An online screening can be a useful first step, especially when access is limited, but it cannot replace a diagnostic assessment under controlled listening conditions.
Screening or diagnostic evaluation: what do you need?
A screening is a preliminary check that can help you decide whether to seek care. It does not establish the cause, type, or full extent of hearing loss. A diagnostic evaluation can assess how much hearing loss is present, whether it differs by ear, and what kind it is. The 2024 age-related hearing-loss guideline identifies pure-tone audiometry as the gold standard for determining ear-specific severity and type.
The guideline also cautions that online tests cannot ensure sound-controlled conditions. Background noise, speakers or headphones, and the room itself can affect results. Treat a screen as a prompt—not a diagnosis—if symptoms continue or the result is concerning. Read the 2024 guideline from the American Academy of Otolaryngology–Head and Neck Surgery.
Which route should you choose?

Do I need a hearing test?
Consider arranging an evaluation if you often struggle to understand speech, ask people to repeat themselves, turn the television up louder than others prefer, strain to hear, or hear better in one ear. These are possible signs, not a diagnosis. The FDA advises considering a doctor or hearing professional for testing when hearing problems arise. See the FDA’s information on hearing loss.
NIDCD’s adult self-check suggests considering an audiologist or ENT if you answer “yes” to three or more questions. The questionnaire was adapted from a 1990 publication and is a screening prompt, not a diagnostic score. Review NIDCD’s self-check, last updated August 16, 2022.

What happens in a professional evaluation?
An audiological examination assesses the type and amount of hearing loss and can help determine appropriate next steps. Depending on the findings, those steps may include hearing care or referral for medical or surgical evaluation. A medical exam can check for potentially treatable causes such as infection, injury, deformity, or earwax.
When contacting a provider, ask which tests are included, whether results will be assessed for each ear, who will explain them, and how the clinic handles referrals if a medical cause may be involved. For a remote appointment, ask whether the service can complete the assessment you need or whether an in-person test is part of the plan. FDA guidance explains the role of an audiological examination.

Can I take a hearing test online?
Yes, you can use an online screening as a convenient first check. Its result is limited by the listening environment: a typical home setup cannot guarantee the controlled sound conditions used for a diagnostic assessment. If you have persistent difficulty or the screen concerns you, arrange professional follow-up rather than relying on repeated online tests.
How to compare providers before booking
- Credentials: Ask who will perform and interpret the test, and verify that the person is licensed where you live.
- Diagnostic scope: Find out whether the appointment is a screen or a diagnostic evaluation, and what testing is included.
- Conditions and access: Ask whether the assessment is in person or remote and whether the setup can provide the testing you need.
- Follow-up: Clarify how results are explained, what happens if the test finds a concern, and whether referrals are available.
- Local cost and coverage: Request the fee and check with your insurer or relevant coverage provider. Costs and coverage vary by location and provider; there is no single general price that applies everywhere.
Is getting a hearing test the same as getting hearing aids?
No. Testing helps assess hearing; hearing aids are treatment devices. In the United States, adults aged 18 or older who perceive mild to moderate hearing loss may buy over-the-counter hearing aids without an audiological exam. That option does not identify the cause or confirm the type and degree of a person’s hearing loss. FDA says OTC hearing aids are not intended for children or people with severe or profound hearing loss, who need professional care appropriate to their circumstances. For prescription hearing aids, the FDA says: “The FDA does not require a medical examination or hearing test for prescription hearing aids, but your State may require you to visit an audiologist or other licensed professional first.” Requirements can therefore depend on state rules. Buying a device, whether OTC or prescription, is not a substitute for finding out why you have hearing difficulty. Check the FDA’s guidance on getting hearing aids.
