Psychological horror games build suspense by making players anticipate danger, interpret unsettling cues, and doubt what they know. Instead of leaning on repeated loud reveals, they can shape sound and visuals over time, make familiar spaces behave strangely, limit information or control, and let story details acquire new meaning as play continues. These techniques can also include an occasional sudden scare; the distinction is whether surprise is a punctuation mark or the main source of fear.

Why anticipation can be more unsettling than a jump scare

A jump scare delivers a sudden event. Suspense makes the player wait for one—or wonder whether one is coming at all. In the gap between a cue and its explanation, the player supplies possibilities: something may be behind a door, a sound may signal a threat, or a familiar hallway may no longer be safe.

The developers of The Tartarus Key describe atmosphere as tension built through visuals and sound, with fear partly coming from what a player imagines might be present. Their distinction is not that an atmospheric game must avoid every sudden movement or noise. A surprise can still appear, but repeating a familiar cue can make it predictable and less effective. The design goal is to make the player’s expectation do much of the work, rather than relying on the same startling reveal again and again. The developers explain the distinction in a March 21, 2022 Steam dev log.

How sound and visuals build a sequence of tension

A frightening image or sound does not work in isolation: its timing and relationship to earlier cues can change how the moment feels. A quiet stretch, a repeated sound, a glimpse of movement, and then a reveal can create a rising expectation that a single loud effect may not.

Graja, Lopes, and Chanel studied an adaptation of Konami’s P.T., using galvanic skin response and participant reports of perceived emotion. They report that the ordering of effects can prompt emotional responses and that sound can contribute to a gradual build-up of psychological impact. This supports treating audiovisual pacing as a design tool, not following a universal formula: the study does not establish one best sequence for every game or player. The study was published in IEEE Transactions on Games in 2021.

How strange spaces create unease without a visible enemy

A space can feel threatening even when nothing attacks. A corridor that loops, a transition that disorients, or a room whose layout seems impossible can make the player question their sense of direction and the rules of the game world. That uncertainty shifts attention from “Where is the enemy?” to “Can I trust this place?”

Jiawen Zhang’s 2026 Southern Methodist University thesis examines abnormal environmental design in a purpose-built Half-Life 2 level, “Echoes of Manor.” It discusses non-Euclidean layouts and disorienting spatial relationships, and describes tension-release cycles and light puzzle elements as ways to sustain engagement and avoid boredom. This is a focused design study, not proof that every player will react the same way to spatial distortion. Read the thesis at Southern Methodist University.

When limited information or control helps—and when it does not

Restricting what a player can see or do can make vulnerability tangible. A confined setting, sparse knowledge about the outside world, and a first-person viewpoint can prompt players to infer what might be happening beyond their immediate view. The uncertainty is most effective when the game gives it stakes: a sound, a risk, or a decision that makes the unknown matter.

Mienke Fouché’s analysis of Iron Lung describes how its confined environment, limited control, sparse information, and first-person perspective contribute to the experience. The article’s analysis of reviewer responses also identifies a trade-off: missing information can stimulate imagination, but some reviewers felt that a lack of threats impeded immersion. Withholding information is therefore not automatically suspenseful; if players cannot sense meaningful danger or consequence, uncertainty may feel empty instead. How layered stories make earlier clues feel different

Suspense can come from understanding a clue only later. Audio logs, brief scenes, voice-over, production design, and sound can add context over multiple encounters, changing how players interpret what they have already seen. A story need not explain every mystery immediately to feel deliberate; the discoveries need to give the player something consequential to reconsider.

Housemarque creative director Gregory Louden described this intention for Returnal as wanting a story that stays with players: “Not haunting like jump scares, but haunting like something that stays with you.” He explains that repeated cycles reveal narrative layers through audio logs, cinematics, voice-over, production design, and sound design. What makes a psychological horror game actually unsettling?

Usually, it is the relationship between expectation and evidence. The game gives the player enough cues to imagine a threat, but not enough to settle what is happening. Space, sound, limited agency, and story can reinforce that uncertainty in different ways. The approaches above are design possibilities supported by particular studies, creator accounts, and case analyses—not guaranteed effects across all games or players.
