Pain scales turn a person’s report—or, when self-report is not possible, structured observation—into information that can be communicated and tracked. They do not objectively measure pain or identify its cause. The right scale depends on the person’s communication and sensory abilities and on whether the goal is a quick intensity rating or a fuller picture of pain’s effects.

What a pain scale can—and cannot—tell you

“Pain is a subjective experience, meaning only the person experiencing pain can describe how much pain they feel and how it affects their life,” explains the National Institutes of Health (NIH). NIH also notes that no single blood test, scan, or device accurately and completely measures another person’s pain. A scale makes a report easier to communicate and follow over time; it is not an objective reading or a diagnosis.

A score is most useful when interpreted alongside the person’s description, history, function, and goals. A number alone does not capture every quality of pain or how much it disrupts daily life, and it should not be treated as a universal instruction to start or change treatment.

How the common pain scales work

Quick intensity scales ask for a response in different formats. Their endpoints and versions matter, so scores from different scales—or differently worded versions—should not automatically be treated as equivalent.

Numeric Rating Scale (NRS)

The person chooses a number, commonly from 0 to 10, between endpoints such as “no pain” and “worst pain.” The response can be given verbally or marked. The VA’s 2008 evidence table describes a 0–10 range; use the endpoint labels specified by the scale in your setting, and record them when relevant.

Visual Analog Scale (VAS)

The person marks a position on a line between two endpoint descriptions. The VA’s 2008 table describes a 100 mm line. NIDA’s 2016 instrument catalog describes a 10-centimeter line whose response may be recorded as the number of millimeters from the left end, yielding a 0–100 mm measure. These are descriptions of VAS formats, not a reason to assume every form is identical.

Verbal descriptor scale

The person selects an ordered word category, such as none, mild, moderate, or severe. Because it relies on words rather than a number or mark, it can suit some people better, but the category labels are still subjective and should be recorded consistently.

Faces scales

The person selects a face intended to represent a level of pain. “Faces scale” does not identify one standardized instrument: versions differ in faces, numbering, and age guidance. Name the exact version used, and consider whether the person can see and interpret its images.

Choose a scale for the person and the purpose

There is no universally best pain scale. Compare the response format with what the person can understand and use, then consider the assessment’s purpose, setting, and need for repeated ratings. SAMHSA’s comparison describes tradeoffs among numeric, faces, verbal or graphic, and visual analog formats (SAMHSA pain scales comparison).

Scale Response format Practical fit and limitations
NRS Spoken or marked number, commonly 0–10 Useful for a quick rating when the person can comfortably translate pain into numbers and understands the stated anchors.
VAS Mark on a line between endpoint descriptions Requires seeing and marking the line; vision, motor, or cognitive limitations may make it impractical.
Verbal descriptor Choose an ordered word category May work better for someone who finds numbers or line-marking difficult, provided the words are understood.
Faces Choose among faces representing pain levels May help some children or people with communication difficulty; visual access, interpretation, and the particular version’s guidance matter.

For consistent follow-up, use the same scale version and endpoint wording when possible. If the person’s abilities or circumstances change, reassess whether the format remains usable rather than assuming a previous score is directly comparable.

When an intensity score is not enough

A one-dimensional rating answers a narrow question about intensity. If the assessment needs to capture broader impact or qualities, add a multidimensional questionnaire rather than asking a single number to do that work.

  • The Brief Pain Inventory assesses pain intensity and disability.
  • The McGill questionnaire addresses sensory, affective, and evaluative dimensions of pain.

A 2024 postoperative guideline distinguishes rapid unidimensional intensity measures from more comprehensive multidimensional assessment. It reports that a 2022 systematic review covered 31 studies and 12,498 participants; those counts describe the evidence base, not proof that one scale is best. The guideline makes weak recommendations for both quick intensity tools (low-quality evidence) and multidimensional assessment (moderate-quality evidence), so these are clinical guidance rather than universal rules (2024 postoperative pain guideline).

Choosing a scale for a child or someone who cannot self-report

Children

Use an age-appropriate, named instrument and follow its specific guidance and local practice. Pediatric guidance lists self-report and behavioral options with tool-specific age ranges; it does not make every faces scale suitable for every child (pediatric consensus guideline). When a person can self-report, a cited clinical chapter identifies self-report as the most reliable way to assess intensity.

When self-report is unavailable

Behavioral observation tools can structure assessment when a person cannot report pain. They are observations, not a substitute for self-report when the person is able to communicate it. Follow the tool’s intended population and setting, and interpret observed behavior in context (clinical chapter on pain assessment).

How to record and interpret a pain rating

  1. Identify the instrument. Record the scale name and, for faces or other variant tools, the specific version.
  2. Preserve the anchors. Note the endpoint wording or response categories used, especially for a numeric rating.
  3. Keep the person’s report distinct from observation. Make clear whether a score came from self-report or a behavioral tool.
  4. Add relevant context. Interpret the rating with function, history, the person’s goals, and the reason for assessment.
  5. Use a consistent format for comparison. When tracking change, repeat the same scale and wording where practical; do not assume scores from different formats mean the same thing.

Clinical practice varies by population, setting, and local policy. Before reproducing or using a particular form, check its terms and the applicable institutional protocol; a general scale description does not establish permission to reproduce a named instrument.
