Capture the prompt, generated SQL, database context, execution identity, and exact error or result; then investigate the matching failure type. A query executing successfully is not proof that it is correct or safe.
Capture the failure before changing the agent
Keep a reproducible record of the exact natural-language request and generated SQL. Also record the database engine and version, the SQL dialect configured for the agent, the schema and descriptions supplied to it, the identity used to execute the query, and the exact error or result. For a wrong-result report, preserve an independently established expected answer or a small approved test case where possible.
This record helps distinguish a generation problem from an execution-context problem. A query may fail because it uses syntax the engine does not support, or because the execution identity lacks permission. Conversely, a query may run without error and still select the wrong records or calculate the wrong result. Microsoft cautions that generated responses can be incorrect, incomplete, or irrelevant, and may not meet user expectations.
Classify the symptom and follow the relevant path

For a parse or execution error
Check whether the SQL targets the engine and dialect the agent was actually given. The same task can require different syntax: Oracle documentation, for example, contrasts Oracle’s FETCH FIRST with SQLite’s LIMIT. Inspect identifier spelling and quoting, date and string expressions, function support, and type expectations. If the database returns an error, retain its exact text and any error code alongside the query; also check whether the execution identity has access to the referenced objects.

For a query that runs but is wrong
Trace the result back to the requested meaning. Verify that the agent chose the right tables and joined them on the right keys; that joins have not duplicated or dropped rows; and that filters, null handling, grouping, date ranges, ordering, and limits match the requested grain and scope. Then check whether a business term has a definition the agent can use. A schema can describe structure without explaining an organization’s metric or category.
Natural language that sounds clear to a person can still leave data meaning unresolved. Oracle gives “Show all employees who were born in CA” as an example request. The agent still needs the relevant schema and an unambiguous interpretation of “CA” in that database. If the intended meaning is not encoded in the data context, ask a clarifying question or provide a trusted view or tool that expresses it.

For unsafe access or mutation
Determine which identity executed the SQL and what that identity is allowed to do. Treat the ability to query data and the authority to modify it as separate requirements: an exploratory agent often needs only restricted, read-only access. If the task truly requires writes, constrain that capability through database permissions and trusted application controls rather than relying on the model to avoid dangerous statements.
For slow or unexpectedly expensive SQL
For SQL Server, inspect estimated or actual execution plans and Query Store evidence, then examine any identified query anti-patterns. Treat a suggested index, query rewrite, or schema change as a candidate for development or test. Do not apply a model-proposed change directly to production without validating its behavior and effects.

Repair schema, dialect, and business context
Before changing models or adding prompt rules, make the information available to the agent accurate and useful. Provide the current tables and columns, data types, primary and foreign keys, and relevant constraints. Add concise descriptions for fields whose names are overloaded or whose meaning depends on local business practice. Include a small number of representative question-to-query examples when they clarify expected joins, filters, or calculations.
Set the SQL dialect to the database engine that will execute the query. A mismatch can produce invalid syntax even when the prompt and schema are otherwise sound. Keep the schema context current as the database changes; stale names, types, or relationships can lead to missing identifiers or incorrect joins.

Do not expect table names alone to communicate every semantic relationship. Microsoft’s Agent Framework engineering article describes a case where schema-shape inference did not connect values such as “Diners” and “Ice Cream” to the broader concept “food.” If a request depends on a business definition, state the definition explicitly in the context, resolve it with the user, or expose an approved view or tool that implements it.

Review generated SQL before execution when the workflow permits it. For a reported wrong result, compare the output with a known answer derived independently from representative data. Include cases that exercise the relevant joins, filters, boundaries, and aggregation rather than checking only that the query returns rows.

- Joins: Confirm the join keys and relationship; check for duplicated or missing records.
- Filters and dates: Verify inclusivity, time boundaries, null behavior, and whether the filter matches the user’s intended scope.
- Aggregation: Check that the grouping level and calculation match the requested business measure.
- Ordering and limits: Confirm that sorting and row limits do not hide records needed to answer the question.
Some tools can attempt self-correction after a database execution error. Oracle documents that option for its SQL tool, but a correction that removes a syntax error does not establish that the query expresses the right business meaning. Use review and known-answer checks for semantic correctness.
Make database controls the security boundary
Prompts can guide behavior, but they should not be the control that protects records or prevents unauthorized operations. Microsoft’s SSMS Agent Mode documentation states, “Copilot’s approval system isn’t a security boundary.” The database permissions and trusted service layer must enforce what the agent can reach.
- Use a dedicated identity with least privilege. Grant only the permissions and objects needed for the agent’s task; prefer read-only access for exploratory queries.
- Restrict data at the database level. Use relevant views and, where appropriate, row- and column-level controls rather than exposing every table.
- Enforce tenant scope outside model control. In a multi-user system, do not give a generic SQL execution tool broad access and rely on the model to remember a tenant filter. Bind caller identity and row restrictions in trusted server-side logic or database policies.
- Do not build SQL by inserting raw user input. Use parameterized queries or a constrained query-building path in the application.
- Treat approvals as review, not authorization. An approval step can support human oversight, but the database and service permissions must still deny disallowed access or operations.
Google Cloud warns that generic SQL execution tools can create cross-user disclosure risks in multi-user systems when access depends on prompt instructions. Microsoft’s documentation likewise emphasizes least privilege and explains that SSMS Copilot uses the connected user’s permission context. Configure and verify those boundaries independently of the generated SQL.
Test fixes away from production
Change one cause at a time where practical: correct dialect or schema context, clarify a business definition, adjust permissions, or revise a query path. Re-run the preserved failure case and the relevant known-answer checks in a development or test environment. For performance work, inspect the plan and Query Store evidence and validate proposed code or schema changes there before considering production deployment. Microsoft’s Agent Mode guidance specifically recommends implementing proposed code or schema changes in test or development before production.
When assessing or selecting an agent implementation, compare the actual controls and observability rather than assuming that natural-language quality alone determines suitability. Check its supported engine and dialect, the quality and freshness of schema inputs, whether SQL is visible for review, how execution and mutation can be controlled, which identity runs queries, what errors and query history are available, and whether tenant and row/column restrictions are enforced outside model instructions.
