Apple Watch Cardio Fitness is an estimate of VO₂ max, not a direct measurement. The watch infers it from your heart-rate response and motion during certain workouts. A cardiopulmonary exercise test (CPET), by contrast, measures oxygen in the air you breathe in and out while you exercise. Both relate to aerobic fitness, but they use different methods and serve different purposes.
What VO₂ max means
VO₂ max describes the maximum amount of oxygen your body can consume during exercise. It is commonly expressed relative to body mass, in milliliters per kilogram per minute (mL/kg/min). Apple uses the term for its Cardio Fitness feature, but the Watch does not directly measure oxygen consumption.
How Apple Watch estimates Cardio Fitness
Apple says Apple Watch Series 3 or later can estimate VO₂ max from heart-rate and motion sensor data during an Outdoor Walk, Outdoor Run, or Hiking workout in the Workout app. The calculation also takes account of age, sex, weight, height, and medications that might affect heart rate. Activity and signal conditions
Not every workout produces an estimate. Apple’s HealthKit documentation says qualifying outdoor activity should be relatively flat, with adequate GPS and heart-rate signal quality and sufficient exertion. It describes a grade of less than 5% incline or decline and a heart rate of approximately 130% or more of resting heart rate as conditions for generating samples. Indoor workouts and push workouts are not supported for this feature.

Getting an initial reading
Apple says to wear the Watch for at least 24 hours and complete several qualifying workouts before expecting an initial estimate. The first workout itself does not create a HealthKit VO₂ max sample. Once readings are available, find them in Health > Heart > Cardio Fitness; the Health app offers daily, weekly, monthly, six-month, and yearly views. Apple may also notify you if a result is low for your age and sex. That notification is a feature indicator, not a diagnosis.
How a clinical VO₂ max test differs
In a maximal CPET, the person exercises on a treadmill or stationary bike while intensity increases every few minutes, usually until exhaustion. A face mask lets the testing equipment directly analyze oxygen in inhaled and exhaled air. “VO₂ max test” can also refer to a submaximal protocol that estimates the value without pushing someone to maximal exertion. Apple notes that such methods are often used because they can take less time, cost less, and be more comfortable than maximal CPET. So when comparing results, distinguish a direct maximal CPET measurement from a prediction based on submaximal exercise.

How accurate is Apple Watch VO₂ max?
The Watch provides a prediction, not a lab-equivalent reading. Apple’s documentation explains the inputs and activity conditions, but the available product information does not establish one error margin that applies to every Watch model and wearer. Accuracy should not be assumed to be identical across people or interchangeable with a CPET result.

Apple also warns that some heart-rate-limiting conditions or medications may lead to an overestimate. If your heart-rate response is affected, or you need a value for a clinical decision or formal performance assessment, discuss the appropriate measurement method with a qualified professional rather than treating a Watch estimate as a substitute.

Apple’s current consumer support page lists a supported Cardio Fitness range of 14–65 mL/kg/min. Its HealthKit developer documentation lists 14–60 mL/kg/min. These Apple product-documentation ranges differ; neither should be read as a definition of healthy or unhealthy VO₂ max. Apple says its Cardio Fitness classifications are available for adults aged 20 and older.
