A spider-web-inspired implant called TRAFFIC restored normal blood glucose in diabetic mice in an early study, and a separate experiment showed that a longer version could be removed laparoscopically from dogs. Those results make it a promising research device—not a proven diabetes treatment: the animal experiments were distinct, and the sources reviewed do not establish human efficacy, approval, or availability.

What is the TRAFFIC implant?

TRAFFIC stands for “Thread-Reinforced Alginate Fiber for Islets enCapsulation.” It is a polymer thread designed to carry insulin-producing islet cells inside a continuous coating of alginate hydrogel. The concept was inspired by water beading on spider silk, but the device is not made from spider silk.

In the original design, two sterile nylon sutures were twisted into a helix and folded to support nanoporous surface coatings. The thread releases calcium ions, which help form an alginate layer around it; that layer contains the islet cells. The porous, helical surface helps the hydrogel remain attached.

The coating is intended to let glucose, oxygen, nutrients, and insulin pass between the encapsulated cells and surrounding tissue while shielding the cells from immune attack. Unlike a large number of separate microcapsules, the cells are carried along one connected thread that could be grasped and retrieved as a unit. Cornell’s January 2018 report describes the original device and experiments.

Why use a continuous coating?

The spider-web image describes the initial inspiration: water droplets form beads along a strand. The researchers changed the design so hydrogel would cover the thread uniformly rather than sit in separate beads. In the report, Cornell’s Minglin Ma said, “You don’t have any gaps between capsules,” explaining that gaps could be undesirable in relation to scar tissue.

What did the animal experiments show?

The original mouse and dog experiments tested different outcomes. The mouse study examined blood-glucose control; the canine work tested whether a longer implant could be retrieved. The dog experiment did not demonstrate diabetes control.

The 2021 study is titled “Nanofibrous Encapsulation of Insulin Producing Cells for the Treatment of Type 1 Diabetes”. Cornell’s December 2023 account discusses that work and a separate follow-on strategy.

How might the thread protect islet cells?

Islet cells normally respond to rising blood glucose by releasing insulin. After transplantation, however, cells may be attacked by the immune system, and an implant also has to provide enough oxygen and nutrients for the cells to function. TRAFFIC’s alginate coating is intended to act as a protective barrier without blocking the exchanges needed for glucose sensing and insulin release.

That design goal does not mean immune-related risks have been eliminated or that every encapsulated cell will survive. Cornell’s 2023 account identifies long-term cell survival and oxygen and nutrient delivery as ongoing challenges for clinical application. Encapsulation, graft capacity, cell supply, and the duration of function are therefore important questions for future work, not settled advantages over other transplantation approaches.

Can doctors remove the implant?

Removability is a central feature of the concept. Because the cells sit on a connected thread, the implant is intended to be retrieved as one piece rather than collected as many dispersed capsules. Cornell reported laparoscopic removal of ten-inch devices in dogs. Veterinary surgeon James Flanders described using two small incisions, a camera port, and a grasping device to introduce and remove the implant.

That canine procedure supports the feasibility of retrieval in the tested animals; it does not establish how the device could be implanted or removed in people. Retrieval could matter if transplanted cells fail or if stem-cell-derived cells raise safety concerns, including the possibility of unwanted growth. Ma described removability as a design advantage, saying, “When they fail or die, they need to come out.” It is not evidence that all such risks have been solved.

Is the “string” available to people or a cure for type 1 diabetes?

No human efficacy, regulatory approval, or patient availability is established by the sources reviewed. Cornell’s 2023 report describes insulin injections or pumps as standard care and presents encapsulated-islet devices as research moving toward clinical application. TRAFFIC should not be treated as a current alternative to prescribed insulin or as a cure.

A separate approach called SHEATH should not be confused with the original TRAFFIC design. SHEATH uses a subcutaneous pocket that is first vascularized around temporary nylon catheters; encapsulated islet devices are inserted later. Cornell’s 2023 report describes procedural work in pigs and says additional challenges remain before long-term clinical use. This is a distinct strategy, not evidence that the original abdominal TRAFFIC implant has entered human treatment.

What would need to improve before clinical use?

  • Long-term cell function: The encapsulated cells must continue to survive and release insulin over a useful period.
  • Oxygen and nutrient delivery: Cells need an adequate supply through the surrounding tissue and coating.
  • Immune protection: The coating must protect the cells while still allowing the exchanges required for glucose sensing and insulin release.
  • Scale and capacity: A practical implant must carry enough functioning cells for its intended use.
  • Procedure and retrieval: Implantation and removal need to be workable and safe in people, not only in animal procedures.

The animal studies offer evidence for particular pieces of the concept—glucose control in mice and retrieval in dogs—but they do not answer these clinical questions as a whole.
