To create a form in Formstack Forms, open the Forms area, choose Create, start from a template or a blank form, then add and configure fields, customize the form, and review it before sharing. If the form needs to work directly with Salesforce records, use Forms for Salesforce instead; its setup follows a different workflow.
Choose the right Formstack product first
Use standard Formstack Forms for a general online form that may selectively sync data into Salesforce. Use Forms for Salesforce when the form should be built directly around Salesforce data and objects. These products have different creation steps, so do not assume a standard Forms tutorial applies to a Salesforce form. Formstack explains the distinction in its Salesforce data-model guide.
Create a form in standard Formstack Forms
1. Start a new form
In Formstack Forms, go to the Forms tab and choose Create. Name the form and set its URL and language, then choose a template or start with a blank form. Exact button labels may change as the interface is updated; Formstack’s basic form editing guide documents this starting flow.
2. Choose a template or a blank form
A template gives you a prebuilt structure to adapt, which can reduce initial setup. A blank form gives you more control over the questions and order from the outset. You can also choose a starting theme—default, light, or dark—and change the theme later in the builder.

3. Add fields that match the answers you need
Drag fields from the Form Fields area into the form. A good field choice makes the form easier to complete and the answers easier to use:
- Short Answer: for a simple, single-line response, such as a name or reference number.
- Long Answer: for a detailed response. Formstack documents a limit of up to 60,000 characters for this field.
- Dropdown: for one selection from a list of options.
- Radio buttons: when respondents should choose one option from a visible set.
- Checkboxes: when respondents may select more than one option.
Field labels become the questions respondents see, so make them specific and understandable. Formstack’s field editing guide covers field types and settings.

4. Configure and arrange the fields
Select a field to edit its type, label, options, size, language, or field logic. Put questions in a natural order and group related questions into sections so respondents can follow the form without having to infer why information is being requested.
Keep the form as simple as the task allows. Formstack warns that builder performance can decrease as the number and complexity of fields increase, but it does not give a fixed field-count threshold.

5. Customize the appearance
Choose a theme and adjust the form’s colors, fonts, or images to suit its purpose. Formstack also describes CSS editing for more customized styling. Its drag-and-drop form builder page outlines these appearance options.
6. Add behavior and connections if needed
Depending on the account and plan, Formstack describes features such as conditional logic, Smart Lists, data routing, email confirmations and notifications, and integrations. Use conditional logic when a later question should depend on an earlier answer, and verify that any feature you need is available in your account before building around it.

7. Review and test before sharing
Preview the form and submit test responses. Check required fields, each conditional path, confirmation messages, notifications, and the experience on the respondent side. Formstack specifically says AI-generated forms are starting drafts rather than finished forms: its Help Center notes that they need further review and testing before distribution. The same practical review is useful for forms built manually.

If the form should create or update Salesforce records directly, follow the Forms for Salesforce workflow rather than the standard Forms steps above. Formstack’s guide describes creating the form inside the Formstack app in Salesforce, choosing a primary object and related objects, then adding fields from those objects or general input fields before publishing. Publish or share the form
Once the form is reviewed, use the sharing or publishing controls available in your current Forms interface to distribute it. Formstack’s available documentation for a Share tab, hosted URL, embed code, and preview/publish steps is explicitly for the legacy Workflow Forms feature; do not assume those exact labels or steps apply to standard Forms. Check the current standard Forms interface for its sharing options.
Be careful when editing a form with submissions
Before deleting a field from a form that has already collected responses, consider the stored data. Formstack says deleting a field removes its data from the Submissions page. It also says an accidentally deleted field can be recovered within seven days. Consult the field management guide before removing a field that may matter to existing submissions.
Current Formstack Forms plan details
On Formstack’s pricing page, checked October 7, 2026, the Forms plan was listed at $83 per month when billed annually or $99 per month with monthly billing. The displayed plan included one builder user, 25 forms, 1,000 submissions per form, and 2 GB of storage. Formstack also listed a 14-day Forms trial with no credit card required. These are vendor-listed terms and may change; confirm the current Formstack pricing page before relying on them.
