Daily PrEP is taken every day as prescribed. “On-demand” PrEP refers here to the specific 2-1-1 schedule using oral F/TDF around anticipated sex—not occasional use of any PrEP medication. In the United States, 2-1-1 is off-label, not FDA-approved, and not recommended by the CDC, although the CDC describes circumstances in which a clinician may prescribe it off-label for some adults. Which approach is appropriate depends on exposure route, health history, local guidance, and whether you can follow the schedule reliably; discuss options with a clinician before starting or changing PrEP.
What daily and on-demand PrEP mean
PrEP is medicine used to prevent HIV. Daily oral PrEP means taking a pill every day as prescribed. The on-demand option described in CDC guidance is an event-timed oral schedule called 2-1-1, using F/TDF (Truvada or generic equivalent). It is not a general instruction to take other PrEP medicines only when needed.
CDC says taking PrEP as prescribed is currently the only FDA-approved schedule for preventing HIV. The agency’s public guidance also says there is scientific evidence that 2-1-1 provides effective protection for gay and bisexual men having anal sex without a condom. Those statements describe different things: the first concerns FDA-approved schedules; the second describes evidence for a specific population and exposure.
How the 2-1-1 schedule works
The schedule is tied to F/TDF and to anticipated sex. CDC describes these doses:

- Take two pills 2–24 hours before sex.
- Take one pill 24 hours after the first two-pill dose.
- Take one pill 24 hours after the second dose.
These are exact, sequential timings—not a general “take a pill when needed” approach. The multi-step schedule may be difficult for someone who cannot anticipate or delay sex, or who may have trouble remembering the follow-up doses.
Who the evidence and guidance cover
United States: CDC guidance
The CDC says there is evidence of effective 2-1-1 protection for gay and bisexual men having anal sex without a condom. It says effectiveness is not known for vaginal sex or for HIV exposure through injection-drug use. In the United States, 2-1-1 is not FDA-approved and is not CDC-recommended. CDC clinician guidance nevertheless describes off-label prescribing for some adult gay and bisexual men who request non-daily dosing, have infrequent sex (for example, less than weekly), and can anticipate or delay sex to allow the first dose at least two hours beforehand. The same guidance says not to prescribe it to people likely to struggle with the complex schedule and to avoid it for people with active hepatitis B.

International guidance
Guidance can differ by country and issuing body. The World Health Organization published an update recommending event-driven oral PrEP as an alternative for men who have sex with men, as part of comprehensive prevention and sexual-health services. That WHO recommendation does not change the FDA or CDC status of 2-1-1 in the United States.
Daily oral PrEP options differ by exposure route
CDC describes daily oral options with different indications. This is why “daily versus on-demand” is not simply a choice between two interchangeable schedules.

CDC also lists cabotegravir (Apretude), an injectable PrEP medication given every two months, among FDA-approved options. It is not an event-driven version of oral 2-1-1.

These considerations can help structure a clinical conversation; they are not a self-screening tool or a prescription guide.

- Exposure route: Does the evidence and guidance for a proposed regimen cover your likely route of HIV exposure?
- Timing and frequency: Is sex infrequent and predictable enough to complete an event-timed schedule, or would a daily routine be more practical?
- Schedule reliability: Can you consistently manage both the pre-sex dose and the timed doses afterward?
- Health history: Discuss hepatitis B and kidney health. CDC warns that episodic F/TDF in people with active hepatitis B can cause hepatic flares; it also says oral PrEP is not appropriate for people with severe kidney impairment. A provider must assess whether a medication is suitable.
- Local guidance: Ask which options are approved or recommended where you receive care, since U.S. and WHO guidance are not identical.
- Routine preference: Consider whether you can maintain a daily habit or prefer a multi-step schedule tied to anticipated sex, while recognizing that preference alone does not establish that a regimen is appropriate.
Starting PrEP, adherence, and broader prevention
Before starting PrEP, a provider must confirm that you do not have HIV. Do not start, stop, or switch schedules without clinical advice. CDC says high adherence is required for daily oral PrEP to be effective. If prescribed daily pills, keep them in a secure, tightly closed container away from children, excessive heat, and moisture. A reminder routine may help some people keep track, but no particular accessory is required.
PrEP prevents HIV, not other sexually transmitted infections or pregnancy. CDC recommends considering it as part of a broader prevention plan and sexual-health care.
