Contact Form V11 is a free, minimal contact-form template listed by Colorlib. Its described layout pairs form fields and a button with a separate information area that moves below the form on mobile. The preview shows a sent-message confirmation, but that screen alone does not verify delivery or a working backend.

It also displays “Your message was sent, thank you!” after submission. These are visible preview-interface elements, not evidence that a message reaches a recipient in a deployed site. The listing provides Download and Preview links, but the download endpoint returned a 404 when checked. License and listing dates
Colorlib’s listing states a CC BY 3.0 license. Because the package could not be retrieved, its included license notices could not be checked; review any notices shipped with a downloaded copy before adapting or redistributing it. The listing page is dated March 6, 2021, while a separate “Released” field says 2025. Those fields conflict, so neither should be treated as a confirmed release history.
