The best PC city-building game depends on what you want to manage: a modern metropolis, a network of production islands, a settlement fighting to survive, or a single busy institution. For hands-on contemporary planning, start with Cities: Skylines II. Choose Anno 1800 for trade and supply chains, Frostpunk 2 for survival and political pressure, or Two Point Hospital for focused tycoon management.
Which city-building game should you play?
Use the kind of decisions you enjoy to narrow the choices. These games share building and management, but their core loops are different: some emphasize freely shaping streets and districts, while others make growth depend on production, survival, politics, or running a particular business.

It is the closest fit here to an open-ended urban-planning sandbox: your attention stays on how the city is arranged and how its systems respond to change. Anno 1800 — for cities powered by trade
Anno 1800 suits players who want settlement growth to depend on managing island economies. Manor Lords — for medieval settlement growth
Manor Lords offers a medieval setting in which resource management and battles accompany the growth of a settlement. Two Point Hospital — for running one institution
Two Point Hospital is an adjacent tycoon option, not a city builder. Its narrower scope is the point: you manage a hospital instead of coordinating an entire metropolis. Choose it when you want the feedback loop of running a complex institution without taking on city-wide traffic, zoning, and services.
