AI governance often has policies, principles and oversight bodies on paper. The harder test is whether someone is clearly responsible for a system’s decisions, can intervene when risks emerge, and must explain what happened. Evidence from OECD governments suggests those operational controls lag behind governance structures. That makes accountability a major implementation gap—not a reason to neglect technical safeguards such as reliable data, accuracy, security and explainability.
What accountability means in AI governance
In practice, accountability means that identifiable people or bodies must answer for an AI initiative’s decisions and outcomes, and have the authority and evidence needed to review or change them. It is related to, but distinct from, responsibility, legal liability, transparency and technical performance.
Responsibility can be divided among people who build, procure, operate or review a system. Accountability requires clear ownership of the overall initiative’s quality and review, as well as defined responsibility for individual parts of its output. The OECD says: “Additionally, clear structures need to be in place to ensure appropriate accountability and oversight mechanisms — considering who is responsible for each element of the AI system’s output and who is accountable to the quality or review of outputs across the AI initiative.”
The OECD also writes that “Government AI systems should generally be answerable and auditable, which helps to reinforce the OECD AI principle on accountability.” These statements appear in its 2025 report, Governing with Artificial Intelligence: AI in regulatory design and delivery.

What the OECD evidence says—and what it does not
The OECD’s Digital Government Outlook 2026 reports results from its 2025 survey of government practices in 36 OECD countries. The figures show a gap between having governance structures and putting operational checks in place:

The contrast matters: an oversight body may offer guidance or monitor developments without having the authority or capacity to audit systems directly or enforce corrective action. OECD reports that guidance and monitoring were more common than hands-on audit and enforcement. A committee or advisory body is therefore not, by itself, proof that a system can be paused, changed or withdrawn.

These are reported practices in central government across the surveyed OECD countries—not worldwide adoption rates or measures of private-sector practice. They show a pattern in reported mechanisms, but do not prove that weak accountability caused any particular harm.
What accountable governance requires across an AI system’s lifecycle
1. Assign owners and decision rights
Name who approves a system, owns its risk decisions, reviews its outputs and responds when it fails. Specify who can approve, pause, modify or retire its use. Separate responsibility for individual system components from accountability for the initiative’s overall quality and outcomes. An advisory group can coordinate expertise, but it needs a clear route to someone empowered to act.

2. Connect assessment to ongoing review
A pre-deployment risk assessment is a starting point, not a permanent clearance. Document testing before use, monitor the system in operation, handle incidents and conduct post-deployment audits. Monitoring should be capable of surfacing changes in performance, data or context; incident procedures should identify who investigates and who decides whether use can continue. A one-time approval cannot address issues that arise later.
3. Make decisions answerable and auditable
Keep records sufficient to explain how a system was selected, tested, approved and used, and to support a justified review of its outputs. Logs can contribute evidence, but collecting logs alone does not show who made a decision, whether it was appropriate or what remedy followed. Documentation should support both internal scrutiny and any external review required in the relevant context.

4. Provide useful transparency and feedback
Where appropriate, explain what an AI system does, which institution is responsible and how a person affected by an outcome can ask questions or challenge it. Transparency is not merely publishing a technical description: it should help people understand where to take concerns. OECD survey results indicate that formal transparency standards and open algorithm registers were not common among the surveyed governments.
5. Equip people to perform oversight
Rules and assigned roles only work if staff have the skills and procedures to carry them out. In the OECD’s 2025 survey, 32 of 36 countries (89%) reported AI-skills training programs for government staff. Training is an implementation condition, not a substitute for decision authority, monitoring or audit.

Technology remains part of accountability
Accountability is not an alternative to sound engineering. Poor data quality, unreliable performance, weak security or systems that cannot be meaningfully examined can make responsible oversight impossible. OECD identifies data quality, explainability, accuracy, reliability and human oversight as relevant to accountable government AI use.
The practical distinction is between building safeguards into a system and ensuring someone is answerable for whether those safeguards work in real use. Technical controls can reduce or reveal risk; governance must assign people to assess the evidence, respond to failures and make consequential decisions.
A practical way to assess an AI governance approach
When evaluating a framework, policy or program, ask whether it provides:
- Identifiable owners for system components and the wider initiative, with explicit approval and intervention rights.
- Reviews that can change, pause or stop deployment—not only recommend improvements.
- Controls spanning development, deployment, operation and retirement.
- Ongoing monitoring, incident response and post-deployment audit.
- Records that support explanation and review, plus appropriate transparency and feedback channels for affected people.
- Staff with the skills and practical procedures to exercise oversight.
There is no universal framework ranking established by these measures; legal duties and appropriate controls depend on the system and jurisdiction. For voluntary implementation guidance, the NIST AI RMF Playbook organizes suggested actions around Govern, Map, Measure and Manage. It is guidance, not binding law. NIST says the page was updated June 10, 2026, and that it will be updated after AI RMF 1.0 is revised.
