To audit an AI agent, keep a protected, chronological record that connects each consequential action to the request that triggered it, the human and agent identities involved, the authority and policy applied, the evidence consulted, any approval, and the action’s actual outcome. A transcript or tool-call list alone cannot reliably show why an action happened, whether it was authorized, or what changed.
What an agent audit trail needs to show
NIST’s glossary defines an audit trail as a chronological record of system activity that is sufficient to reconstruct and examine events surrounding or leading to a security-relevant transaction, from its inception to its results. For an agent, that means recording more than the command it issued: preserve the context and authority needed to explain the command, and the result needed to verify what happened.
A reviewer should be able to follow a run from its trigger through proposal, policy decision, approval or denial, execution, and any resulting state change. The record should connect the relevant human principal, agent identity, delegation or workflow, tool and target, supporting evidence, and outcome. NIST’s summary of comments on agent identity notes that conventional logs may not explain why an action occurred, what authority governed it, what information influenced it, or whether it was approved.
There is not yet one settled agent-audit standard. NIST’s identity work began as a concept-paper effort and its agent-evaluation work is ongoing; neither should be treated as a finalized requirement. The practical record design below is a synthesis of the guidance, not a mandatory NIST schema.

Model the operation as connected events
Use distinct events rather than overwriting one record as an operation progresses. Stable identifiers and correlation data let an auditor join activity across the agent runtime, policy service, approval workflow, tool, and target system.

Capture read, propose, execute, deny, fail, approve, retry, and rollback events where they apply. Failed and blocked attempts are evidence too: they help show whether a control worked. NIST SP 800-12 explains that application-level logs can record data opened and closed and specific read, edit, or delete actions; parameters can make a record more useful than a bare command name.
Build the record in seven steps
1. Define event scope and risk levels
Inventory the agent’s possible actions, tools, target systems, and data classes. Decide which event types to capture and how much detail is justified by the sensitivity of the data and the impact of the action. Include denied and failed attempts as well as successful ones. A low-risk read and a high-impact external change may need different approval, monitoring, and evidence detail.
2. Assign stable identity and correlation context
Give each event and run a stable identifier and timestamp. Preserve the initiating human or system principal, agent or service identity, parent-child delegation lineage, workflow correlation IDs, tool identity, and target resource. Record agent, model, runtime, and policy versions when available so a reviewer can identify the configuration in effect.

NIST’s summary of public comments records suggestions for metadata such as model version and provenance, operating policy, parent-agent lineage, and the human principal authorizing deployment. Those suggestions reflect community input, not an adopted NIST schema or normative requirement.
3. Separate proposal, authorization, and execution
Record what the agent proposed, what the policy or authorization layer decided, and what the tool actually executed as separate facts. Preserve the trigger or request reference, normalized arguments and target, risk classification, authorization result, approval identity and scope, and final outcome. Do not infer successful execution from a proposal or approval event.
For high-impact actions, keep the agent’s proposal separate from execution. OWASP’s AI Agent Security Cheat Sheet recommends an independent policy or execution component that validates scope, privilege, and approval state. Bind approval to the precise actor, tool, target, parameters, time, and expiry; a general “approved” flag should not authorize a materially different action. Fail closed if the approval check, policy lookup, risk classification, or audit logging fails.
4. Link claims to their supporting evidence
When an agent makes a factual claim or bases a consequential decision on retrieved material, preserve a reference that allows a reviewer to locate the relevant source again. Include useful retrieval or version context, subject to privacy and data-protection rules. A record that says only “document consulted” is weak if no one can determine which version or passage informed the decision.

NIST’s agent-evaluation project describes machine-readable audit trails that map decisions to supporting documents and probes for faithfulness, completeness, and sufficiency: whether a source supports a claim, whether the agent captured the source’s full message, and whether the source meets the evidentiary burden. The project describes probes during a workflow and after completion as research approaches, not requirements for every deployment.
5. Keep records beyond the agent’s unilateral control
Restrict who can write, alter, delete, and read audit records; an agent or ordinary operator should not be able to quietly rewrite the history used to assess its own actions. Depending on the system’s needs, consider append-only or write-once storage, digital signatures, secure timestamps, independent monitoring, and protected exports. NIST SP 800-12 discusses access controls, signatures that can alert reviewers to alteration, and write-once media as integrity measures. These mechanisms still require suitable access controls and operational design.
6. Protect sensitive content and set retention deliberately
Audit data can expose identities, prompts, retrieved information, tool parameters, or confidential outputs. Limit access, encrypt sensitive records where appropriate, and avoid retaining content that is not needed for reconstruction. Where possible, use protected references or redaction without removing the information required to review what occurred.
Set retention and review periods according to the deployment’s sensitivity, incident-response needs, organizational policy, and applicable obligations. NIST SP 800-12 does not establish a universal retention period; NIST’s agent-identity comment summary also records concerns about overcollection, leakage, and profiling. Legal and regulatory retention requirements depend on the system, data, and jurisdiction.

7. Make records searchable and rehearse reconstruction
Provide search and export by agent, principal, run, tool, resource, time, policy, and outcome. Review high-risk events promptly where appropriate, and review broader patterns periodically. NIST SP 800-12 stresses that logs have limited value without timely review.
Test the evidence trail with a sample operation and an independent reviewer. The reviewer should be able to identify what started it, which identities and authority applied, what evidence was consulted, what the agent attempted, whether an approval or denial occurred, what actually executed, and what changed. Record gaps as control failures to address, not as reasons to rely on the agent’s narrative.

A record design can use fields such as these, adjusted to the system and data involved:
- Correlation: event ID, run ID, parent-event ID, timestamp, and workflow correlation ID.
- Identity: initiating human or system principal, agent or service identity, and delegation lineage.
- Configuration: agent, model, runtime, and policy version where available.
- Lifecycle: event type, such as proposed, approved, denied, executed, failed, retried, or rolled back.
- Action: tool or API, target resource, normalized parameters, and requested scope.
- Decision: risk classification, authorization and policy result, decision source, and approval bound to the exact action, with time and expiry.
- Evidence: references to relevant retrieved documents or sources supporting factual claims.
- Outcome: execution result, affected object or state-change reference, and independent verification where available.
- Protection: integrity metadata, storage location, retention class, and access history.
This is a record-design checklist, not a reason to copy secrets or unnecessary personal data into logs. Minimize content while preserving the references and context a reviewer needs.

How to evaluate audit and observability tools
Whether you instrument an agent in-house or evaluate a platform or service, check what it can demonstrate rather than relying on a dashboard’s event count.
- Lifecycle coverage: Does it capture triggers, tool calls, results, errors, denials, approvals, retries, and rollbacks?
- Context and attribution: Can records connect actions to human and agent identity, delegation, policy, version, and target resource?
- Evidence linkage: Can reviewers trace a claim or decision to source material and retrieve relevant versions?
- Independent enforcement: Are permissions and approvals checked outside the agent’s own decision process?
- Integrity and access: Can the agent or an ordinary operator alter or delete records? Are access and integrity controls, and export, supported?
- Review and portability: Can incident-response and audit teams search, retain, and export records in a usable form?
- Privacy and operating cost: Can sensitive data be minimized and retention configured without losing reconstruction evidence?
NIST’s evaluation work informs evidence-linking and evaluation questions; OWASP informs action controls and approval checks. The cited guidance does not benchmark vendors or endorse products.
What current NIST work does—and does not—establish
NIST’s “Building Evaluation Probes into Agentic AI” page was created May 1, 2026, updated May 5, 2026, and marked the project ongoing. It describes early research and an open-source testbed, not a completed standard. The page states the goal is to move beyond “the AI said so” toward understanding what it found, where it found it, and how evidence supports conclusions.
On February 5, 2026, NIST announced a concept paper on agent identity and authority and sought public input on identification, authorization, auditing, non-repudiation, and prompt-injection controls. The comment deadline was April 2, 2026, and has passed. The announcement and summary of comments document a project and respondent suggestions; they do not create a finalized agent-audit requirement.
