A useful hospital or clinic emergency preparedness checklist covers four connected areas: risk assessment and emergency planning, policies and procedures, communication, and training and testing. CMS requirements vary by provider and supplier category, so this general checklist is a planning aid—not a determination of compliance. Identify your exact CMS certification category, then confirm the applicable federal, state, local, accreditation, and facility-specific requirements.
1. Confirm the facility’s scope and who is responsible
Start by establishing which rules apply to each site. A hospital’s requirements are not automatically the requirements for an outpatient clinic, even when both belong to the same health system. CMS describes variation by provider type and by characteristics such as inpatient versus outpatient care.
- Identify the category: Record each location’s CMS certification or provider category, services, and applicable emergency preparedness rule. Check state and local requirements as well.
- Assign accountable leads: Name an executive sponsor, emergency management lead, incident command roles, alternates, and an operational contact for each critical function.
- Define system and site responsibilities: Specify how separately certified locations, clinics, departments, and system leadership participate, who can activate the plan, and how decisions are escalated.
- Control the plan: Set an approval and review process, version history, and distribution method. Staff should be able to find the current plan if normal IT systems are unavailable; maintain an accessible backup format where appropriate.
CMS’s provider guidance describes a common all-hazards framework, but the precise rule and cadence depend on the facility’s category. A shared health-system plan can support coordination, but it should not obscure site-specific responsibilities or requirements.
2. Assess hazards and plan for continuity
Document an all-hazards risk assessment that connects local threats to the services, patients, buildings, and infrastructure the facility depends on. CMS examples include care-related emergencies, equipment or power failure, communications interruption—including cyberattacks—and loss of part or all of a facility or its supplies.

Use local risks, not a generic hazard list
Consider natural hazards; infectious disease and other public health events; mass-casualty incidents and patient surges; utility, equipment, oxygen, supply-chain, and communications failures; cyber disruption; building damage; and loss of a site or service. For each priority hazard, record the assumptions, existing controls, remaining risks, actions to reduce them, and dependencies that could interrupt care.
Set decision points before an incident
For each relevant scenario, define who can activate or escalate the response and what conditions prompt shelter-in-place, service restriction, relocation, evacuation, transfer, or recovery. Identify how clinical leaders and support departments coordinate decisions, and how the facility will maintain essential services while normal operations are disrupted.
Plan around the patients and services actually present
Map the patient groups and care functions that could need additional support. Depending on the facility, this may include people who rely on electricity, oxygen, medications, refrigeration, dialysis, mobility assistance, accessible communication, or other specialized care. These are planning prompts to tailor to the local population and services, not a universal patient inventory prescribed by CMS’s core-elements overview.

3. Turn policies and procedures into usable actions
Policies and procedures should translate the risk assessment and emergency plan into steps that staff can carry out. Tailor them to the applicable provider rule and relevant law; CMS says these components must comply with applicable federal, state, and local requirements.
- State who makes operational and clinical decisions, how leaders are contacted, and how incident management is organized.
- Describe how departments coordinate, including clinical care, facilities, IT, security, communications, and supply functions as relevant to the site.
- Set procedures for patient tracking, continuity of care and records, transfers, evacuation or relocation, and family inquiries.
- Address staffing contingencies, vendor or supply disruption, and how essential services will be restored.
- Specify where staff find role-specific instructions and what they should do if usual communication or information systems are unavailable.
For every procedure, make responsibilities, activation conditions, and escalation paths clear enough that staff can act without relying on an unavailable system or a single unavailable decision-maker.
4. Build communication and coordination that can survive disruption
The communication plan should work inside the facility and with outside partners. CMS’s core framework includes coordination with other healthcare providers, public health agencies, and emergency management officials.

- Maintain current contact paths for employees, volunteers, contracted services, physicians, leadership, emergency responders, public health, emergency management, receiving facilities, and other relevant partners.
- Identify backup methods for internal and external communications when normal phone, network, or messaging services fail, including during a cyber incident.
- Define how the facility will send and receive operational status, resource needs, and offers of assistance through the appropriate authorities or coordination channels.
- Set a process for sharing relevant patient information consistent with applicable privacy and other laws.
- Make messages and contact procedures usable by people with language, sensory, or functional needs.
- Test message routing, contact-list accuracy, and acknowledgement—not just whether a message was sent.
Where relevant, coordinate with the local healthcare coalition. ASPR’s Hospital Preparedness Program implementation guide for 2017–2022 describes coalition planning concepts such as information sharing, resource coordination, plan review, and exercises among participating organizations; it is historical guidance, not evidence of current funding or performance measures.

Train employees, volunteers, and people providing services under arrangement according to their expected roles. Keep records, make sure participants understand what to do, and provide updated instruction when significant policy changes affect their work.
Apply the hospital rule only to hospitals it covers
For hospitals subject to 42 CFR § 482.15, the 2024 annual compilation of the federal regulation specifies initial training, recurring training at least every two years, documentation, demonstrated knowledge, and training after significant policy updates. It also requires plan testing at least twice a year. The rule calls for an annual community-based full-scale exercise or, if that is not accessible, an annual individual facility-based functional exercise. Consult the current regulatory text for other testing options and details.

Make exercises reveal operational gaps
Include the clinical and nonclinical teams needed for the scenario. Exercise command and escalation, internal and external communications, patient movement, resource status, continuity of care, and coordination with outside partners. After each exercise or real response, record the objectives, participants, observations, corrective actions, assigned owners, and deadlines; then verify that fixes were checked in a later exercise or review.
CMS’s core-elements page says plans are reviewed and updated at least annually, while CMS provider guidance emphasizes that requirements vary. Confirm the review and exercise schedule for the exact provider category instead of applying the hospital cadence to every clinic.

6. Track facility status and operational resources
A written plan does not show whether a facility can operate during a disruption. Maintain a current status view, name the person responsible for updating it, and define local minimum operating levels and replenishment or escalation triggers. HHS’s All-Hazards Plan identifies status topics; it does not prescribe universal quantities, staffing ratios, or preparedness targets.

Use the status categories to support a site-specific risk and continuity plan, not as a generic shopping list. Determine quantities and operational thresholds locally based on hazards, services, patient needs, and resupply conditions.
Hospital and clinic requirements are not interchangeable

The federal regulation cited here is the 2024 annual CFR compilation, and CMS’s online rule page notes revisions in 2019. Before relying on a cadence or procedure for compliance, check the current e-CFR, CMS interpretive guidance, the facility’s certification category, state and local rules, accreditation requirements, and facility policy.
