The right option depends on the work, the body region needing support, the worker’s fit, and whether the device’s assistance suits the movements and conditions of the job. Treat either type as a possible way to address remaining ergonomic exposure—not as a replacement for improving the work itself. Current evidence does not establish that either category generally prevents workplace injuries.
What powered and passive mean
A powered exoskeleton uses actuators—such as electric motors, pneumatics, or hydraulics—to generate assistance. A passive exoskeleton has no powered actuators; it uses mechanisms such as springs or counterbalance forces, drawing on energy from the wearer’s movement. These labels describe how assistance is supplied, not how effective or suitable a device is for a particular job. NIOSH’s industrial exoskeleton bulletin and its occupational health equity review describe these categories and their limitations.
Industrial devices are also designed for different body regions and demands. Back-assist devices may be intended for certain lifting or static-holding tasks; shoulder and arm supports may target sustained overhead work or heavy tools. Tool-support and leg-assist devices address other demands. Start by identifying the task and the body region involved, rather than choosing by a powered or passive label.
How to choose for a specific task
Match the assistance to the movement
Ask whether the mechanism supports the posture and movement the job actually requires. Consider the body region, load, repetition, and time spent in the posture. A device suited to a sustained overhead position may not suit frequent reaching in multiple directions; a back-assist device intended for one kind of handling task should not be assumed to suit every lift. Follow the device’s instructions and assess assistance throughout the work cycle, not just in one favorable position.

Check mobility and surroundings
Workers still need to step, bend, reach, recover balance, and respond to changing conditions. Check whether the device’s bulk or restrictions interfere with those movements or with tools, equipment, and moving hazards. For powered devices, assess hazards and controls related to powered components as well. A device that helps in one posture may be a poor fit where the job requires rapid movement or frequent changes in position.
Evaluate fit during real work
Fit and usability affect both powered and passive devices. A size label alone does not establish that a device is comfortable or appropriate: fit can vary across workers and postures, and poor fit may encourage awkward positioning. Assess the candidate device dynamically with different users and representative work motions, checking pressure, comfort, and whether it stays compatible with the job.

Ask what the evidence measures
Do not infer effectiveness from the device category. Ask whether evidence concerns the same task and device, and distinguish short-term biomechanical measures such as muscle activity from actual workplace injury outcomes. NIOSH has called for further evaluation of exoskeleton effectiveness across industrial work and sectors before widespread implementation. The available evidence summarized by NIOSH does not show that powered or passive exoskeletons, as categories, prevent workplace injuries.

Potential benefits and trade-offs
Some laboratory studies have reported reduced muscle activity during particular tasks. NIOSH’s 2020 occupational health equity review reports back-muscle activity reductions of 10–44% during handling tasks, and hip-extensor activity reductions of 24% and neck-muscle activity reductions of 50% in laboratory-based tasks. These are study- and task-specific measurements, not guaranteed results for a workplace device and not evidence of equivalent reductions in injury rates.
Support may also shift demands rather than eliminate them. NIOSH identifies possible pressure wounds or compressed nerves with prolonged use, restricted mobility, changes in balance or center of gravity, hygiene concerns with shared devices, and load transfer to the lower back or legs. If a device enables a worker to hold a tool longer, exposure to vibration, noise, or respirable contaminants could also increase. Monitor the whole job, including exposures that may rise when a task becomes easier to sustain.

Important settings need additional checks
Construction and balance-sensitive work
A NIOSH bibliography published in 2026 summarizes a simulated elevated block-laying study in which the tested shoulder exoskeletons showed minimal and inconsistent reduction in shoulder strain, while balance decreased. This result is specific to the simulated task and tested devices; it is not proof about all shoulder exoskeletons or construction work. It does make balance and recovery from awkward positions important parts of a construction trial.
Healthcare and patient handling
Patient-handling tasks can involve unpredictable patient movement and geometry, tight spaces, medical equipment, infection-control requirements, and the need to protect patient comfort and safety. NIOSH notes that wearable robots are not expected to suit every patient-handling task. Its healthcare discussion frames them as a possible complement to safe patient-handling programs, with attention to rapid responses, fit for women workers, disinfection, and working around equipment.

A practical evaluation checklist
- Identify the residual exposure. Define the posture, movement, body region, load, repetition, and duration that remain after considering ways to redesign the task.
- Choose a candidate for that task. Check that its supported body region and assistance mechanism are intended for the work, and follow the device’s use instructions.
- Trial representative conditions. Have workers perform normal motions and relevant task variations in the actual work environment, not only a static fitting or demonstration.
- Check fit and movement across users. Assess pressure, comfort, reach, bending, stepping, balance, and the ability to recover or respond quickly.
- Monitor the whole exposure profile. Track discomfort, mobility, balance, transferred loads, and whether longer or easier task performance increases exposure to vibration, noise, or contaminants.
- Review safe use and upkeep. Address training, cleaning and hygiene, inspection, and maintenance in line with the device instructions and workplace requirements.
