Clear aligners may be an option for some people with crowded, spaced, or misaligned teeth, but a condition that appears on a common-use list does not prove that aligners alone are right for your bite. Only a licensed dentist or orthodontist can assess your teeth, bite, dental history, and goals and recommend a suitable plan.
What clear aligners can—and cannot—treat
Clear aligners are prescription, custom-made removable trays, not a one-size-fits-all cosmetic product. The FDA describes them as commonly used for mild-to-moderate crowding, gaps, and bite problems. The American Association of Orthodontists (AAO) also lists overbite, underbite, open bite, crooked teeth, and crossbite among conditions aligners may treat. These lists describe possible uses, not a diagnosis or a guarantee that aligners alone will address an individual case.
More complex orthodontic conditions may call for braces or another approach. Some cases are more predictable with braces, and treatment can combine braces and aligners. A practitioner must decide which tooth movements are appropriate and achievable in your case.
An evaluation considers your teeth, bite, dental history, and treatment goals. The practitioner may use X-rays and scans or impressions to plan customized trays. Ask what findings support the proposed plan, which movements it includes, and what limitations may remain.

What treatment involves—and what it asks of you
- Evaluation and planning: A licensed practitioner examines your teeth and bite, reviews your history and goals, and determines whether aligners are appropriate.
- Records and custom trays: Scans or impressions are used to plan a series of trays made for your teeth.
- Daily wear and tray changes: The FDA says patients typically use each tray for one to two weeks and generally wear aligners at least 20 hours a day, or as directed by their practitioner. Follow your own prescription rather than treating these general figures as a substitute for instructions.
- Monitoring and retention: Your practitioner checks progress periodically. A retainer may be recommended after active treatment.
The FDA’s 2026 consumer comparison gives a broad clear-aligner treatment range of six to 18 months. That is not a promise or an individualized prediction: the time needed depends on the movements required and your treatment plan. Ask the practitioner who examines you for an estimate and what could change it.
Aligners or braces: how to compare the options
Neither option is universally faster, safer, or better. Compare how well each can achieve your specific tooth and bite movements, as well as what day-to-day care and follow-up involve.

Ask for a case-specific comparison that also covers expected duration, total cost, refinements, retention, and what happens if treatment does not progress as planned. No general price figure applies to every patient.
Risks, supervision, and remote care
The FDA lists several possible limitations and risks. Treatment may not progress as intended if you do not follow wear instructions; trays can be lost or damaged; and temporary speech changes, irritation, or allergic reactions can occur. Crowns, fillings, or implants may loosen or be dislodged and need repair or replacement. Reported adverse events can include swelling and cuts. Contact your practitioner about pain or a change in your bite.

In-person examination and periodic visits can help identify problems such as an uneven bite, unintended tooth movement, pain, or tooth loss. Virtual visits may be a useful part of some care plans, but remote monitoring within practitioner-supervised care is not the same as treatment without a prescription and professional supervision.
The FDA says clear aligners are prescription devices for use under a licensed practitioner’s supervision, and that it has not authorized clear aligners sold directly to consumers without a prescription and dentist or orthodontist supervision. It has received reports involving unauthorized direct-to-consumer aligners of new or worsening dental pain, jaw pain, bite problems, worsening misalignment, and new gaps. These reports do not establish how often problems occur. The ADA’s policy describes direct-to-consumer dental laboratory services as eliminating the dentist’s role in diagnosis, treatment planning, and treatment management; that is a professional-association policy, distinct from the FDA’s regulatory guidance.

Do not treat “FDA registered” as proof that a specific aligner product is FDA-authorized. The FDA explains that registration or listing is not the same as authorization. Ask your practitioner which product is being used; the FDA says consumers can search its 510(k) database using product code NXC.

- Which movements are realistic, and which parts of my bite or alignment may remain untreated?
- Would braces, a combined approach, or no treatment be reasonable alternatives? Why?
- What examination or records do you need before recommending treatment, and who reviews and approves the plan?
- How many hours a day should I wear each tray, when should I change trays, and how will you check progress?
- How often should I have in-person visits, and whom should I contact if a tray does not fit or symptoms arise?
- What are the likely duration and total cost, including refinements, retainers, and any correction or replacement that may be needed?
- Which aligner product will be used, and what is its FDA authorization status?
