A 2026 review found that adults sleeping 7–8 hours a night had lower observed dementia risk than those sleeping under 7 hours or over 8 hours. The comparison came from prospective cohort studies; it does not show that 7–8 hours prevents dementia or that changing sleep duration will reduce a person’s risk.

What sleep duration was linked to lower dementia risk?

The 7–8-hour range was the reference category in a systematic review and meta-analysis of prospective cohort studies. Across 17 studies involving 1,344,170 participants, the pooled estimates were higher for both shorter and longer sleep than for the reference group.

These are relative comparisons, not percentage-point increases or estimates of any individual’s absolute chance of developing dementia. The review, by Oye-Somefun and colleagues, was published in PLOS ONE on April 8, 2026.

What the study examined

The review assessed prospective cohort evidence on sleep duration, physical activity and sedentary behaviour in community-dwelling adults aged 35 and older. The authors searched five databases for studies published from 1946 through August 2025 and used random-effects meta-analysis to pool results for incident dementia.

For sleep, all 17 studies compared short sleep (under 7 hours) and long sleep (over 8 hours) with the 7–8-hour reference category. The pooled results showed moderate variation among studies for short sleep (I²=41.1%) and substantial variation for long sleep (I²=66.9%). That variation means the observed association was not identical across the studies.

Does the review prove that 7–8 hours prevents dementia?

No. The evidence is observational: researchers followed cohorts and compared sleep patterns with later dementia diagnoses, rather than assigning people to sleep for different durations. The findings identify associations, not cause and effect. They do not establish that sleeping outside the 7–8-hour range causes dementia, or that adjusting sleep duration prevents it.

In particular, longer sleep could be a marker of underlying health changes rather than a cause of dementia. The review cannot settle the direction of that relationship or establish whether associations are independent of pre-existing health risks. The authors call for more studies with middle-aged adults, longer follow-up and tracking of changes in behaviour over time.

  • Physical activity: Across 49 studies involving 2,855,529 participants, regular moderate-to-vigorous physical activity was associated with lower incident dementia (RR 0.75, 95% CI 0.68–0.82). Results varied greatly across studies (I²=92.5%), so the pooled estimate should be interpreted with that high heterogeneity in mind.
  • Sitting: Sitting for 8 or more hours per day was associated with higher incidence (RR 1.27, 95% CI 1.17–1.39), but this estimate came from just three studies involving 295,809 participants. The authors’ risk-of-bias assessment found serious risk in most sedentary-behaviour studies.

Most sleep studies were assessed as having moderate risk of bias. The authors also note unanswered questions about dose-response and whether changing sleep, activity or sitting patterns changes dementia incidence.
