You can do a simple balance check at home by standing on one leg beside a sturdy chair or counter and timing the hold only while you feel safe. For a structured assessment, CDC STEADI uses four progressively harder stances held for up to 10 seconds each. These are different checks with different purposes: neither is a diagnosis, and support and safety matter more than a high score.

Choose the right home balance check

NHS inform describes a one-leg stand as a way to help choose a starting level for its balance exercises. Its guide uses a 30-second hold for that purpose. CDC STEADI’s 4-Stage Balance Test is a separate, structured clinical assessment intended to screen older adults for fall risk; it progresses through four stances, each attempted for 10 seconds.

Do not combine the NHS 30-second exercise-level check and the CDC 10-second stages into a single scale: they are designed for different purposes.

Set up safely before you begin

  • Choose a clear, uncluttered area beside a fixed counter or sturdy chair that cannot slide. Stay close enough to touch it; a wall or stable chair should be within reach.
  • Wear comfortable, flat shoes. Do not exercise when you are unwell. If you have been inactive and this is your first attempt, consider having someone nearby.
  • Keep a hand on the support if needed. Reduce support only when you feel confident; do not attempt an unsupported or harder position just to complete a test.
  • The CDC clinical protocol specifies eyes open and no assistive device. That instruction is for its assessment setting, not a reason to remove a support you need during an unsupervised home check.

How to do the one-leg stand

  1. Stand beside your chosen support, with the chair or counter close enough to hold immediately.
  2. Begin with a hand on the support. If you feel steady and confident, release it; keep your hand close enough to grab the support.
  3. Lift one foot and time how long you can hold the position comfortably. Stop if you feel unsafe, need to grab the support, or lose balance. Do not force a 30-second hold.
  4. If you are using the NHS inform exercise-level check, its guide uses 30 seconds to help select an exercise level. Record your result, then repeat on the other side under the same conditions if safe.

The timed check is not the same as an exercise prescription. NHS balance-exercise guidance suggests holding a supported one-leg exercise for 5 to 10 seconds, doing three repetitions on each side, and building repetitions gradually. It recommends balance exercises at least twice weekly; it does not call for daily maximal-time testing. See NHS balance exercises.

How to follow the CDC four-stage sequence

The CDC STEADI protocol is a structured assessment. In a supervised clinical setting it specifies eyes open, no assistive device, and a stopwatch. At home, do not follow the no-device instruction if you need support to stay safe. Stop the progression whenever you cannot maintain a stance or feel unsafe.

  1. Feet side by side: Stand with your feet together.
  2. Semi-tandem: Place the instep of one foot so it touches the big toe of the other foot.
  3. Tandem: Place one foot directly in front of the other, heel touching toe.
  4. One-foot stand: Stand on one foot.

Attempt each position for 10 seconds. If you cannot maintain a position for that long, stop rather than progressing to a harder stance. The protocol’s “If not, STOP” instruction is a safety rule, not a challenge to push through.

What the CDC result means—and does not mean

In the CDC STEADI protocol, an older adult who cannot hold the tandem stance for at least 10 seconds is considered at increased fall risk. That finding is a screening signal, not a diagnosis, and it is not a universal benchmark for every age or fitness level. CDC guidance says older adults who cannot maintain tandem stance for at least 10 seconds may be considered for physical therapy or an evidence-based fall-prevention program such as Tai Chi.

Track progress in a consistent way

For a useful comparison, repeat the same test under similar conditions. Record the details below; this is a practical tracking method, not a formally validated score.

There is no validated home retest schedule or minimum meaningful improvement established by these timed protocols. Focus on whether the same stance becomes steadier under comparable conditions rather than treating a small time change as proof of medical improvement.

When to get professional advice

Ask a clinician for advice if you are concerned about your balance, have a history of falls, or have a condition that affects balance. NHS guidance advises contacting a GP if you are worried about balance or mobility, or after a fall. See NHS falls guidance. A stopwatch is listed as equipment in the CDC protocol, but it is optional for a basic home check; a phone timer or watch can time a hold. The NHS exercise guidance does not require specialist equipment. CDC STEADI clinical materials are available at CDC STEADI Clinical Resources.
