Unlike many food allergies, which usually cause symptoms within two hours, AGS reactions commonly begin 2–6 hours after exposure to mammalian meat or another product containing alpha-gal. That delay can make the cause hard to recognize, but timing and symptoms alone cannot diagnose AGS; a clinician must interpret them alongside the person’s history and test results.
What are the symptoms of alpha-gal syndrome?
AGS can affect the skin, digestive system, breathing, and circulation. Symptoms may include:
- Hives or an itchy rash
- Nausea, vomiting, severe stomach pain, heartburn, indigestion, or diarrhea
- Cough, shortness of breath, or difficulty breathing
- Swelling of the lips, tongue, throat, or eyelids
- Low blood pressure, dizziness, or faintness
- Anaphylaxis, a potentially life-threatening allergic reaction
Some people have gastrointestinal symptoms without obvious skin symptoms. Reactions can also vary from one exposure to another, and they do not necessarily happen after every exposure. A symptom pattern by itself cannot establish the cause.
How is alpha-gal different from a food allergy?
AGS is itself a food allergy, but it differs from many familiar food allergies in the allergen involved, its usual context, and the delay before symptoms begin. Many common food allergies involve proteins in foods such as milk, egg, peanut, wheat, soy, fish, shellfish, sesame, or tree nuts. AGS is an IgE-mediated reaction to alpha-gal, a carbohydrate found in most mammals but not people; in the United States, it is most often associated with bites from lone star ticks.

Symptoms overlap, so neither a delayed reaction nor a familiar allergy symptom proves which condition is responsible. The AAAAI notes that alpha-gal symptoms may be delayed by 4–8 hours, while the CDC describes the usual interval as 2–6 hours. Reported timing varies; the CDC range is a useful practical summary, not a diagnostic cutoff.
How long after eating does alpha-gal syndrome start?
Symptoms usually start 2–6 hours after exposure to an alpha-gal-containing product, according to the CDC’s symptom guidance. This is substantially later than the timing typical of many food allergies. Because the delay can span an evening, a person may go to bed feeling well and wake in the middle of the night with symptoms. The meal or exposure may not be the only possible explanation, however, and timing varies between people and reactions.

What can trigger alpha-gal reactions?
Alpha-gal is naturally produced in most mammals and is present in the saliva of some ticks. In the United States, the lone star tick is most often associated with AGS; a small number of cases have followed bites from blacklegged and western blacklegged ticks. A tick bite may occur weeks or months before symptoms begin.
Mammalian meat is a common trigger category. The CDC identifies beef, pork, lamb, venison, and rabbit among the red meats that may be relevant. Some people also react to dairy, gelatin, foods cooked with mammal fat, certain medicines, or other products containing mammal-derived ingredients. Others do not react to all of these. Do not assume every person with AGS must avoid every listed product: an allergist or other healthcare provider can help identify individual exposures and guide avoidance.

Can alpha-gal syndrome cause only stomach problems?
Yes. Some people with AGS have gastrointestinal symptoms without the more recognizable signs of allergy, such as hives. Stomach pain, nausea, vomiting, heartburn, indigestion, or diarrhea can also have other causes, including another food allergy, lactose intolerance, or a different gastrointestinal condition. Lactose intolerance is not an immune allergy and does not carry an anaphylaxis risk, but overlapping digestive symptoms mean that symptoms alone cannot distinguish it from AGS.
How is alpha-gal syndrome diagnosed?
Diagnosis is a clinical assessment, not a blood-test result in isolation. The CDC describes an evaluation involving a detailed patient history, physical examination, and a blood test for alpha-gal-specific IgE; clinicians may also use allergy skin tests. The history can help establish whether symptoms fit a delayed reaction and whether tick bites or outdoor exposure may be relevant.

A positive alpha-gal-specific IgE result does not, by itself, mean a person has AGS. Positive results can occur in people without the syndrome, particularly in places where lone star ticks are common. A clinician must consider test results together with symptoms and exposure history. The CDC/CSTE case definition is intended for public-health surveillance and does not replace an individual clinical diagnosis.

A severe allergic reaction or possible anaphylaxis requires immediate emergency care. Breathing difficulty, throat swelling, faintness, or rapidly worsening symptoms are reasons to seek emergency help rather than wait to see whether the reaction passes. The CDC advises immediate emergency care for a severe allergic reaction.

A healthcare provider may prescribe an epinephrine auto-injector or nasal spray and explain how to use it and what to do in an emergency. Anyone using prescribed epinephrine should still seek emergency care. Discuss an emergency plan with a healthcare professional rather than relying on symptom timing to judge severity.
How is alpha-gal syndrome managed?
Work with an allergist or other healthcare provider to identify relevant exposures, decide what to avoid, and learn how to read product labels. Some medicines or vaccines may contain ingredients of concern for some people; ask a clinician or pharmacist about a new medicine or vaccine. Do not stop a prescribed medicine or avoid necessary care on your own.
Preventing further tick bites is also part of CDC guidance. The CDC lists EPA-registered repellents containing DEET, picaridin, IR3535, oil of lemon eucalyptus, para-menthane-diol, or 2-undecanone, as well as clothing or gear treated with 0.5% permethrin. These measures reduce tick-bite risk; they do not treat AGS or guarantee that it will be prevented. New tick bites may reactivate reactions.
How common is alpha-gal syndrome?
The CDC identified more than 110,000 suspected U.S. cases from 2010 through 2022; that figure refers to suspected cases identified during that period, not a count of confirmed cases. In 2023, the CDC estimated that as many as 450,000 people in the United States might have been affected. The actual number remains unknown: AGS is not nationally notifiable, and some people may never be tested. These figures do not establish a precise prevalence or a global case total.
