The cell membrane helps protect a cell by controlling what enters and leaves, but it is not an impermeable shield against bacterial toxins. Some toxins bind to membrane components and use the cell surface as a platform to assemble pores that disrupt the membrane. Cells can detect and repair some damage, but repair is not guaranteed.

What protection does the cell membrane provide?

The plasma membrane is a selectively permeable boundary: its lipid bilayer and associated molecules regulate the movement of substances between a cell and its surroundings. That selectivity helps maintain the conditions a cell needs to function. It does not mean every toxin is blocked. Some bacterial toxins exploit membrane lipids, glycans, or proteins as binding sites, while other bacterial toxins act by routes or at targets beyond the membrane.

The mechanism below focuses on pore-forming toxins, an important group whose activity directly damages the cell membrane. It should not be taken as a description of every bacterial toxin.

How pore-forming toxins damage the membrane

  1. Binding: A toxin released as a soluble protein attaches to a target cell. Depending on the toxin family, it may bind a lipid, a surface glycan, a protein, or more than one type of membrane component. This interaction can concentrate toxin at the cell surface.
  2. Assembly: Toxin molecules gather into larger complexes, often by oligomerizing. The precise order and details vary among toxin families.
  3. Insertion: The assembled toxin changes shape and inserts a membrane-spanning region into the lipid bilayer. Toxins can form different kinds of membrane-spanning structures, including alpha-helical or beta-barrel structures.
  4. Pore formation: The resulting opening lets ions and other solutes cross the membrane in ways the intact cell normally controls. This can disrupt ion gradients and interfere with cell function.

The sequence is a useful general model, not a universal script: the receptor, assembly process, and structure of the pore depend on the toxin.

Cholesterol-dependent cytolysins: one specific example

Cholesterol-dependent cytolysins illustrate how a toxin can exploit a particular membrane component. Soluble monomers in this family bind cholesterol-rich membranes and assemble into large, ring-shaped complexes. A 2018 review in Biophysical Reviews describes complexes containing around 40 monomers. That approximate count applies to the reviewed cholesterol-dependent cytolysin complexes; it is not a general pore size or assembly count for bacterial toxins as a whole.

How cells respond to membrane injury

A pore can allow calcium to enter and potassium to leave the cell. These changes can act as signals that membrane injury has occurred. Cells may respond by remodeling the membrane and removing or internalizing pore material. In a 2008 review, F. G. van der Goot and colleagues describe how cells can sense pore formation, mount defenses, and sometimes repair membrane damage and survive: “Bacterial pore-forming toxins: The (w)hole story?”

Repair is not assured. The outcome depends on the toxin, the extent and type of membrane damage, the exposure, and the cell’s response. If injury is too severe or cannot be reversed, the cell may die. Review literature also notes that how cells repair stable toxin pores and restore normal balance is not fully understood.

What this protection does—and does not—mean

  • The membrane’s selectivity is a defense, not a guarantee that toxins cannot reach or damage a cell.
  • Some pore-forming toxins bind membrane components and create openings that disrupt normal permeability.
  • Cells have responses that can limit or repair some membrane injuries, but the result depends on the particular toxin and cell.
  • Cholesterol dependence is a feature of a specific toxin family, not a property to assume for all bacterial toxins.
