For weight management in the United States, alternatives to Ozempic include Wegovy, Zepbound, Saxenda, Qsymia, Contrave and orlistat products such as Xenical and over-the-counter alli. Their side effects differ by medicine: nausea and other digestive symptoms are common with incretin medicines, orlistat has characteristic digestive effects, and Qsymia and Contrave have distinct non-digestive risks. There is no reliable universal ranking of which has the fewest side effects.

First, what is the difference between Ozempic and Wegovy?

Both contain semaglutide, but they are different branded products with different approved indications. Ozempic is a type 2 diabetes medicine; Wegovy is the semaglutide product approved for chronic weight management. They should not be treated as interchangeable simply because they contain the same active ingredient. The National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) lists weight-management medicines as additions to reduced-calorie eating and increased physical activity, not replacements for them.

What medicines are alternatives to Ozempic for weight loss?

In the United States, FDA-approved options for chronic weight management include the following medicines. Approval and eligibility depend on the product, a person’s age and clinical circumstances, so this list is not a recommendation that every option is suitable for every reader.

These medicines differ in route as well as side-effect profile. The table summarizes patterns, not a ranking: an adverse effect that is manageable for one person may be a major concern for another.

How do their side effects compare in practice?

Wegovy, Zepbound and Saxenda: digestive effects and product-specific cautions

Nausea, diarrhea, vomiting, constipation and abdominal discomfort are prominent common effects of these incretin-based medicines. The Wegovy label also lists headache, fatigue and dizziness among common reactions. Their labels include a warning related to thyroid C-cell tumors observed in rodents; whether that finding applies to humans is unknown. Check the current label for the specific product rather than assuming that warnings are identical across medicines in this group.

FDA has reported finding no increased risk of suicidal behavior or ideation with the GLP-1 receptor agonists it evaluated and requested removal of that warning from the Saxenda, Wegovy and Zepbound labels. Older material may still repeat the warning, so use current product labeling when discussing risks.

Qsymia: pregnancy, mood, sleep and cognitive considerations

Along with effects such as constipation, tingling, insomnia and dry mouth, Qsymia prescribing information warns about mood and sleep disorders, cognitive effects, metabolic acidosis and changes in renal function. Qsymia is contraindicated during pregnancy because of fetal risk. Pregnancy plans or the possibility of pregnancy are therefore important to discuss with a clinician before considering it.

Contrave: blood pressure, seizure and opioid considerations

Contrave can raise blood pressure and heart rate, and seizure risk is an important screening consideration. Opioid use or treatment also matters when assessing whether it is appropriate. A clinician needs to review these factors and the person’s other medicines and health history before prescribing it.

Orlistat: digestive effects and kidney warnings

Orlistat’s characteristic side effects are gastrointestinal, including abdominal discomfort, flatulence and fecal urgency. FDA added a warning to the over-the-counter alli label about rare risks of acute kidney injury and kidney stones. Kidney concerns should be raised with a clinician or pharmacist when considering orlistat.

Can you identify which medicine has the fewest side effects?

Not from side-effect percentages reported in separate studies. The American Diabetes Association’s Standards of Care in Diabetes—2025 explains that clinical trials vary in their conditions, so adverse-reaction rates from one drug’s trials cannot be directly compared with rates from another drug’s trials. That means a lower percentage in one study does not establish that its medicine is safer or easier to tolerate than another.

A useful comparison is instead whether a medicine’s known effects and cautions fit a person’s health history, preferences and treatment needs. Individual tolerability varies, and a clinician can help interpret current product-specific labeling.

  • Medical history: Ask how relevant conditions, including kidney concerns, affect the options being considered.
  • Pregnancy: Discuss pregnancy or plans for pregnancy; Qsymia is contraindicated during pregnancy.
  • Seizure history, opioid use and blood pressure: These are especially important when evaluating Contrave.

NIDDK and FDA describe these medicines as being used alongside reduced-calorie eating and increased activity. Continuing treatment depends on benefit and tolerability; a clinician can advise what to do if effects are difficult to manage.

Are compounded GLP-1 medicines equivalent alternatives?

FDA warns about risks associated with unapproved compounded GLP-1 products, including dosing errors and administration concerns. Do not assume that a compounded product has the same FDA review or status as an FDA-approved medicine, or that it is interchangeable with one. Discuss questions about a prescribed product with the clinician who manages your care.
