A workplace flu policy works when it makes staying home practical, tells employees when they can return, and gives managers a clear way to respond when someone becomes ill on shift. For most U.S. workplaces, use symptom improvement and fever status together—not a fever-only test—and provide flexible leave, a simple reporting route, and a separate rule for healthcare settings.

What should a workplace flu policy say?

Write a short, non-punitive policy that applies to flu-like and other respiratory symptoms. It should tell employees what to do when sick, how to report an absence, how leave and work coverage will be handled, and what must be true before they return. Identify which locations and employee groups are covered, and specify any separate rules for healthcare, laboratory, or other high-risk work.

For a general workplace, CDC’s guidance dated August 18, 2025 says people may resume normal activities when symptoms are improving overall and they have been fever-free for at least 24 hours without fever-reducing medication. Both conditions must be met. State this plainly in the policy and check whether a local or sector-specific requirement sets a different or stricter rule. OSHA’s seasonal-flu employer guidance uses a 100°F (37.8°C) fever threshold and advises workers with fever and respiratory symptoms to stay home until at least 24 hours after the fever ends without medication. How should employees decide whether to stay home?

Use respiratory symptoms as well as fever

Ask employees with respiratory symptoms that are not improving to stay home and contact their manager or HR. A fever check alone misses some flu cases. If the policy uses OSHA’s 100°F (37.8°C) threshold, explain that it is a fever trigger, not a way to rule out flu or contagiousness.

Be precise about flu without fever

CDC’s archived employer guidance, reviewed March 22, 2024, says people with suspected or confirmed flu but no fever should stay home for at least five days after symptoms begin. That is a flu-specific statement in archived guidance; do not silently turn it into a universal current rule. How can employers make staying home workable?

A policy cannot reduce exposure if employees feel they must work while sick. Explain the available leave routes, how to notify the workplace, and how to request telework or an alternate schedule when the job allows it. Name the manager or HR contact employees should use, and plan how essential tasks will be covered.

  • Use flexible, non-punitive sick leave practices that encourage sick employees to stay home; OSHA recommends this approach.
  • Allow telework or alternate schedules where duties can be done remotely.
  • Make the reporting process simple, including for employees caring for sick household members or children.
  • Tell managers not to pressure visibly ill employees to remain on shift.

CDC advises general employers to review and communicate policies before flu season. Its archived employer guidance also recommends annual review. Reissue the current policy before the season and revisit it when guidance or workplace conditions change. CDC archive: Stay Home When Sick

What should happen if someone gets sick at work?

  1. Report symptoms: Employees should notify a supervisor or the designated contact as soon as symptoms arise.
  2. Stop close-contact work: The employee should avoid close-contact tasks while arrangements are made to leave.
  3. Arrange prompt, low-contact departure: Managers should minimize contact with coworkers and follow the site’s procedures for leaving.
  4. Handle follow-up carefully: Share only necessary exposure information, and have HR address confidentiality questions under applicable law.

Why do healthcare workplaces need a separate rule?

Healthcare personnel have guidance beyond the general workplace return rule because their work involves patient care. CDC says healthcare personnel with fever and respiratory symptoms should be excluded from work until at least 24 hours after the fever has resolved without fever-reducing medication, and its healthcare guidance includes additional patient-care precautions. Keep these healthcare requirements distinct in the policy so that a rule intended for an ordinary workplace is not mistaken for sufficient guidance in a clinical setting. Employers in healthcare should follow the relevant CDC and facility protocols for their personnel and patient-care activities.

What should HR check before publishing?

The CDC and OSHA pages cited here provide public-health and workplace guidance, not a jurisdiction-by-jurisdiction legal review. Before issuing the policy, HR or counsel should check the rules that apply where employees work, including paid leave, accommodations, privacy, documentation, wages, and collective or other employment agreements. State clearly which internal contact handles questions or requests under those requirements.
