It is the most widely used and studied form, and a 2022 critical review found substantial evidence for its bioavailability, efficacy, and safety—but no evidence that other forms are more bioavailable, effective, or safer.
Why monohydrate is the evidence-backed default
The NIH Office of Dietary Supplements (ODS) identifies creatine monohydrate as the most widely used and studied form. A 2022 critical review concluded that monohydrate is the only form with substantial evidence supporting bioavailability, efficacy, and safety. That does not mean every alternative has no research; evidence for alternatives is limited or mixed, and none has established an overall advantage over monohydrate.
ODS relays an International Society of Sports Nutrition position describing creatine monohydrate as the most effective nutritional supplement currently available for enhancing high-intensity exercise capacity and lean body mass during exercise. This is ISSN’s statement as quoted by ODS, not a separate finding by ODS.

Monohydrate is 88% creatine by weight, according to NIH ODS. When comparing products, focus on whether the ingredient is actually creatine monohydrate and compare the price for an equivalent serving; a different form or format is not proof of better results.

What dose protocols do NIH ODS describe?
NIH ODS describes two approaches for adults. These are protocols presented in its fact sheet, not personalized medical or training advice.
- Loading followed by maintenance: 20 g per day for 5–7 days, split into four 5 g portions, followed by 3–5 g per day.
- Daily use without loading: about 3–6 g per day for 3–4 weeks to produce ergogenic effects.
What to know about side effects and safety
Weight gain is the most consistently reported effect of creatine use. NIH ODS attributes it to water retention and possibly muscle-protein synthesis. In some strength-training study contexts, participants gained 1–2 kg of total body weight in a month; that is a reported study outcome, not a guaranteed result for an individual.

NIH ODS also lists occasional nausea, diarrhea and related gastrointestinal distress, muscle cramps, and heat intolerance. These effects are not inevitable, and the fact sheet does not establish that every user experiences them.
Safety guidance differs in emphasis. NIH ODS says evidence indicates several years of use is safe. The National Center for Complementary and Integrative Health (NCCIH) notes reports of possible liver and kidney effects, advises people at risk of kidney problems to consult a health care provider and be monitored, and says long-term effects have not been well studied. NCCIH also says there are no data documenting safety in children or adolescents and cites warnings from pediatric and sports-medicine organizations against teen use of performance-enhancing supplements. These statements address different populations and evidence summaries, so they do not support a blanket claim of universal safety. People with kidney concerns, and anyone considering creatine for a child or teenager, should seek qualified medical guidance.

Powder, capsules, or gummies?
Powder is a common product format, not a quality endorsement. A 2025 GSSI Sports Science Exchange article, citing Nutrition Integrated 2024 data, reports that three quarters of U.S. creatine products are ready-to-mix powders, 16% are capsules or tablets, and 7% are gummies. It also reports that 93% of ready-to-mix powders are unflavored. These figures describe U.S. product formats, not comparative effectiveness, product quality, or the value of any particular brand.
If convenience or taste matters, choose a format you will use consistently, but compare the ingredient panel and price per 3–5 g serving rather than assuming that a form or format commands a better result. The cited sources do not establish a current brand or price comparison.

How to choose what is worth buying
- Start with the ingredient: choose creatine monohydrate if you want the form with the strongest evidence base.
- Compare like with like: check the ingredient panel and calculate price per 3–5 g serving, not just the container price.
- Choose a usable format: plain, unflavored powder is a straightforward option; capsules or other formats may suit personal convenience, but their existence does not demonstrate greater efficacy.
- Do not pay extra for an unproven edge: the reviewed evidence does not establish that HCl, ethyl ester, buffered creatine, or other marketed forms outperform monohydrate.
