FDA guidance calls for trained operators, hearing protection, seizure readiness, and other safeguards, while Mayo Clinic describes treatment in a health care provider’s office or clinic.
Why TMS should not be improvised at home
TMS uses magnetic fields to stimulate nerve cells in the brain. Repetitive TMS applies repeated magnetic pulses; Mayo Clinic says it is generally used for depression when other treatments have not been effective. Treatment requires more than placing a magnetic device near the head: the device must be used within its labeled indication, and clinicians must consider the person’s health history, potential risks, and the particular system’s instructions.
The FDA describes rTMS systems as Class II medical devices and its special-controls guidance covers device labeling, training, patient screening, adverse-event management, and safety controls. It calls for trained personnel, monitoring for seizures, and emergency equipment and procedures. Those safeguards are why a home setup—or an unverified consumer gadget marketed for stimulation—is not a substitute for supervised clinical care. What a supervised treatment course involves
For depression, Mayo Clinic describes a general schedule of sessions five days a week over four to six weeks. Individual protocols vary, and that schedule should not be treated as a prescription for any particular person. A clinician determines whether treatment is appropriate and follows the device’s labeling and safety procedures.

Because treatment is a course of repeated sessions, discuss practical access, the expected schedule, and how the clinician will assess your response before starting. Possible side effects and serious risks
Mayo Clinic lists scalp discomfort or pain, headache, tingling or facial-muscle twitching, and lightheadedness among common side effects. It also identifies seizures, mania, and hearing loss when ears are not adequately protected as uncommon but serious risks. The page notes that more study is needed to establish long-term side effects; it does not provide a numerical rate for these risks.

During rTMS, hearing protection is part of the safety procedure. FDA guidance says: “Patients and those operating the device should always wear earplugs or similar hearing protection devices with a rating of 30dB of noise reduction during rTMS treatment.” The clinic should specify or provide suitable protection. Earplugs do not make unsupervised TMS safe.
What to tell your clinician before treatment

- Implanted devices or metal in or near your head, including any electronic implant.
- A history of seizures or epilepsy.
- All medications you take.
- Pregnancy or the possibility of pregnancy.
- Relevant neurological or psychiatric conditions.
These are prompts for a clinical conversation, not a self-screening checklist. Risks and restrictions depend on the individual and the specific device, so do not decide on your own that an implant, medication, or condition is safe or disqualifying.

If you are considering TMS, ask a qualified clinician whether it is appropriate for your situation and indication. Useful questions include how the system’s labeled indication applies to you, what screening and monitoring will be used, how the clinic handles a seizure or other adverse event, what hearing protection it supplies, and what schedule, costs, and coverage to expect. Treatment options are not interchangeable; compare them with a clinician based on indication, evidence, personal risks, device compatibility, access, and cost.
