Apple Watch Cardio Fitness is an estimate of VO₂ max, not a direct measurement. It can help you track broad changes over time, but an individual reading may differ by several mL/kg/min from a laboratory test. Apple’s validation results were stronger than those reported in two small independent studies, so there is no single accuracy percentage that applies to every wearer.
What Apple Watch Cardio Fitness measures
VO₂ max is the maximum amount of oxygen your body can use during exercise, expressed in milliliters per kilogram of body weight per minute (mL/kg/min). Apple Watch Cardio Fitness estimates this value from your heart-rate response to physical activity; it does not measure oxygen consumption directly.
Apple says Series 3 or later can generate an estimate during an Outdoor Walk, Outdoor Run, or Hiking workout recorded in the Workout app. Indoor workouts do not qualify. The supported estimate range is 14–65 mL/kg/min. The Health app displays readings and trends. Apple’s current support page, published September 15, 2026, says an initial estimate may require at least 24 hours of wear, several qualifying workouts, and passive measurements.

It describes conditions such as relatively flat outdoor terrain, sufficient exertion, and adequate GPS and heart-rate signals. The full current commercial formula is not disclosed, and the detailed technical paper describes a watchOS 7-era algorithm rather than confirming every later software version. Apple’s technical paper explains the validation and its stated measurement conditions.

How accurate is the estimate?
The available studies do not support one universal accuracy figure. Apple’s company-reported validation and independent laboratory comparisons differ in methods, participants, watch generation, and error measures.
| Study | Comparison and sample | Reported result |
|---|---|---|
| Apple, 2021 | Apple validation cohort; watch estimates assessed against its validation reference. | Mean error 1.4 ± 4.7 mL/kg/min; ICC 0.86. Apple also reported an error below 1 MET. |
| JMIR mHealth and uHealth, 2024 | 19 people; Apple Watch Series 7 estimate after outdoor running compared with a laboratory graded cycle test. | Mean absolute percentage error 15.79%; ICC 0.47 (95% CI 0.06–0.75). Laboratory mean was 45.88 and watch mean 41.37 mL/kg/min. |
| PLOS One, 2025 | 30 participants, predominantly highly fit; compared with indirect calorimetry. | Mean underestimation 6.07 mL/kg/min (95% CI 3.77–8.38); mean absolute percentage error 13.31% (95% CI 10.01–16.61); mean absolute error 6.92 mL/kg/min (95% CI 4.89–8.94). Limits of agreement: −6.11 to 18.26 mL/kg/min. |
The studies measure different things: mean error describes average bias, absolute error describes the typical size of a miss without regard to direction, limits of agreement describe the spread of differences, and ICC describes reliability or agreement within a sample. They are not interchangeable. Apple’s figures are company-reported validation, not a promise for each wearer. The independent studies are small and use different protocols, so their results should not be generalized to all Apple Watch users. The 2024 study found overestimation among lower-fit participants and underestimation among highly fit participants. The 2025 study also found average underestimation in its predominantly highly fit sample.

In Apple’s 2021 study, 78% of eligible outdoor pedestrian workouts longer than 5.75 minutes produced an estimate. Among participants who completed at least 10 qualifying workouts, 93% received at least one estimate within those first 10. Those are availability results from Apple’s study, not measures of accuracy for an individual reading.

What can affect your Apple Watch estimate?
Workout type, terrain, and signal quality
Only qualifying outdoor walks, runs, and hikes count. Apple’s technical paper identifies relatively flat terrain, adequate exertion, and good GPS and heart-rate signals as conditions for generating a useful estimate. Poor signal quality or an unsuitable workout may prevent a reading or weaken its usefulness.
Wear time and repeated workouts
Apple says the initial estimate can require at least a day of wear, several qualifying workouts, and passive measurements. Consistent all-day wear, beyond workout sessions, can help the estimate. Repeated outdoor pedestrian workouts make it more likely that the watch can produce estimates.

Health profile and medication
Age, sex, height, weight, and medications that may affect heart rate are among the factors Apple says it considers. Incorrect profile information can produce consistently inaccurate results. Apple also warns that heart-rate-limiting conditions or medications can lead to overestimation; recording relevant medication in Health may improve estimates, but the algorithm does not fully individualize effects and dose.
Extra effort or altered movement
A heavy backpack, carrying a child, walking on sand, using an assistive device, or pushing a stroller can change the effort or gait in ways the watch may not capture. That can affect accuracy or availability.

Short-term conditions and environment
Dehydration, caffeine, extreme heat, and a recent move to high altitude can raise heart rate and may lead to an underestimated reading. Pregnancy-related physiological changes may also make results inaccurate.
Health conditions and fitness level
Arrhythmias, pain, pacemakers or cardiac-assist devices, chronotropic incompetence, and conditions that limit walking can disrupt the relationship between heart rate and movement, reducing accuracy or preventing an estimate. Study findings also suggest that bias can vary with fitness level, but the small samples do not establish a universal direction for all users. If you have a relevant health concern, ask a clinician how to interpret the number.
Quick Recap
How to get a more useful trend
- In the Health app, check that your age, sex, height, and weight are correct. Record relevant medications that affect heart rate.
- Wear the watch consistently, then record repeated outdoor walks, runs, or hikes with a reliable heart-rate and GPS signal. Allow for several workouts before expecting an initial estimate.
- Compare readings over time under similar conditions. When a value shifts, consider unusual heat, illness, dehydration, altitude changes, carried loads, or medication changes.
- Treat the result as an estimate for fitness tracking. If an exact assessment matters, discuss clinical testing with a healthcare professional.
