Make enemy health legible by keeping three jobs separate: identify each foe, track its damage, and decide how much of its condition players can see. You can show a broad state such as “badly hurt” without revealing exact hit points, and a health cue does not have to change the game’s rules.
Choose what players should know
Agree on the amount of health information that helps the group make decisions without undermining the uncertainty you want. You can disclose exact current HP, show a bar without numbers, use a few named condition bands, or reveal only a specific rules threshold. The labels and thresholds should fit your game: D&D’s “bloodied” terminology is not a universal rule.
Most D&D Dungeon Masters track monster HP secretly, according to the 2024 Basic Rules’ DM’s Toolbox. That does not prevent you from signaling a broad state. In a historical D&D article, Dragon 375 producer Chris Sims advised telling players when a monster becomes bloodied and when that state changes. That is D&D-era design advice from May 19, 2009—not a current rule for every game or a requirement to reveal exact totals. Read the historical article.
Compare the main cue options

When choosing, consider precision (exact numbers or an approximate state), visibility (always shown or available on request), table burden (a quick spoken description or repeated manual updates), and accessibility (whether the cue works without relying on color, small text, or constant visual attention). These are practical comparison points, not claims of measured effectiveness.

Identify duplicate enemies before tracking damage
If several foes look alike, assign each a stable identity before combat. A memorable feature such as “scar” or “helmet” works, as do letters such as A, B, and C on the miniatures or tokens. The 2024 D&D Basic Rules recommend distinguishing duplicate monsters with unique features or labeled stickers. See the DM’s Toolbox.
Use the same identifier in your notes and at the table. If the miniature marked “scar” takes damage, record it against “scar,” not an interchangeable monster entry. Keep the identifier distinct from health or condition markers so that, for example, a letter identifies enemy B while a separate symbol indicates that it is injured.

Track damage consistently without revealing the total
Once each enemy has an identity, choose one method for recording its damage and use it throughout the encounter. The 2024 D&D Basic Rules note that recording damage taken can be easier than subtracting each hit from starting HP. You can put damage totals beside enemy letters on an initiative list, including one players can see, without showing remaining HP. The DM’s Toolbox describes this approach.
For example, if the foes are A, B, and C, your private or shared list can record each foe’s accumulated damage under its letter. The list’s visibility is separate from what you disclose about remaining health: decide whether players see only names, the damage taken, or a derived condition such as “hurt.”

Add a clear cue at an in-person table
Start with a cue that is easy to maintain: say the condition aloud, write a label on a card, place a paper token beside a miniature, or mark the miniature. D&D’s official guidance suggests tools such as index cards, sticky notes, tokens, and colored plastic rings as reminders. These can be homemade aids; a scrap of paper or a marked token is enough.
Colored rings or status tokens for miniatures can help attach a visible cue to a particular foe, but make the meaning explicit. A ring might indicate “badly hurt,” “poisoned,” or “enemy B”—those are different pieces of information and should not be left for players to guess. Homemade rings, paper markers, and cards work just as well as purchased aids.
Make the cue readable without color
Do not communicate health or identity through color alone. Microsoft’s Xbox Accessibility Guidelines state, “Color alone should never be used to represent information,” and recommend pairing color with elements such as symbols, shapes, or text labels. That guidance is written for Xbox design, but the principle is useful for tabletop cues too. Read Microsoft’s Guideline 103.

Pair a colored ring with a letter, icon, shape, or word. For example, a red marker could also carry a “hurt” label or a distinct notch pattern. If visual cues are easy to miss at your table, say the state aloud when it changes.

Virtual tabletop controls vary by platform. Fantasy Grounds documents enemy health display options that include a dot, bar, hover display, tooltip, or no player-facing display. Choose the mode that matches your intended disclosure: a persistent bar for immediate feedback, a hover or tooltip mode for less prominent information, or no display when health should remain hidden. Check the view from the player side, not only the GM’s view. GM and player settings can differ, and a setting that appears informative behind the screen may expose a different amount—or nothing—to players. Other virtual tabletops may use different controls or labels.
Keep health, identity, and other conditions distinct
A marker can mean “poisoned,” “near defeat,” “targeted,” or “this is enemy C.” If one kind of marker could be mistaken for another, label it or define it clearly before using it. Likewise, connect narration such as “staggering” or “barely standing” to a consistent health band if players are expected to use it for tactical decisions. Otherwise, describe the fiction without implying an unannounced mechanical threshold.
