Cellular satellite service adds a satellite link to a compatible ordinary phone through a participating mobile carrier; a satellite phone is a separate handset designed to connect to a satellite network. Carrier services commonly focus on messaging and selected apps, while a dedicated satellite phone such as Iridium Extreme supports satellite voice calling as well as messaging. Which one fits depends on the functions you need, your device and carrier, and where you plan to use it.
How the two services differ
Cellular satellite service—also called direct-to-device or direct-to-cell—connects a satellite to a supported everyday device through a participating mobile network. The US Spectrum Innovation Hub, an NTIA resource, defines direct-to-device technology as enabling communication between a satellite and a device such as a cell phone or laptop without specialized hardware or large ground antennas. A dedicated satellite phone is a purpose-built handset that connects to a satellite network. It is separate from your regular mobile service and requires compatible satellite service. United States: T-Mobile T-Satellite
T-Mobile describes texting and selected satellite-ready apps on compatible devices in outdoor areas where users can see the sky. It warns that data speeds are limited, performance varies, and gaps or time-outs may occur. Canada: Rogers Satellite
Rogers describes messaging and selected satellite-ready apps on supported phones and select smartwatches, along with text-to-911 and other satellite functions. United Kingdom: O2 Satellite
O2 says its service works alongside its mobile network on compatible smartphones without extra hardware or an app. It supports messages, location sharing, and selected apps, but not standard voice calls—including emergency calls—over O2 Satellite. O2 Satellite
Australia: Telstra Satellite Messaging
Telstra describes a Starlink Direct to Cell service that requires a compatible device, current software, an eligible plan, and outdoor visibility of the sky within its stated coverage area. When a dedicated satellite phone makes more sense
Consider a dedicated satellite phone if you need satellite voice calling rather than relying on a carrier service whose functions may be limited to messages or selected apps. Iridium describes the Extreme 9575 as a rugged satellite handset with voice calling, Short Burst Data, enhanced SMS and email messaging, integrated GPS and location messaging, and one-touch SOS. Iridium says its low Earth orbit network covers the Earth, including remote high-latitude areas. Network reach does not mean a handset will work in every location: line of sight, service arrangements, device condition, and local restrictions still matter. A dedicated handset also means carrying and maintaining another device and verifying its service arrangements. Check current coverage, plans, and rules for the places you intend to visit before relying on it.

Consider a satellite communicator if you do not need voice calls
A satellite communicator is a separate category, not a satellite phone. Garmin says its inReach communicators use the Iridium satellite network for two-way text messaging and interactive SOS beyond cell-tower range; an active satellite subscription is required.
