For children with ADHD, evidence-based care is not a choice between an app and a prescription. It is a plan matched to the child’s age and needs, often combining caregiver training, classroom or educational support, and—when appropriate—medication. The American Academy of Pediatrics (AAP) sets out different recommendations for preschoolers, school-age children, and adolescents; the U.S. Centers for Disease Control and Prevention (CDC) explains parent training, while the UK’s NICE guideline describes a targeted role for cognitive behavioral therapy (CBT).
Which ADHD treatments are recommended at each age?
The AAP’s 2019 U.S. clinical practice guideline divides its recommendations into preschool (age 4 to the sixth birthday), elementary and middle school (age 6 to the twelfth birthday), and adolescence (age 12 to the eighteenth birthday). The recommendations below summarize that guideline; care should be individualized with the child’s clinician.

These recommendations come from the AAP’s 2019 clinical practice guideline. Its recommendations are not a numerical ranking of treatments or a one-size-fits-all prescription.
What does parent training in behavior management involve?
Parent training in behavior management (PTBM) is a structured intervention: a therapist or trained provider teaches caregivers strategies for supporting positive behavior and responding to problem behavior. It is not simply general parenting advice or a workbook to work through alone. The CDC describes it as an effective treatment and the recommended first choice for children under 6; the AAP also includes parent training in care for school-age children.

For young children in particular, the treatment focuses on skills caregivers can use in everyday interactions. The CDC distinguishes this approach from play therapy and talk therapy, which it says have not been shown to improve ADHD symptoms in young children. How do classroom interventions and school supports fit?
Behavioral classroom interventions address behavior in the learning environment. Educational interventions and individualized instructional supports address the child’s access to learning and participation. The AAP treats educational and instructional support as a necessary part of treatment for school-age children and adolescents, not an optional extra after clinical care. The CDC likewise identifies school as part of treatment planning.

In the United States, an IEP or Section 504 plan may be used to organize individualized school support. Which supports are appropriate depends on the child and the school context; the guidelines do not prescribe one universal plan. These terms are specific to the U.S. education system and should not be assumed to apply in other countries.
When is medication considered, and what does follow-up involve?
The AAP recommends FDA-approved ADHD medication alongside behavioral or parent-training intervention for children ages 6–11, and recommends it for adolescents with the young person’s assent. For preschoolers, the guideline allows methylphenidate to be considered when behavioral intervention has not led to significant improvement and moderate-to-severe functional disturbance continues. These are clinical recommendations, not instructions to choose a drug or dose without a prescriber.

Medication decisions and follow-up belong with the child’s clinician. The AAP recommends titrating medication to maximize benefit while keeping side effects tolerable. It also recommends screening for co-occurring emotional or behavioral, developmental, and physical conditions, since these can affect assessment and care. Families can discuss how treatment is affecting the child’s day-to-day functioning, any unwanted effects, and whether the plan needs adjustment.
When might CBT be useful?
NICE’s UK guideline says to consider a course of cognitive behavioral therapy for young people who have benefited from medication but continue to experience significant impairment in at least one domain. This is a targeted option for ongoing difficulties, not a general substitute for age-appropriate ADHD treatment. See NICE guideline NG87 recommendations.

How should families think about apps, games, and workbooks?
Apps, games, and books may be tools a family chooses to use, but they should not be treated as replacements for assessment, evidence-based treatment, or school support. The guidance described here centers on trained caregiver intervention, classroom and educational supports, and clinician-guided medication where appropriate. Neither the AAP nor CDC sources cited here endorse a particular retail workbook. A workbook can at most be an aid to caregiver learning alongside evidence-based training, not a substitute for a trained provider.

- Discuss the child’s functioning with a clinician. Describe where difficulties are affecting daily life and ask about assessment, including screening for co-occurring conditions.
- Match support to age and setting. Consider caregiver training, behavioral classroom intervention, educational supports, and medication according to the child’s age, needs, preferences, and the applicable guideline.
- Connect home, school, and clinical care. Where appropriate, coordinate with the school so educational or behavioral support addresses the settings in which the child is having difficulty.
- Review the plan over time. Discuss functional benefit, tolerability, and continuing impairment with the clinician; medication, if used, requires monitoring and adjustment.
The AAP describes ADHD as a chronic condition and recommends ongoing care rather than treating one intervention as a complete, one-time fix. The AAP guidance cited here is from 2019, CDC materials are U.S.-focused, and NICE NG87 is UK guidance. Local recommendations, services, and school systems can differ.
