The available options do different jobs: older PC software can map joystick movement to MIDI, USB adapters can make some gameport joysticks appear as game controllers, and a separate circuit can connect MIDI instruments to an old sound-card gameport. Those are not interchangeable.
If you want musical control, you need a way to read the joystick and software or firmware that translates its movement into MIDI messages, plus a route to your MIDI destination. If you want to use a joystick for retro games, look for a gameport-to-USB HID adapter instead.
Which conversion do you need?

The distinction is important because a gameport connector does not guarantee one universal signaling method. A DB15 plug can carry analog joystick signals, but some later sticks use proprietary digital protocols. Compatibility depends on the joystick and the adapter or firmware.
Use legacy software to map a joystick to MIDI
The Converter 1.5 manual documents a software route for a computer that already has gameport input. It shows joystick A’s X axis assigned to pitch bend and its Y axis assigned to modulation-wheel controller data on MIDI channel 1. This is a historical software method, not evidence of current plug-and-play compatibility with modern computers.

The manual also cautions that gameport axis readings may be inaccurate, especially for pitch control, and that heavy MIDI or audio interrupt activity can affect accurate axis-range readings. This route is most relevant if you have a compatible legacy PC and can tolerate calibration and performance limitations.
Why a gameport-to-MIDI adapter is different
The mlaux “Game port to MIDI adapter” is for connecting MIDI devices through the MIDI-capable pins of an old sound-card joystick/gameport socket. Its documented design uses a DA-15 connector, two DIN-5 MIDI sockets, a 74HCT04 hex inverter, an H11L1M optoisolator, four 270-ohm resistors, a 0.1 μF capacitor, and a 1N4148 diode.

That circuit handles MIDI connections on the sound card; it is not a joystick sensor, a joystick-to-USB converter, or a device that maps stick movement to MIDI notes or controls.
DIY USB adapters usually target game controllers, not MIDI
The Necroware project documents a gameport-to-USB adapter built around an Arduino Pro Micro with an ATmega32U4. Its stated output is USB HID controller input. The parts list also includes a DB15 female connector, four 100 kΩ resistors, and a four-position DIP switch; firmware selects among supported joystick protocols. A Pro Micro’s USB capability alone does not make this a USB-MIDI converter: MIDI-capable firmware and a validated joystick-to-MIDI implementation would still be needed.

Compatibility is model-dependent. In the project’s description of the DB15 interface, four pins carry buttons and four carry analog axes, with analog voltage varying from 0 V to 5 V. Later gameport sticks may use proprietary digital protocols, so a matching connector is not proof that a given adapter will work. Check the project’s support information for your exact joystick before building.
Calibration is also part of the build. Necroware says to center analog axes when the adapter initializes over USB. The project further warns that the MicroUSB board version has a fragile connector and recommends its USB-C board variant.

A ready-made USB HID option for retro gaming
raphnet describes a PC gameport-to-USB adapter for standard controllers with two axes and four buttons. Its product page states support for Windows, Linux, and Mac OS X, and describes low-latency operation and auto-calibration. The calibration instructions say to connect the joystick before USB, hold it centered for ten seconds, then move it through its full range.
The page reports the adapter as out of stock and displays $27.99, while noting that prices can change. Treat it only as an adjacent option for USB game-controller use: the page does not claim MIDI output.

Choose the right path
- For retro gaming: use a gameport-to-USB HID adapter that explicitly supports your joystick’s protocol.
- For musical control on a legacy PC: the documented software route can map axes to MIDI messages, but expect calibration and accuracy limitations.
- For MIDI instruments on an old sound card: a gameport MIDI interface may provide the relevant MIDI connections, but it will not turn joystick movement into MIDI.
- For a standalone USB-MIDI converter: the sources reviewed do not verify a finished device or maintained turnkey design that performs this exact conversion.
When assessing a future design, check which joystick protocols it supports, how many axes and buttons it reads, whether its USB output is MIDI or HID, how it calibrates, and whether it needs host software to bridge the signal to a MIDI destination.
