Human studies do not show that diet drinks reliably increase hunger or cravings. Appetite findings are limited, however, and cravings are not measured consistently enough to conclude that these drinks either cause or curb them. The answer also depends on what a diet drink replaces: sugar-sweetened drinks and water are not equivalent comparisons.
What studies say about hunger and cravings
Hunger is a subjective appetite measure; cravings are a separate experience. Neither should be inferred from calorie intake or body weight alone.
Hunger
A systematic review of trials involving blends of aspartame and acesulfame-K found no difference in subjective appetite in the four trials that reported it. The review could not pool those appetite results because the available reporting and data were insufficient (systematic review).
A 2025 review of randomized trials in weight-management programs found a small decrease in hunger during weight loss (standardized mean difference −0.16; 95% CI −0.30 to −0.01). During weight maintenance, it found no significant hunger effect (2025 review). These findings do not establish a reliable effect for every person or every diet drink.

Cravings
The available evidence does not establish that diet drinks cause cravings or suppress them. Craving outcomes are less consistently measured than appetite, so the lack of a clear result should not be read as proof of no effect in every context.
Why the comparison drink matters
A 2024 systematic review of adult randomized trials found different energy-intake results depending on the alternative beverage. Compared with sugar, non-nutritive sweeteners were associated with lower energy intake; compared with water, they were not associated with a statistically significant change. These are energy-intake findings, not direct measurements of hunger or cravings.

Both estimates are from the 2024 systematic review. They do not show that a diet drink changes appetite directly. Replacing a sugary drink may reduce energy intake relative to that sugary drink; choosing a diet drink instead of water has not shown a clear energy-intake benefit in these trials.
What weight-related evidence can—and cannot—tell you
The 2025 Dietary Guidelines Advisory Committee’s USDA/NESR review concluded that, in adults and older adults, low- and no-calorie sweetened beverages compared with water or lower consumption were not associated with changes in body composition or obesity risk. The evidence was graded moderate, and the review covered literature published from January 2000 through May 2023 (USDA/NESR review).

Body composition and obesity risk are not appetite ratings. These findings therefore do not prove that diet drinks leave hunger or cravings unchanged, nor do they show that the drinks control appetite. The review’s evidence cutoff also does not include studies published after May 2023.
Why findings may differ
- Different alternatives: A diet drink substituted for a sugary beverage poses a different question from one compared with water.
- Different outcomes: Hunger ratings, cravings, calorie intake, and body weight measure different things.
- Different time frames: A short-term appetite test is not the same as repeated use over weeks or months.
- Different drinks and participants: Sweetener type, formulation, population, and whether someone is losing or maintaining weight can affect what a study tests.
For example, the 2025 weight-management review reported a small hunger decrease during weight loss but no significant effect during maintenance. That is not a contradiction so much as evidence that the setting and outcome matter.

How to apply the evidence
If you enjoy diet drinks, the evidence does not give a reason to assume they will make you hungrier or trigger cravings. It also does not justify treating them as a dependable appetite-control tool. If your practical choice is between a sugary drink and a low- or no-calorie sweetened one, trial evidence supports lower energy intake with the sweetened alternative on average; if you are comparing it with water, a clear energy-intake advantage has not been established.
One behavioral weight-management trial compared non-nutritive sweetened beverages with water among UK adults with overweight or obesity in a structured program. High attrition limits certainty and how broadly the result applies; it is not a universal test of ordinary diet-drink use (USDA/NESR SWITCH trial summary).

“Diet drinks” also covers different sweeteners and products. Findings about an aspartame/acesulfame-K blend or a particular study setting should not automatically be generalized to every beverage.
