An AI agent’s kill switch should do more than stop text generation: it must block further tool actions, limit what the agent can reach, and give an operator a way to assess effects that have already occurred. The reliable approach is layered—enforce policy at the point of execution, require approval for high-impact actions, contain the agent’s permissions, and plan recovery separately.

What an AI agent kill switch needs to stop

For a tool-using agent, stopping its response is not necessarily the same as stopping its work. A run may already have dispatched a tool call, changed data, or affected an external service. A practical interruption capability should stop additional tool dispatch, constrain or revoke the run’s access where needed, preserve its state and records, and define how people handle completed or partially completed actions.

OWASP’s official AI Agent Security Cheat Sheet recommends that users be able to interrupt and roll back agent operations. Those are related but distinct capabilities: interruption limits what happens next; rollback or compensation addresses effects that may already have happened. Approval classification also does not, by itself, authorize execution—the execution component still needs independent validation.

Where should an AI agent stop and ask for human approval?

Pause before an action that could cause material harm, expose sensitive information, make an irreversible change, or exceed the agent’s assigned scope. Set the threshold according to the consequences of the action rather than asking for approval on every low-risk step.

  • Require approval: for high-impact or irreversible operations, such as destructive changes or actions that affect external systems.
  • Allow within limits: for routine, reversible work that stays inside explicitly granted project and resource boundaries.
  • Deny by default: when the action is unknown, the policy check fails, or a reviewer cannot verify what is being approved.

A meaningful review shows the exact proposed action, target, and parameters. Bind the approval to those details, the actor, and a limited validity period; do not let a general “allow agent?” decision silently authorize different actions later. OWASP also recommends replay protection, step-up authentication for critical actions, and idempotency where possible.

Approval prompts can become routine rather than informative. Anthropic reports that Claude Code users approved roughly 93% of permission prompts, which it presents as evidence of approval fatigue in that product context—not as a rate for agent systems generally. Anthropic also reports that an OS-level sandbox approach reduced permission prompts by 84% in its Claude Code experience; that result is not a guaranteed reduction elsewhere. Enforce the policy at the tool boundary

Check a proposed tool call immediately before it produces a side effect. Validate the tool, arguments, actor identity, target, and permitted scope. Reject out-of-scope destinations and actions, and pause ambiguous or high-risk calls for review. If policy lookup, approval, or audit logging is unavailable, fail closed rather than execute without the required control.

This boundary matters in multi-step workflows. OpenAI notes that input guardrails run only for the first agent, output guardrails only for the final agent, and tool guardrails only on tools to which they are attached. A check at the beginning or end of a chain therefore does not automatically protect every intermediate tool call. Put validation beside the tool that can change data or affect an external system. OpenAI’s Agents SDK guardrails documentation describes these coverage limits.

For high-risk approvals, bind authorization to the normalized parameters and intended target, and use short-lived authorization so the decision cannot be replayed indefinitely. Where possible, make operations idempotent so retrying a call does not duplicate its effect. These controls strengthen an approval gate; they do not replace the gate itself.

Contain the agent’s reach independently

Assume that a prompt or approval process can fail. Limit the damage an agent could cause by restricting its identity, writable paths, project scope, and network access. The environment should enforce these limits even if the model proposes an action outside them.

  • Give the agent only the credentials and permissions required for its task.
  • Use a sandbox or virtual machine to confine files it can modify.
  • Restrict outbound network access to expected destinations; block or require review for unfamiliar ones.
  • Keep high-impact credentials and unrelated projects outside the agent’s accessible environment.

These are design principles, not copy-and-paste product settings. Anthropic describes a Claude Code sandbox configuration that permits reads, confines writes to the workspace, and denies network access by default. OpenAI describes sandbox boundaries for writable paths and network access, alongside managed policies for expected and unfamiliar destinations. What to do when a run is blocked or looks suspicious

  1. Stop further dispatch. Do not send additional tool actions for the affected conversation while the event is being reviewed.
  2. Do not blindly retry. A retry may repeat or compound an action that already completed.
  3. Preserve the evidence. Keep request IDs, responses, tool calls and outputs, approval decisions, and relevant application records.
  4. Route the event to an accountable operator. Review what ran, what was blocked, and which effects need remediation before resuming.

OpenAI’s misuse monitoring guidance recommends stopping further actions and preserving records when a request is blocked. Some OpenAI Agents SDK approval flows can pause execution, retain serialized run state, and resume the same run after a person approves or rejects a sensitive tool call. Callable approval rules fail closed when arguments cannot be safely inspected. This is a useful pause-and-review pattern, not proof of a universal emergency stop across platforms; see the Agents SDK human-in-the-loop documentation.

Plan rollback separately from interruption

A monitor can flag a concern after a tool action has completed. In documented OpenAI API contexts, misalignment monitoring is asynchronous; configured webhooks in some request modes send alerts without automatically stopping the conversation, and Chat Completions is not covered by that monitoring system. Even a blocked request does not undo earlier actions, as OpenAI explains in its monitoring guidance.

Design recovery around the side effects your application allows. Define transaction boundaries, keep backups where appropriate, use idempotency to reduce duplicate effects, and specify compensating actions for changes that cannot simply be reversed. Assign a human incident-review path. These are engineering measures to address the documented limits of interruption and monitoring; they are not automatic guarantees supplied by an alert or stop control.

Compare controls by what they actually do

Approval, sandboxing, monitoring, interruption, and rollback solve different problems. When assessing an agent design, compare controls on these practical dimensions rather than treating them as interchangeable kill switches.

Question What to establish
Where does it act? Before a tool executes, at the environment boundary, or asynchronously after behavior is observed.
Who enforces it? The agent, a tool wrapper, an independent policy service, or the operating environment.
What can it constrain? Tool types and arguments, identity and project scope, filesystem access, network egress, or rate and spend limits.
What happens on failure? The system pauses for review, denies by default, or continues while emitting an alert.
What happens to prior effects? Nothing automatically, application-managed compensation, or a separately verified transaction or recovery mechanism.

A strong design combines controls across these dimensions: execution-time checks prevent unauthorized actions; approvals add a decision point for consequential work; containment limits what remains possible; monitoring and preserved records support response; and separately designed recovery addresses completed effects.
