Yes. Removing names and other obvious identifiers can reduce the risk that health data will be linked to a person, but it does not always make the data impossible to identify. Re-identification typically requires a distinctive record, another source that connects a person to relevant details, and a way to match the two. The risk depends on the data, the recipient, and the information available to them.
How does health data get re-identified?
A record can stand out without containing a name. A combination of attributes—such as age, location, dates, or an unusual medical history—may be distinctive enough to match against another source. That source might be a public record, a dataset held by an organization, a news report, or information the recipient already knows.
Identification generally depends on three things working together:
- A distinguishing record: The released data contains a combination of details that sets one person apart.
- A source of identity: Another source links a person’s name or identity to some of those details.
- A way to connect the records: The recipient can match the details in the released data to the identity source.
A distinctive record alone does not necessarily reveal who it belongs to. If no relevant identity source is available, or the records cannot be reliably matched, the person may remain unknown. Conversely, information that seems harmless in isolation can become identifying when combined with other details.

Does removing names make medical data anonymous?
Not necessarily. Removing direct identifiers such as names is an important risk-reduction step, but other fields can act as clues. Combinations of demographic or geographic details, dates, rare diagnoses, unusual treatment histories, and uncommon events may distinguish a person.
Structured fields are only part of the picture
Clinical narratives can contain identifying context even when a dataset’s columns look clean. A note may describe a rare event, a distinctive sequence of procedures, or other circumstances that a person familiar with the patient could recognize. Under HIPAA Safe Harbor, the specified identifiers must be removed wherever they appear, including in unstructured text.
Data dictionaries and field provenance also matter. A field with an obscure name or shorthand value may still encode an identifier or reveal more than its label suggests. Reviewing only the visible column names is not enough to understand what a dataset contains.

Prior knowledge can change the risk
A recipient does not need a comprehensive public database if they already know relevant facts about a person. For example, someone who knows about a relative’s unusual series of procedures might recognize a record describing that history. A rare event reported in the media can also provide clues for a match.

In the United States, HIPAA provides two methods a covered entity may use to meet the Privacy Rule’s de-identification standard. They differ in how risk is assessed. Neither means that the data has literally zero chance of being linked back to a person.

Under either method, some residual risk remains. An Expert Determination is tied to the anticipated disclosure context: the data, recipient, available outside information, or release design may change. A conclusion for one release should not automatically be treated as valid for every later release.

HIPAA’s de-identification provisions apply within the Privacy Rule’s scope, including to covered entities and business associates. They should not be treated as a universal rule governing every health app, data broker, employer, or researcher.
Codes used to reconnect records
HIPAA distinguishes a re-identification code controlled by the covered entity from ordinary identifiers. A code used for possible later re-identification must not be derived from or translatable to the individual, and the mechanism for re-identification must be protected. HHS guidance also describes circumstances in which keyed cryptographic hash-derived values may be used under Expert Determination when the keys are not disclosed and the method meets the applicable requirements.

Risk reduction starts with the purpose of sharing and the recipient’s access—not with a one-size-fits-all list of fields to remove. NIST’s technical guidance describes several possible sharing models. Each offers a different balance of access, control, and analytical usefulness.

After selecting a sharing model, organizations can use a combination of data transformations and access controls. Common method families include removing direct identifiers, transforming quasi-identifiers, and generating synthetic data. Depending on the purpose and data, transformations may include generalizing details or suppressing selected values.
A practical review workflow
- Define the purpose and recipient. Specify the analysis the data must support and who will receive it or access it. The appropriate protections depend in part on the anticipated context.
- Inventory the fields and their origins. Review structured fields, free-text content, derived variables, and data dictionaries. Record what each field means and whether it can encode identifying details.
- Look for distinctive combinations. Assess combinations of demographic and geographic attributes, dates, rare cases, unusual histories, and other clues a recipient might recognize.
- Consider likely identity sources. Identify reasonably available outside information and recipient knowledge that could help connect a record to a person.
- Choose transformations and access controls for the use case. Remove or change identifying details as appropriate, and decide whether the release should be public, synthetic, query-based, or restricted to a protected environment.
- Review and document the resulting risk. Use an appropriate disclosure-review function and a measurable standard; document decisions and test the release in its intended context. Revisit the assessment when the data, recipient, outside information, or release design changes.
Tools that simply mask personal information are not necessarily sufficient for de-identification. NIST cautions that masking tools may lack the functionality needed for proper de-identification. A redaction feature should not be treated as a substitute for examining linkage risk and documenting the decisions made.

Reducing detail can lower the chance that records will be distinguished or matched, but it can also limit what researchers and analysts can learn. Generalizing dates or locations, suppressing uncommon values, or changing other attributes may make some analyses less precise or less useful.

The right balance depends on the release purpose, the recipient, and the risks associated with the data. A transformation that works for one analysis or sharing model may not work for another. For Expert Determination, analytical usefulness does not by itself demonstrate that the required risk conclusion has been met.
How is de-identification different from data security?
De-identification changes the data to reduce its identifiability. Security safeguards protect electronic protected health information (ePHI) from improper access or disclosure. These are related but separate protections, and an organization may need both.
Under the HIPAA Security Rule, regulated entities must use reasonable and appropriate administrative, physical, and technical safeguards for ePHI. HHS says that decisions about those safeguards should consider factors such as the entity’s size, infrastructure, costs, and the probability and criticality of risks. Access controls and other security measures do not, by themselves, make a dataset de-identified.
