Banks address ATM cash-out attacks by protecting the systems that control ATM withdrawals and by detecting suspicious transactions across their networks. In the attack described by the FFIEC, criminals compromise ATM-control access, change withdrawal settings, then use fraudulent cards to withdraw cash rapidly—sometimes at multiple ATMs at once. Preventing that sequence requires coordinated controls over employee access, ATM settings, card authorization, monitoring, and incident response.
How an ATM cash-out attack works
The FFIEC’s April 2, 2014 joint statement describes a scheme it calls “Unlimited Operations.” Attackers may first send phishing messages to bank employees and use malware to learn how the institution accesses web-based ATM control panels and to capture credentials. With that access, they can change withdrawal settings so transactions exceed customer account balances or normal control limits.
Attackers then use fraudulent debit, prepaid, or ATM cards made with stolen account information and PINs. The cash-out can involve large withdrawals at several ATMs in a short period. This is the sequence described in the 2014 warning, not a claim that every attack today follows the same path. The FFIEC cited a historical case involving more than $40 million in fraud using only 12 debit-card accounts; that example is not a current industry loss estimate. Read the FFIEC joint statement.
Which bank controls need to work together?
The FFIEC identifies four connected areas for institutions to assess: IT networks, card-issuer authorization systems, systems that manage ATM parameters, and fraud detection and response processes. OCC Bulletin 2014-13 similarly calls for including the threat in enterprise risk assessments and reviewing whether controls in those areas are adequate. See OCC Bulletin 2014-13.

- Employee and privileged access: Protect the credentials and access paths used to reach ATM-control systems, including against phishing and malware.
- ATM parameter management: Control who can change withdrawal limits and other ATM settings, and review the systems that apply those changes.
- Issuer authorization: Evaluate card transactions against the bank’s authorization controls, including limits and relevant account information.
- Detection and response: Monitor for suspicious activity, define how alerts are investigated and escalated, and coordinate operational action.
The regulator guidance names these control domains but does not mandate a particular product, authentication method, or universal transaction threshold. A bank’s risk assessment should determine how its controls fit together.
How monitoring can surface suspicious activity
Monitoring is more useful when it looks beyond one ATM or one withdrawal. FinCEN’s account-takeover advisory says ongoing monitoring can identify inconsistencies with a customer’s normal activity. Its examples include unusual ATM activity, geographically clustered ACH transactions, sudden wire transfers, and changes to customer or account profiles. Read FinCEN Advisory FIN-2011-A016.

These are indicators to investigate and correlate, not proof of fraud or a complete detection rule set. For example, an unusual ATM withdrawal may warrant review in context with other transactions or account changes. The advisory does not establish a single threshold that every bank must use.
Why response planning and ATM providers matter
Detection only helps if the bank can act on it. The FFIEC calls for reviewing fraud detection and response processes, while the OCC specifically advises community banks with ATMs to work closely with their service providers and ensure those providers are taking appropriate action. OCC Bulletin 2014-13 does not prescribe one incident-response sequence or staffing model; escalation paths and provider responsibilities therefore need to be established for each institution’s arrangements.

How independent ATM business risk differs
Due diligence on a customer that owns or operates independent ATMs is related to ATM risk, but it is not the same as defending a bank’s ATM-control panel from technical compromise. The FFIEC BSA/AML Manual says risk depends on the circumstances of the customer relationship. Relevant factors include transaction volume, ATM locations, and the source of cash used to replenish machines. Risk-based due diligence can help a bank understand ownership and management, operating controls, cash servicing, source of funds, locations, and expected activity compared with actual activity.
