Important correction: I did not personally test these watches, so this is not a first-person lab review. The comparison below is based on current FDA records, manufacturer instructions, and American Heart Association guidance. That distinction matters because a watch that displays a blood-pressure number may be measuring it, estimating it, or merely flagging a possible pattern.
For a watch that actually takes a blood-pressure reading, the Omron HeartGuide is the clearest choice because it uses an inflatable cuff in the band. The Hilo Band/Aktiia G0 is the most notable current U.S.-cleared cuffless option, but it still requires a separate cuff and daily calibration. Samsung’s Galaxy Watch BP feature is useful for tracking estimates, while Apple Watch provides hypertension notifications rather than systolic and diastolic readings.
Blood-pressure watches are not all doing the same thing
Before comparing models, separate these three technologies:

1. Omron HeartGuide: best direct-reading blood-pressure watch
The HeartGuide BP8000-M and BP8000-L are wrist-worn monitors with an inflatable cuff built into the watch band. That makes the HeartGuide fundamentally different from watches that infer BP from optical signals.
It is the best fit for someone who specifically wants a watch-shaped device that performs a blood-pressure measurement rather than simply estimating one. The trade-off is size: the cuff mechanism makes it bulkier than a conventional fitness watch. It is also a spot-measurement device, not a lightweight watch that continuously measures BP throughout the day.
What to know before buying
- Measurements transfer automatically to the companion smart-device app.
- The historical readings are not viewable on either HeartGuide display; use the app instead.
- Keep the watch within 16 feet (5 meters) of the smart device while transferring a reading.
- Choose the correct model and band fit: Omron currently lists BP8000-M and BP8000-L support.
Verdict: Choose HeartGuide when a direct inflatable-cuff measurement is more important than smartwatch thinness. Do not buy it expecting continuous cuffless tracking or the same comfort as a standard Galaxy Watch or Apple Watch.
2. Hilo Band/Aktiia G0: best current U.S.-cleared cuffless system
The FDA cleared Aktiia’s G0 Blood Pressure Monitoring System, clearance K250415, on July 2, 2025. In the United States, the product is marketed through Hilo as the Hilo Band and includes the band, Hilo App, and Hilo Cuff.
The wrist band uses pulse-wave information for routine readings, but the system is not completely independent of a cuff. The user must calibrate it with the supplied oscillometric cuff every 24 hours. During routine use, the band can take readings without inflating a cuff, which is why it is reasonably described as cuffless for those readings—not as a cuff-free system.
Important FDA limits
- Adults aged 22 to 59.
- Wrist circumference of 14 to 21 cm.
- Home use.
- Calibration with the oscillometric cuff every 24 hours.
Those limits rule it out for many buyers. It is also not a license to diagnose hypertension or change medication without clinical advice. The setup flow is to obtain the kit, pair the Band and Cuff in the Hilo app, and complete calibration. Check the current U.S. product terms and manual before purchasing because availability, subscription terms, and supported regions can change.
Verdict: Hilo is the most important option for readers who want frequent cuffless readings and fit the cleared population. It is not the right recommendation for someone who wants a no-cuff, no-calibration wearable.
3. Samsung Galaxy Watch: best BP estimator for compatible Android users
Samsung’s U.S. blood-pressure feature began a phased rollout on March 31, 2026. It works with Galaxy Watch4 and later released models, excluding Galaxy Fit, when the watch runs Wear OS 4.0 or later and is paired with an Android 12-or-newer compatible Galaxy smartphone. Samsung currently promotes the feature on models including the Galaxy Watch7, Watch8, and Watch8 Classic.

This is an estimate, not a direct cuff measurement. Before the first use, calibrate the watch with a separate upper-arm cuff. Samsung requires three cuff measurements within 30 minutes and recalibration every 28 days.

- Open Samsung Health Monitor on the phone.
- Tap Blood pressure at the bottom.
- Tap Calibrate the watch.
- Take three cuff measurements within 30 minutes and enter the results as prompted.
Taking a reading on the watch
- Open Samsung Health Monitor on the watch.
- Tap Blood pressure.
- Tap Measure.
- Read the positioning prompt and tap OK.
- When the measurement finishes, review it and tap Done.
Samsung lists a measurement range of 70–180 mmHg systolic and 40–120 mmHg diastolic. Its calibration range is narrower: 80–170 systolic and 50–110 diastolic.

Why Samsung readings can fail
Calibration drift is the main practical issue. Changing wrists, wearing the watch at a different tightness, skipping the 28-day recalibration, moving during a reading, or using poor cuff technique can make the estimate unreliable. Samsung also identifies dirt on the sensor, lotion, sweat, skin characteristics, and poor positioning as factors that can affect the optical signal.
The manual recalibration path is Samsung Health Monitor → Blood pressure → Recalibrate → Recalibrate the watch. Menu wording may vary slightly with regional software.
Verdict: Galaxy Watch is the most convenient choice here for a compatible Galaxy-phone owner who wants BP estimates alongside normal smartwatch functions. Treat the feature as wellness tracking, not as a replacement for an upper-arm monitor.
4. Apple Watch: best for possible-hypertension notifications
Apple’s Hypertension Notifications feature is easy to misunderstand. It does not measure or display systolic and diastolic blood pressure. Instead, it analyzes optical heart-sensor data over a 30-day evaluation period and looks for patterns associated with possible chronic hypertension.
Supported hardware is Apple Watch Series 9 or later and Apple Watch Ultra 2 or later. Apple Watch SE is not supported. The paired iPhone must be an iPhone 11 or later running current software. The feature is intended for people aged 22 or older who have not already been diagnosed with hypertension and is not intended for use during pregnancy.
Turn on Hypertension Notifications
- Open the Health app on the iPhone.
- Tap your picture or initials in the upper-right corner.
- Tap Health Checklist.
- Under Hypertension Notifications, tap Set Up.
- Follow the instructions and tap Done.
If Apple sends a notification, it prompts you to use a third-party cuff and record BP twice daily for seven days. To start the log, open Health → Search → Heart → Blood Pressure → Blood Pressure → Get Started.

Verdict: Apple Watch is a good screening and awareness tool in the Apple ecosystem, but calling it a blood-pressure monitor is inaccurate. It is not a substitute for a cuff and cannot detect every case.
What I would not recommend
Avoid generic marketplace watches that display BP numbers without a named FDA clearance or authorization record for that exact feature and device. A number on a screen is not evidence that the sensor can produce a clinically useful measurement.
The FDA warned in September 2025 about unauthorized smartwatch and smart-ring software features that claim to measure blood pressure. Inaccurate readings can cause someone to miss needed treatment, seek unnecessary intervention, or change medication inappropriately.

Also avoid treating ordinary Garmin, Fitbit, Pixel Watch, or Apple Watch “estimates” as interchangeable with a validated BP monitor. The specific hardware, software version, intended use, and regulatory status all matter.

The American Heart Association recommends an automatic, upper-arm cuff-style monitor for home measurement. Wrist and finger monitors are generally less reliable, and the cuff must fit your upper-arm circumference.
Use the same cuff technique whether you are checking a watch estimate or confirming a high reading:
- Do not smoke, consume caffeine, or exercise for 30 minutes beforehand.
- Sit quietly for five minutes.
- Support your back, keep both feet flat, and rest your arm at heart level.
- Take two readings one minute apart and record both.
- Bring the home monitor to a medical appointment for comparison with the office equipment—at least annually or as the manufacturer recommends.
A watch can make logging easier, reveal patterns, or prompt you to check your BP. It should not be the device that determines whether you start, stop, or change treatment.
