To correct duplicate or outdated information in an electronic health record (EHR) note, first verify the patient and encounter, identify the exact statement and compare it with reliable source information, then follow your organization’s authorized correction process. If the note is signed or finalized, use the approved amendment, correction, addendum, or retraction workflow so the original remains traceable. If patient identity is uncertain, stop and escalate rather than editing.

Start by confirming the patient, encounter, and note

Before changing documentation, verify the patient using your organization’s identification procedures and check the encounter, service date, author, and source of the disputed content. Accurate patient identification is a safety practice in the ONC SAFER Guides. If the concern could involve a wrong-patient entry or an overlay between records, pause and route it to the organization’s health information management (HIM) or patient-identity process. Do not try to resolve an identity problem by editing the note yourself.

Locate the specific note, section, and sentence or data element at issue. Distinguish genuinely repeated content from material that was accurate when recorded but is no longer current. Compare it with relevant source documentation and the encounter context; avoid changing unrelated facts simply to improve the prose.

Choose the correction path based on note status

Determine whether the note is an unsigned draft or has been signed or finalized, and confirm who is authorized to make the change. EHR labels and permissions vary, so local policy and system instructions govern the actual steps.

AHIMA’s terminology toolkit distinguishes these actions, but the EHR may use different labels. In that toolkit, an amendment is a broad alteration after documentation is complete and signed; a correction clarifies inaccuracies; an addendum adds omitted information; and a retraction changes how erroneous material is displayed while retaining a prior version. Use your organization’s defined workflow rather than assuming the terms map identically across systems.

Make the change traceable

For a finalized note, use the EHR’s correction, amendment, or addendum function—not an undocumented overwrite. Record the current date and time, the identity of the person making the change, the reason, and any required electronic signature. Keep the original entry accessible and readable, and make clear that a change occurred.

AHIMA’s Integrity of the Healthcare Record: Best Practices for EHR Documentation (2013 update) says the original entry must remain viewable along with a date and time stamp, the name of the person making the change, and the reason. HL7’s EHR-S FM Release 2.1.1, requirement RI.1.3.2#02, says a system “SHALL provide the ability to tag a Record Entry as an amendment, a correction of erroneous information and the reason, or an augmentation to supplement content.” These are professional guidance and a functional standard, respectively; follow applicable law and current local policy for operational requirements.

Check whether the information has propagated

A change in the source EHR may not update every place where the information was copied or shared. Check whether the affected content appears in another note, a printed record, a patient portal, a coding or operational workflow, or a health information exchange (HIE). Identify the originating source and follow the organization’s process for communicating corrections to connected records or users.

Journal of AHIMA’s June 6, 2022 discussion of amendments in interoperable settings highlights source tracking, version control, clear ownership, routine audits, and communication processes. Those practices help teams distinguish the authoritative record from a downstream copy; do not assume that editing one system corrects all copies.

Close the loop and address recurring duplication

Complete any required review, approval, and notification steps. If the duplicate wording appears repeatedly, consider whether a template, copy-forward habit, or workflow is contributing to it, and route that concern through the appropriate process. Correct only the affected information; retain historical context when it remains clinically and legally relevant.

If you are a patient requesting a correction

Patients and caregivers do not directly edit the clinical record. In the United States, the Office of the National Coordinator for Health Information Technology (ONC) advises contacting the provider’s office to learn its correction process and describing the requested change. Use the office’s form if one is available; a letter may be useful for a complex request. ONC’s Check It guide, last updated April 1, 2026, says the provider has 60 days to respond unless it requests an extension. If the provider disagrees, the patient can respond formally and ask that the original request and denial be associated with the affected record. This is patient-facing U.S. guidance, not a universal staff workflow.

What to verify before you finish

  • The patient and encounter are correct; any suspected overlay or wrong-patient issue was escalated.
  • The exact duplicated or outdated content was compared with an appropriate source.
  • The note’s draft or finalized status and the editor’s authority were confirmed.
  • The chosen action—correction, addendum, or retraction—matches the issue and local terminology.
  • The original entry, change author, date and time, reason, and required signature are retained in the record.
  • Potential downstream copies and required communications were considered.

Requirements differ by jurisdiction, organization, and EHR. NHS England guidance, for example, also emphasizes an audit trail and recording the reason and date/time for an amendment or deletion, but it applies to the UK environment and should not be treated as U.S. law.
