Bugs pass code review because review is a limited human examination of a change, not proof that the change is correct. Reviewers can lack context, face an oversized diff, focus on visible polish, miss behavioral edge cases, trust weak tests, or overlook risks that need specialist knowledge. Teams can reduce those risks with smaller changes, clearer intent, deliberate behavioral and test review, and qualified reviewers—but no checklist or approval gate catches every defect.
Why code review does not guarantee bug-free code
A reviewer sees a change from the outside, often without the author’s full understanding of the problem, system history, or intended user workflow. Approval means the change was judged acceptable under the review conditions; it is not a formal proof of correctness. The available studies do not establish a universal percentage of bugs that escape review.
A 2018 Google case study combined 12 interviews, a survey with 44 respondents, and analysis of review logs covering 9 million changes. Those figures describe the study’s methods and scale, not its bug-detection rate: Google Research’s study of code review at Google.
Common reasons bugs pass review
The reviewer lacks the author’s context
A changed line can look reasonable in isolation and still break a surrounding module, workflow, or assumption. Reviewing only the visible diff makes that harder to notice. Google’s engineering guidance recommends considering the broader file and system context, thinking like a user, and asking for clarification when code is difficult to understand: Google’s guide to what reviewers should look for.

Large changes overload attention
As a change grows, reviewers have more behavior and interaction to hold in mind. Google’s author guidance says large changes can lead to extensive back-and-forth and frustration, sometimes causing important points to be missed or dropped. It also recommends making changes small and self-contained where practical: Google’s guidance on keeping changes small. This is practitioner guidance, not a controlled estimate of how many extra defects large reviews cause.
Review drifts toward visible polish
Naming, formatting, and style are easy to spot in a diff. Behavioral failures may depend on an unusual input, ordering, state transition, or interaction outside the changed lines. Google’s review standard prioritizes design and functionality and cautions against blocking changes over personal style preferences: Google’s code review standard.
Tests exist but do not challenge the faulty behavior
A test may cover a happy path while missing the condition that breaks production behavior. The meaningful question is not just whether tests were added, but whether they would fail if the implementation were wrong and whether changes to the code could make them pass falsely. Google’s guidance puts it plainly: “Tests do not test themselves, and we rarely write tests for our tests—a human must ensure that tests are valid.” See Google’s reviewer guidance on code and tests.

Concurrency and specialist risks need focused expertise
Race conditions, deadlocks, privacy issues, and security flaws can be difficult to identify through a quick read or by simply running the program. A reviewer may need deeper domain knowledge or time to reason through execution order, permissions, and data flows. Google recommends qualified review for complex topics such as concurrency, privacy, and security.

Security is not always an explicit review goal
A 2023 study examined 20,995 keyword-selected review comments across four OpenStack and Qt projects; authors manually classified 614 as security-related. The study reported that security defects were not prevalent in the review discussions it analyzed and identified “Not worth fixing the defect now” and disagreement between developer and reviewer as common reasons security defects were not resolved. These selected projects and comments do not measure security-review effectiveness everywhere: the OpenStack and Qt security-review study.
In a separate 2022 online experiment with 150 participants, explicitly asking reviewers to focus on security increased the probability of vulnerability detection eightfold in that experiment. The security checklist tested did not significantly improve the result further. This is a study-specific outcome, not a guaranteed production effect: “Less is More”.

How to make reviews more likely to catch defects
1. Keep each change small and self-contained
Split unrelated work into separate changes where practical. Include the relevant tests and enough context in the description for a reviewer to understand the problem and intended outcome. Smaller changes reduce the amount of behavior that must be considered at once; they do not eliminate the need for careful review.
2. Explain intent and risk
In the review description, state what the change is meant to do, who or what it affects, important assumptions, and the behaviors most likely to fail. This gives reviewers a concrete basis for challenging the implementation instead of guessing at its purpose.

Review every assigned human-written line, then inspect relevant surrounding code and system behavior. Trace how the change interacts with callers, stored state, and user workflows. If the implementation is hard to follow, request clarification rather than approving an unclear change.

4. Walk through failure-prone behavior
Match the review to the risks of the change. Consider the relevant cases rather than mechanically checking every category:
- Boundary values and unusual or invalid inputs.
- State transitions, retries, and error paths.
- Ordering, permissions, and user-visible outcomes.
- Concurrency, shared state, and interactions with other components.
5. Review what tests prove
Inspect assertions and coverage, not just the test count. Ask whether a test would fail for the likely defect, whether it covers the important branch or boundary, and whether an accidental implementation change could still leave the test green.
6. Match reviewers and checks to the risk
Bring in qualified reviewers for security, privacy, concurrency, accessibility, or other specialist concerns. Use automated tests and static analysis as complementary evidence, not substitutes for understanding the change. A study of security review recommends combining manual review with automated detection for broader coverage; neither establishes that any one combination catches every flaw.

7. Balance speed with code health
Review depth should reflect the consequences and complexity of a change. Google’s review standard recognizes that time constraints can lead teams to take shortcuts while also cautioning against demanding perfection for every change. The aim is a sound, understandable change—not comments on every personal preference.

Different studies measure different things, so their numbers should not be treated as a general bug-escape rate. For example, a 2023 mutation study analyzed 633 merge requests and 78,000 mutants; it reported that 38% of all mutants and 60% of productive mutants were resolved through code changes or test additions. Those are mutant outcomes in that dataset, not escaped production bugs: “Please fix this mutant”.
Likewise, a paper titled “Code Reviews Do Not Find Bugs. How the Current Code Review Best Practice Slows Us Down” presents its authors’ argument about review practice, not a settled universal conclusion: Microsoft Research’s paper page. Review remains useful as one layer of quality work, but the cited work does not support a universal miss rate or a guarantee that adding reviewers, a checklist, or an approval gate will catch every defect.
