Not yet, based on the human evidence described here. Optogenetics has helped researchers test how specific cells and neural circuits affect behavior and disease, and those findings may guide treatments using other technologies. A 2021 report showed partial visual-function recovery in one person after an optogenetic intervention targeting the retina. That is an important human proof of concept, but it does not demonstrate treatment of a disorder in the brain.

What optogenetics does

Optogenetics combines genetic targeting with light-sensitive proteins, called opsins, so selected cells can be activated or inhibited with light. Researchers can target cells by characteristics such as their location, connections, or gene expression, then use light to test what those cells or circuits do. This makes optogenetics useful for investigating whether a particular circuit contributes causally to a behavior or disease.

There are two different routes from this research toward treatment. Direct translation means putting optogenetic components into a person and using light as part of the intervention. Indirect translation means using what optogenetic experiments reveal to select or refine a treatment that uses another modality, such as electrical stimulation or medication. A treatment informed by optogenetics is not itself optogenetic therapy. Lüscher and colleagues’ 2025 Nature Neuroscience roadmap notes that “Many of these translational pathways do not rely on the direct application of optogenetics in humans.”

What has been tried in people

A retinal proof of concept, not a brain treatment

In 2021, researchers reported partial visual-function recovery in one blind patient with late-stage retinitis pigmentosa. The intervention used an intraocular adeno-associated viral vector to deliver the gene for the light-sensitive opsin ChrimsonR. Engineered goggles detected changes in incoming light and projected light pulses onto the retina, activating retinal ganglion cells that expressed the opsin. The result was partial recovery, not normal vision.

The report in Nature Medicine was a case within an ongoing phase 1/2a study. A single case can show that an approach is feasible enough to investigate, but it cannot establish how well it works across a population or whether it is a routine treatment. Although the retina is neural tissue and part of the visual system, this intervention targeted the eye. It does not show that opsins can be delivered safely and effectively to the human brain to treat a neurological or psychiatric disorder.

How the approaches differ

Why direct treatment of the brain is difficult

Moving from circuit experiments to a useful brain intervention involves more than finding a cell type that responds to light. The 2025 roadmap identifies several challenges for direct translation:

  • Choosing a suitable target: The disorder needs a sufficiently well-defined cell population or circuit whose manipulation is expected to help.
  • Achieving specificity: Gene delivery and light exposure must reach the intended cells without producing unwanted effects in other cells or circuits.
  • Delivering light to the target: Light must reach the relevant neural tissue at a useful level. Implanted optical fibers are used in research, but their use does not establish a practical clinical light-delivery system for every brain target.
  • Addressing duration, safety, and regulation: A gene-based intervention may be difficult to reverse, making safety and regulatory considerations central to clinical development.
  • Building the right evidence: A mechanistic finding in animal research, or an early human result in a different organ, cannot by itself establish safety or effectiveness for a human brain disorder.

What circuit research may contribute now

The most immediate translational contribution is mechanistic: optogenetics can help researchers identify cells and circuits involved in symptoms, which may help them choose targets for other treatment technologies. The NIH BRAIN Initiative describes support for first-in-human trials of invasive and non-invasive central nervous system technologies, including circuit-level activation. That program description does not establish that those trials use optogenetics, so it should not be taken as evidence of a direct optogenetic brain therapy.

For a patient or caregiver, the practical distinction is whether a proposed intervention actually uses light-sensitive proteins and light to control cells, or whether optogenetics only helped researchers understand the target. The evidence summarized here supports the latter as a research contribution; it does not establish direct optogenetic treatment for a human brain disorder.
