model or operating-system version. Prefer Windows Update or the manufacturer’s current support mechanism for supported hardware; do not use random driver-download sites.

After an operating-system upgrade, an old driver installer may fail because it was never designed for that Windows release. Compatibility guidance or pending updates may be relevant, but neither is guaranteed. A Microsoft Q&A thread reporting this wording during Sony Vaio driver installation is a community exchange, not a definitive diagnosis: Microsoft Q&A thread.
The legacy SMS Installer exception
Microsoft documented a specific scripting cause: an SMS Installer package uses an Execute Program action without waiting for the launched program to exit. The child process still has a required file open when the script continues, producing the I/O message. This is not a general Windows setting for ordinary end users.
If you author or rebuild that package:
- Open the SMS Installer script.
- Locate the Execute Program action.
- Open its settings.
- Enable Wait for program to exit.
- Rebuild or rerun the package.

When to replace media or hardware
- If one disc fails on multiple readers, replace or legitimately reacquire the disc.
- If multiple known-good discs fail in one reader, investigate or replace that optical drive and its connection.
- If large-file copies and archive extraction also fail, test storage and RAM before blaming installers.

- Do not flash firmware intended for another motherboard or optical-drive model.
- Do not install pirated or modified packages.
- Do not leave antivirus disabled or delete registry and system files because of a generic “fix” guide.
- Do not continue using a disk that produces read errors without backing up important data.
Final checklist
- Cancel Setup and restart Windows.
- Decide whether one source or many installers fail.
- For a disc, clean it, test another reader, and copy its contents locally.
- For a download, obtain a legitimate fresh copy, verify its hash when supplied, and re-extract it.
- Run from a local folder with adequate free space and appropriate permissions.
- Check security logs, Event Viewer, storage diagnostics, and memory when failures are system-wide or move between files.
- For old drivers, verify model, architecture, and supported Windows version.
- For SMS Installer scripts, enable Wait for program to exit on the relevant Execute Program action.
