A gradual break-in schedule
Use the insoles in the shoes and socks you expect to wear on your hike. Keep the first sessions light and varied rather than starting with a long continuous walk. Guy’s and St Thomas’ NHS Foundation Trust advises wearing insoles for up to an hour on day one, then increasing by 30 minutes to an hour each day if they remain comfortable. It says many people can wear them all day comfortably after one to two weeks (Guy’s and St Thomas’ NHS Foundation Trust guidance).
Schedules differ: Dorset HealthCare University NHS Foundation Trust gives its patients an example of 30–60 minutes on day one, 1–3 hours on day two, 3–7 hours on day three, and 7 or more hours on day four. It also advises following the target set by the orthotist. This faster example is not a standard schedule for every hiker or every orthosis (Dorset HealthCare orthotics guidance).
- Day one: Wear the insoles at home for a short period, using up to an hour as a conservative example. Take them out sooner if rubbing or discomfort begins.
- Following days: If there are no sore or red areas, abrasions, or concerning discomfort, add roughly 30 minutes to an hour of wear per day. For prescribed devices, use the schedule from your orthotist instead of this general example.
- Before a hike: Build up with ordinary walks in the same footwear and socks you plan to use. The cited clinical guidance does not establish a hike-specific mileage progression or a deadline by which every pair will be ready.
Check the insole and shoe fit
A new insole occupies space inside a shoe and can change how your foot sits or contacts the footwear. Check that it lies flat and that your toes and heel have enough room; the shoe should not feel newly tight or force your foot to slide. A bulky original inlay may be removable in some shoes, while some feet need footwear with more depth or width. Do not trim or alter a prescribed orthosis without advice from the service that supplied it (Bexley Musculoskeletal Health orthotics guidance).

Insoles can be functional, providing support or reducing strain, or accommodative, cushioning the foot or easing pressure. The appropriate type depends on the foot problem and, for a prescribed device, clinical judgment (Dorset HealthCare orthotics guidance).
Know when to stop and inspect
Stop the activity as soon as you feel pain or discomfort or see skin turning red. Remove the footwear and inspect the area rather than walking on it to see whether it settles. Look for red marks, abrasions, blisters, swelling, discoloration, warmth, or broken skin. The American Academy of Dermatology Association recommends stopping immediately at pain, discomfort, or redness (AAD blister guidance).

A new device can feel unfamiliar, and mild early aching may occur as your body adapts; persistent discomfort, severe pain, or continued rubbing is a reason to stop and reassess the fit. If the insole is prescribed, contact the clinician or orthotics service for guidance rather than modifying or abandoning it on your own (Dorset HealthCare orthotics guidance).
If you have reduced sensation or circulation concerns, take particular care with skin checks and follow your healthcare provider’s foot-care advice, since you may not notice friction promptly.

Reduce friction while you adapt
Blisters are associated with friction and pressure. Stiff or poorly fitting footwear, moisture, and wrinkled socks can contribute, so an insole change is a good reason to recheck the whole shoe-and-sock fit rather than assuming the old fit still works (Royal College of Podiatry blister guidance).
- Wear socks that fit smoothly and manage moisture. Padded or double-layer walking or sports socks are an option, but they cannot correct a shoe that is too tight.
- Keep feet dry; for longer walks, carrying a dry spare pair is a practical precaution.
- At the first hot spot, stop, remove the footwear, and inspect the skin. Do not continue until a small rub becomes a painful blister.
- A securely applied soft bandage or moleskin may protect a known friction area. Treat it as protection, not as a reason to ignore pain, redness, or a poor fit.
Most ordinary blisters heal naturally; keep them clean and covered. Seek medical advice for increasing redness, pus, pain, or swelling, or if foot symptoms persist (AAD blister guidance; Royal College of Podiatry blister guidance).

Aftermarket insoles and prescribed orthoses are different cases
For a removable aftermarket hiking insole, judge the combination of foot comfort, stability, shoe volume, and whether the insert sits flat. Do not assume the insole itself will prevent blisters: footwear fit and friction still matter.
For a prescribed orthosis, the orthotist’s wear schedule, intended activity, footwear fit, and follow-up plan take precedence over generic advice. Contact the prescribing service if it rubs or remains uncomfortable, and do not trim, alter, or discontinue it without their guidance.
