To add human approval to an AI workflow, pause the specific consequential action before it runs, show a qualified reviewer the proposal and relevant context, and give that person real authority to approve, reject, request changes, escalate, or stop execution. Record the decision and define what happens if no decision arrives. An approval button alone is not meaningful oversight if the reviewer lacks information, time, authority, or a way to prevent the action.

Where should human approval sit in an AI workflow?

Put the gate immediately before the action whose consequences warrant review—not merely after the AI produces an answer. For example, if an AI drafts a customer response, the gate belongs before sending if an incorrect message could materially affect the customer. If the AI only summarizes material for an employee who makes the consequential decision later, the appropriate review point may be that later decision instead.

Start by mapping the workflow: what the AI proposes, what happens next, who could be affected, how an error or delay could cause harm, and whether the action can be reversed. Use that assessment to decide which actions need a gate and how much review they need. This is a practical risk-based design method, not a checklist prescribed by NIST.

Then set a clear boundary for delegated action. Specify what the AI may do on its own, what it may recommend but not execute, and what must pause for a human decision. Scope each approval to the particular action and context the reviewer saw. If the proposal or material context changes, require a fresh decision rather than carrying approval forward automatically.

How to build the approval gate

  1. Define the risk and gate. Identify the action, affected people, likely consequences of error or delay, and reversibility. Decide which actions must pause and whether the risk calls for a specialist, an escalation route, or an independent check.
  2. Assign an accountable reviewer. Name a role—not just a team inbox—with the relevant competence, training, and authority to decide. Define who covers absences and how urgent cases or disagreements are handled. NIST calls for documented roles, responsibilities, communication lines, and training; the choice of role and coverage arrangement depends on the workflow.
  3. Show decision-useful context. Present the proposed action, relevant inputs or evidence, known system limitations, and what approval will cause. Make uncertainty or missing information visible when available. These are implementation recommendations: neither the EU AI Act nor NIST provides a universal screen layout.
  4. Pause downstream execution. Hold the action until an authorized decision arrives. Offer distinct outcomes: approve, reject, request revision, or escalate. Provide a safe stop or interruption mechanism where appropriate, and decide in advance what happens if the reviewer is unavailable or the approval request times out.
  5. Record the decision and monitor results. A useful record can include the proposal, relevant system or workflow version, reviewer role, decision, time, and reason or requested change. Track rejected, overridden, escalated, timed-out, and corrected cases, then reassess the gate when the workflow or its risks change. This event schema is a recommended implementation pattern, not a universal legal requirement.

What makes a human review meaningful?

A reviewer needs enough understanding and authority to exercise independent judgment. For covered high-risk AI systems, Article 14 of the EU AI Act describes oversight capabilities that include understanding system capabilities and limitations, monitoring its operation, correctly interpreting outputs, disregarding or overriding outputs, and intervening or stopping the system safely. It also addresses the risk of people relying automatically or excessively on AI output.

  • Information: The reviewer can see what action is proposed and the context needed to judge it, rather than being asked to accept an unexplained result.
  • Competence and time: The reviewer understands the task and can give it adequate attention. Training and workload should match the decisions being reviewed.
  • Authority: The reviewer can refuse the recommendation, request revision, escalate, or stop the action without being penalized for disagreeing with the system.
  • Operational control: A rejection or stop actually prevents execution; it is not merely recorded after the action has occurred.
  • Accountability: The organization can determine who reviewed what, under which workflow, and what decision followed.

A checkbox without these conditions is weak evidence of effective oversight. Adding a human does not automatically eliminate risk: review must be proportionate to the consequence, the system’s autonomy, and the context of use.

How should rejection, revision, escalation, and failure work?

Define the gate as a workflow with explicit states, not a notification with an approve button. A practical design distinguishes these outcomes:

  • Approve: Release only the action and context the reviewer approved.
  • Reject: Cancel the proposed action and route the case to an appropriate alternative, if one exists.
  • Request revision: Return the case for changes. Once the proposal or material context changes, send it back for review.
  • Escalate: Route uncertain, disputed, or higher-risk cases to a named role with appropriate expertise or authority.
  • Stop or interrupt: Prevent further execution when the reviewer identifies a safety concern or the process needs to be halted.

Also decide what happens when approval cannot be obtained: the reviewer is absent, the request expires, a notification fails, or the workflow encounters an error. For consequential actions, a safe default is to keep execution paused and route the case to a defined backup or escalation path; do not silently treat missing approval as consent. That is a workflow design recommendation, not a universal rule stated by the cited standards.

How much review is enough?

Scale oversight to the possible harm, the system’s autonomy, and the conditions in which it is used. A reversible, low-impact suggestion may need a lighter control than an action affecting someone’s rights, safety, access, or finances. Also consider whether a single reviewer has the right expertise and whether an independent check is justified. More review is not automatically better if it creates delay or encourages rubber-stamping; target human judgment where it can meaningfully change the outcome.

  • Risk coverage: Which actions are gated, and does the gate account for consequence and reversibility?
  • Decision quality: Can reviewers see relevant context, limitations, and a clear description of the action?
  • Authority: Can the assigned person reject, override, escalate, and stop execution?
  • Failure handling: What happens on timeout, tool failure, missing context, disagreement, or an unavailable approver?
  • Auditability: Can records connect the proposal, decision, reviewer, time, and workflow or system version?
  • Operational cost: Does review burden and latency fit the risk, and is review focused where human judgment adds value?

What do NIST and the EU AI Act require or recommend?

NIST AI Risk Management Framework

NIST describes the AI Risk Management Framework (AI RMF) as voluntary guidance, organized around Govern, Map, Measure, and Manage. Its Core treats governance as continual across the AI system life cycle, calls for clear roles and training, and says human oversight processes should be defined, assessed, and documented in accordance with organizational policies. NIST also says AI RMF 1.0 is being revised, so check its current status when relying on it. The AI RMF Playbook offers voluntary suggested actions based on AI RMF 1.0.

NIST’s separate Risk Management Framework Authorize step offers a useful analogy for decision rights: it calls for a senior official to decide whether system security and privacy risk is acceptable, with authorization approved or denied. That is an RMF authorization process, not a direct instruction for every generative AI workflow. See NIST’s Authorize step.

EU AI Act Article 14

Article 14 of Regulation (EU) 2024/1689 addresses human oversight for high-risk AI systems covered by the Act. The consolidated text cited here is dated 2026-07-27. Its requirements include oversight measures proportionate to the system’s risk, autonomy, and context of use, and capabilities for reviewers to understand and monitor the system, interpret its output, disregard or override it, and intervene or stop it safely. Article 14(5) adds a two-person verification provision for a defined remote biometric identification case, with stated exceptions; it is not a general two-person approval rule for AI workflows.

Legal applicability depends on the system, its use, and jurisdiction. The Act’s high-risk provisions should not be treated as applying to every AI tool or approval step. Consult the consolidated Regulation (EU) 2024/1689 and qualified legal advice for a specific classification or compliance decision.

What should the approval record contain?

NIST calls for oversight processes to be defined, assessed, and documented; the EU AI Act also includes logging provisions for covered high-risk systems. Neither establishes the event fields below as one universal schema. As a practical starting point, record:

  • The proposed action and the relevant input or context reviewed.
  • The system or workflow version that generated the proposal.
  • The reviewer’s role and the time of the decision.
  • The outcome—approval, rejection, revision request, escalation, or stop—and any reason or change needed to interpret it.
  • Whether the action was ultimately executed, and any correction or follow-up that matters to monitoring.

Use those records to look for patterns such as frequent overrides, repeat missing information, timeouts, or a high volume of approvals made without meaningful review. Reassess the gate when these patterns—or changes to the model, workflow, intended use, or affected people—alter the risk.
