A heart transplant replaces a failing heart with a donor heart. A durable left ventricular assist device (LVAD)—the type most people mean by a “mechanical heart pump”—supports blood flow while the patient’s own heart remains in place. Neither option is right for everyone: the choice depends on medical suitability, donor access, treatment demands, and the patient’s goals.

What is the difference between a heart transplant and an LVAD?

A heart transplant replaces the failing heart with a deceased donor heart. A durable LVAD is implanted to draw blood from the left ventricle and pump it into the aorta, helping the heart circulate blood. It supports the patient’s heart; it does not replace it or cure heart failure.

“Mechanical heart pump” can refer to more than one technology. This comparison is about durable LVAD therapy, not short-term pumps used in some emergencies or a total artificial heart, which is a different treatment. A VAD can support the left ventricle, right ventricle, or both in selected patients.

How do the treatments compare?

When might an LVAD be used instead of a transplant?

As a bridge to transplant or another decision

An LVAD may support circulation while a patient waits for a donor heart. It may also provide time for recovery or reassessment of transplant candidacy. Whether it changes a person’s eligibility or removes the need for transplant depends on that person’s clinical course and care-team assessment.

As destination therapy

Some people who are not suitable candidates for transplant may receive an LVAD as long-term destination therapy. An LVAD is therefore not always a temporary bridge, and receiving one does not mean that someone will later receive a transplant.

What ongoing care does each option involve?

After a transplant

Transplant recipients need lifelong immunosuppressive, or anti-rejection, medicines and ongoing follow-up. Monitoring addresses rejection, infection, graft dysfunction, and complications associated with long-term medicines, which can include kidney disease, diabetes, high blood pressure, osteoporosis, and cancer.

With an LVAD

LVAD care involves managing the device, controller and power supply, caring for the driveline, responding to alarms, and following an emergency plan. Patients usually also need anticlotting treatment. NHLBI patient guidance says, “You will need to take medicines exactly as your doctor prescribes to prevent clots.” The patient and caregiver need to understand the daily equipment routine and know whom to contact for device concerns.

Neither treatment is simply “easier.” One requires reliable medication use and monitoring; the other adds hands-on device and power responsibilities. The practical fit depends on the person, their support network, and the resources available through their care team.

What are the risks?

Transplant risks

The donor heart can be rejected or develop graft dysfunction, and immunosuppression increases infection risk. Long-term effects of anti-rejection medicines can also affect other organs and health conditions. The transplant team monitors for these problems over time.

LVAD risks

LVAD complications can include bleeding, blood clots, infection, stroke, and device malfunction. Because an LVAD may primarily support the left ventricle, the function of the right side of the heart also matters; failure of the unsupported side is a potential concern. Individual risk depends on the patient and treatment circumstances.

What do survival figures say—and not say?

OPTN/SRTR’s 2024 Annual Data Report, published in 2026, reports 91.2% one-year survival and 80.1% five-year survival among U.S. adult heart-transplant recipients transplanted in 2017–2019. These are outcomes for that defined transplant cohort, not an individual forecast and not a comparison with LVAD recipients.

NHLBI patient guidance gives an approximate 85% one-year survival figure after heart transplant and describes survival decreasing by roughly 3%–4% per subsequent year. That rounded patient-facing estimate and the registry’s cohort-specific figures come from different reporting contexts; they should not be combined into one curve.

OPTN/SRTR recorded 4,636 heart transplants in the United States in 2024 and reported that adult candidates awaiting transplant continued to outnumber transplants performed. Donor supply is one reason transplant access and timing cannot be assumed.

These transplant figures do not establish which treatment produces better survival for a particular person. A current, population-matched head-to-head estimate for transplant versus durable LVAD is not established here; separate statistics from different populations should not be used to infer superiority. LVAD outcomes vary with factors including device generation, patient acuity, treatment intent, and follow-up period.

How should a patient and care team make the decision?

There is no universal ranking. The 2024 International Society for Heart and Lung Transplantation guideline emphasizes individualized candidate evaluation and also addresses durable mechanical support. A team may weigh:

  • Whether the person meets transplant criteria and whether an LVAD is medically appropriate.
  • Heart anatomy and function, including right-heart function, and kidney and liver status.
  • Other health conditions and the person’s overall clinical situation.
  • Expected wait for a donor heart and whether an LVAD could serve as a bridge or destination therapy.
  • Whether the patient and caregiver can manage the treatment’s daily demands and emergency plan.
  • The patient’s priorities, life goals, and preferences about treatment and end-of-life care.

The American Heart Association’s advanced-heart-failure guidance, reviewed June 19, 2025, puts the values question plainly: “Your life goals and end-of-life preferences help decide what’s best for you.” A conversation with an advanced-heart-failure and transplant team can connect those priorities to the medical options and likely care demands.
